    Abstract [en]

    The Developmental Coordination Disorder Questionnaire (DCDQ) is a parent-completed measure designed to identify subtle motor problems in children of 8 to 14.6 years of age. The purpose of this study was to extend the lower age range to children aged 5 to 7 years, revise items to ensure clarity, develop new scoring, and evaluate validity of the revised questionnaire. Additional items with improved wording were generated by an expert panel. Analyses of internal consistency, factor loading, and qualitative/quantitative feedback from researchers, clinicians, and parents were used to select 15 items with the strongest psychometric properties. Internal consistency was high (alpha = .94). The expanded questionnaire was completed by the parents of 287 children, aged 5-15 years, who were typically developing. Logistic regression modeling was used to generate separate cutoff scores for three age groups (overall sensitivity = 85%, specificity = 71%). The revised DCDQ was then compared to other standardized measures in a sample of 232 children referred for therapy services. Differences in scores between children with and without DCD (p < .001) provide evidence of construct validity. Correlations between DCDQ scores and Movement Assessment Battery for Children (r = .55) and Test of Visual-Motor Integration (r = .42) scores support concurrent validity. The results provide evidence that the revised DCDQ is a valid clinical screening tool for DCD.

    Abstract [en]

    This paper examines how current understandings of childhood participation and motor development provide opportunities for using new technologies (such as virtual reality—VR) for children with neurodevelopmental disorders. Specifically, the International Classification of Functioning, Disability and Health is used to conceptualize the role of technology in treatment across body structures and body function, activity performance, and participation (WHO 2007, 2012). First, we review the particular motor control and learning mechanisms that have been implicated in children with atypical motor development, like DCD. This section will highlight avenues for targeted remediation. Next, VR-based rehabilitation systems are reviewed in relation to neurodevelopmental disorders, focusing first on CP and second on more recent applications for children with DCD. We describe the evolution of particular design innovations in virtual rehabilitation including recent advances using tangible interfaces, as well as other methods targeting cognitive function more specifically. Benefits of these various treatments will be viewed through the lens of current theory and evaluated at the level of child and family outcomes. Finally, we consider the broader aspects of the potential for technological innovation in rehabilitation and its impact on brain function, activity competence, and longer-term participation.

    Bakgrund

    Kunskap om den egna eller närståendes funktionsnedsättning och dess konsekvenser underlättar vardagslivet. Habiliteringen erbjuder ett femtiotal kurser/grupper varje termin. Kursutbudet presenteras i en webbkatalog, vilket minskat kostnader och kursadministrativ tid. Digitaliseringen förväntades öka tillgängligheten till kursutbudet. Patienter/närstående/nätverk och personal upplevde motsatsen.

    Syfte

    Förbättringsarbetets syfte var att förbättra tillgängligheten till kursutbudet genom att göra webbkatalogen mer tillgänglig och användarvänlig.

    Studiens syfte var att ur ett verksamhetsperspektiv beskriva vilka faktorer på mikro-, meso- och makronivå som påverkade hur webbkatalogen användes i verksamheten.

    Metod

    Förbättringsarbetet planerades och genomfördes med stöd av förbättringsrampen och Nolans förbättringsmodell.

    Datainsamling till studien gjordes via fokusintervjuer och analyserades med kvalitativ innehållsanalys. Koppling till mikro-, meso- och makronivå gjordes med The Clinical Adoption Framework (CAF) som teoretisk referensram.

    Resultat

    Förbättringsarbetet ledde till förbättringar men nådde inte de uppsatta målen. Innehållsanalysen definierade sex faktorer som kunde sorteras till mikro- och mesonivå.

    Slutsatser

    För att ett digitalt verktyg ska uppfattas användbart behövs kunskap hos ledningen om verksamheten och dess processer på mikronivå innan beslut fattas, så att rätt förutsättningar och stöd kan ges på mesonivå. Personalens delaktighet är en viktig faktor.

