Bakgrund: Globalt behöver 56,8 miljoner människor palliativ vård varje år. Sjuksköterskan har ett ansvar för att ge en god palliativ vård i hemmet. En utmaning för sjuksköterskan är att lindra lidande, skapa en trygg vårdrelation och samtidigt främja välbefinnande under patientens sista tid i livet. Syfte: Beskriva sjuksköterskors upplevelser av palliativ vård inom hemsjukvården. Metod: En litteraturöversikt som inkluderade kvalitativa studier och analyserades med induktiv ansats baserad på 12 vetenskapliga artiklar. Artiklarna analyserades utifrån de fem steg som beskrivs av Friberg (Friberg, 2023). Artiklarna som inkluderades identifierades i databaserna CINAHL och PSYCHINFO. Resultat: I resultatet framkom två teman och sex subteman. Sjuksköterskors upplevelser av palliativ vård i hemsjukvården visar att de blir känslomässigt påverkade både privat och på ett professionellt plan. Känslor av att inte räcka till, tidspress, hög arbetsbelastning och etiska utmaningar bidrar till att sjuksköterskorna behöver strategier för att klara av att vara professionella i sitt arbete. Slutsats: Sjuksköterskor möter flera utmaningar inom palliativ vård i hemsjukvården vilka hanteras med stöd av olika copingstrategier, där humor nämns som en vanligt förekommande strategi. Sjuksköterskors förståelse för användning av copingstrategier kan bidra till samhället genom att ge goda förutsättningar för en god omvårdnad inom hemsjukvården.

Abstract [en]

Background: Globally, 56.8 million people require palliative care each year. Nurses have a responsibility to provide quality palliative care in the home setting. A key challenge for nurses is alleviating suffering, fostering a secure care relationship, and simultaneously promoting well-being during the patient's final stage of life. Aim: To describe nurses’ experiences of palliative care within home healthcare. Method:  A literature review that included qualitative studies was analyzed using an inductive approach based on 12 scientific articles. The articles were analyzed based on the five steps described by Friberg (Friberg, 2023). The included articles were identified through the databases CINAHL and PsycINFO. Results: The analysis revealed two main themes and six subthemes. Nurses' experiences of palliative care in home healthcare reveal that they are emotionally affected both personally and professionally. Feelings of inadequacy, time pressure, high workload, and ethical challenges contribute to the need for nurses to develop strategies to remain professional in their work. Conclusion: Nurses face several challenges in providing palliative care within home healthcare, which they manage through various coping strategies, with humor frequently cited as a common strategy. Nurses’ understanding of how to use coping strategies can help society by supporting the conditions needed to provide good quality care in home healthcare.
