Bakgrund: Kommunikation utgör en central del av palliativ vård och har stor betydelse för hur patienter upplever trygghet, delaktighet och värdighet i livets slutskede. För sjuksköterskor innebär dessa samtal ett betydande ansvar, då kommunikationen behöver anpassas efter patientens individuella behov, hälsotillstånd och känslomässig situation. Trots kommunikationens centrala betydelse inom palliativ vård är kunskapen om sjuksköterskors erfarenheter av kommunikation i livets slutskede fortfarande begränsad. Ökad kunskap inom området kan bidra till förbättrad kommunikativ kompetens och stärka möjligheten att erbjuda personcentrerad och trygg palliativ vård. Syfte: Syftet var att undersöka sjuksköterskors erfarenheter av kommunikation med patienter i livets slutskede inom palliativ vård. Metod: Studien genomfördes som en kvalitativ litteraturöversikt med induktiv innehållsanalys. Datainsamlingen genomfördes i databaserna CINAHL, PubMed, PSycInfo och MEDLINE. Vetenskapliga artiklar publicerade mellan 2015 och 2026 inkluderades. Artiklarna kvalitetsgranskades med hjälp av en granskningsmall och analyserades enligt Fribergs femstegsanalysmodell. Resultat: Resultatet belyser både kommunikationens betydelse i livets slutskede och de utmaningar som sjuksköterskor möter i kommunikationen med patienter inom palliativ vård. Kommunikationens betydelse omfattade relation och trygghet, anpassning av kommunikation samt erfarenhetens betydelse. Utmaningar i kommunikationen omfattade osäkerhet och brist på kunskap, organisatoriska hinder samt emotionell belastning. Slutsats: Slutsatsen visade att kommunikation i livets slutskede var både central och komplex. Det finns ett behov av ökad utbildning samt organisatoriskt stöd för att stärka sjuksköterskors kompetens och trygghet i kommunikationen.

Background: Communication is a central part of palliative care and influences how patients experience safety, participation, and dignity at the end of life. For nurses, these conversations involve considerable responsibility, as communication must be adapted to the patient’s individual needs, health condition, and emotional situation. Despite its importance, knowledge of nurses’ experiences of communication in end-of-life care remains limited. Increased knowledge in this area may strengthen communicative competence to support person-centred and safe palliative care. Aim: The aim of the study was to explore nurses’ experiences of communication with patients at the end of life in palliative care. Method: The study was conducted as a qualitative literature review with an inductive content analysis. Data collection was performed in the databases CINAHL, PubMed, PsycInfo, and MEDLINE. Scientific articles published between 2015 and 2026 were included. The articles were quality appraised using a review template and analysed according to Friberg’s five-step analysis model. Results: The findings highlight both the importance of communication in end-of-life care and the challenges nurses experience when communicating with patients in palliative care. The importance of communication included relationship and security, adaptation of communication, and the significance of experience. Challenges in communication included uncertainty and lack of knowledge, organizational barriers, and emotional burden. Conclusion: The conclusion indicates that communication at the end of life is both central and complex. There is a need for increased education as well as organizational support to strengthen nurses’ competence and confidence in communication.
