Abstract [sv]

Introduktion: Andelen äldre i samhället ökar, med stigande ålder blir behoven av vård och rehabilitering större. Det pågår en omställning till god och nära vård där vården kan ske mer i hemmet. I den kommunala hälso- och sjukvården arbetar man ofta i team där arbetsterapeuter och fysioterapeuter ansvarar för rehabiliteringen. Det finns begränsad kunskap om hur insatserna utförs vid rehabilitering i hemmet.Syfte: Syftet med studien var att utforska rehabiliteringspersonalens utförande av insatser vid teambaserad rehabilitering av äldre personer i hemmet.Metod: Kvalitativ metod med induktiv ansats. 8 observationer av gemensamma besök, genomförda av 6 arbetsterapeuter och 5 fysioterapeuter som intervjuats efter besöken. Data är från Re@home projektets fas 2.  Materialet har analyserats med kvalitativ innehållsanalys.Resultat: Ur resultatet framkom tre kategorier: 1) Professionell kunskap, 2) Samarbete och samverkan, 3) Organisationen. Konklusion: Resultatet i studien visar att rehabiliteringspersonal till stor del arbetar med ett personcentrerat arbetssätt genom att använda sin kunskap och flexibelt anpassa insatserna utifrån patientens situation. Det fanns ett unikt teamarbete i hemmet, där personal, patient och anhöriga samarbetade praktiskt men även vid planering av insatserna. Rehabilitering i hemmet kan skapa förutsättningar för det personcentrerade partnerskapet där anhöriga och patient är en del i teamet.

Abstract [en]

Introduction: The proportion of elderly people in society is increasing, with increasing age the need for care and rehabilitation becomes greater. The rehabilitation is transferred to take place more at home. Usually, occupational therapists and physiotherapists work in teams and are often responsible for rehabilitation. There is limited knowledge how the interventions are executed in home rehabilitation. Aim: The purpose of the study was to explore the rehabilitation professionals’ performance of interventions in team-based rehabilitation of older people at home.Method: Qualitative method with inductive approach. 8 observations of visits were conducted with 6 occupational therapists and 5 physiotherapists who were interviewed after the visits. Data is from Re@home project phase 2. The material has been analysed with qualitative content analysis.Results: The results revealed three categories: 1) Professional knowledge, 2) collaboration and cooperation, 3) Organization. Conclusions: The study shows that rehabilitation professionals largely work with a person-centered approach by flexibly adapting their interventions based on the patient’s situation.  There was a unique teamwork in the home, where professionals, patients and relatives collaborated practically but also in planning the interventions. Rehabilitation at home can create conditions for the person-centred partnership, where relatives and patient are a part of the team.