    Abstract [en]

    We assessed the effect of information sources on Ebola-specific knowledge and behavior during the 2014-2015 Ebola virus disease outbreak in Sierra Leone. We pooled data from 4 population-based knowledge, attitude, and practice surveys (August, October, and December 2014 and July 2015), with a total of 10,604 respondents. We created composite variables for exposures (information sources: electronic, print, new media, government, community) and outcomes (knowledge and misconceptions, protective and risk behavior) and tested associations by using logistic regression within multilevel modeling. Exposure to information sources was associated with higher knowledge and protective behaviors. However, apart from print media, exposure to information sources was also linked to misconceptions and risk behavior, but with weaker associations observed. Knowledge and protective behavior were associated with the outbreak level, most strongly after the peak, whereas risk behavior was seen at all levels of the outbreak. In future outbreaks, close attention should be paid to dissemination of information.

    Bakgrund Ett sätt att mäta patienternas upplevelser av bemötande är att mäta tillfredsställelsen. Under bemötande ingår humanistiskt arbetssätt och ett professionellt förhållningssätt. Enligt kompetensbeskrivningen bör en tandhygienist bemöta och behandla alla människor likvärdigt med respekt oavsett individuella förutsättningar. Genom gott omhändertagande kan tillfredställelsen bland patienterna höjas. Syftet var att undersöka patienternas grad av tillfredställelse av bemötandet från tandhygienister inom Folktandvården (allmäntandvården), samt hur patienterna upplever tandhygienisternas professionella förhållningssätt. Metod Studien var en kvantitativ tvärsnittsstudie. Urvalet bestod av personer som var 18 år eller äldre. Fem folktandvårdskliniker valdes strategiskt ut genom klusterurval. Deskriptiv statistik gjordes av bakgrundsvariabler och redovisades i tabeller. Jämförelser av variabler ålder, upplevd munhälsa samt väntetid utfördes med chi-två-tester. Resultat Majoriteten av deltagarna var tillfredsställda med tandhygienisternas bemötande. Samtliga deltagare uppgav att de blivit bemötta på ett respektfullt och hänsynsfullt sätt. Statistiskt signifikant skillnad avseende bemötande i relation till väntetid kunde påvisas. Gruppen där besöket började i tid var mer tillfredsställda än dem som fick vänta. Genom självbedömningen av Humanismskalan påvisades att tandhygienisterna bemötte dem utifrån ett humanistiskt förhållningssätt. Slutsats Studien visade att majoriteten av deltagarna visade tillfredsställelse med tandhygienisternas bemötande och professionella förhållningsätt samt att väntetid påverkade graden av tillfredställelse.

    AIM:

    The aim of the present study was to evaluate the effect on peri-implant mucosal inflammation from the use of a novel instrument made of chitosan in the non-surgical treatment of mild peri-implantitis across several clinical centers.

    MATERIALS AND METHODS:

    In this 6-month multicenter prospective consecutive case series performed in six different periodontal specialist clinics, 63 implants in 63 patients were finally included. The subjects had mild peri-implantitis defined as radiographic bone loss of 1-2 mm, pocket probing depth (PPD) ≥4 mm and a positive bleeding on probing (mBoP) score. The patients were clinically examined at baseline and after 2, 4, 12 and 24 weeks, and radiographs were taken at baseline and at 3 and 6 months. Treatment of the implants with the chitosan brush seated in an oscillating dental drill piece was performed at baseline and at 3 months. Reductions in the clinical parameters (PPD and mBoP) were compared between baseline and the later examination time points.

    RESULTS:

    Significant reductions in both PPD and mBoP were observed at all time points compared with the baseline clinical measurements (p < 0.001). The mean PPD and mBoP at baseline were 5.15 mm (4.97; 5.32) and 1.86 (1.78; 1.93), respectively, whereas the mean PPD and mBoP at 6 months were 4.0 mm (3.91; 4.19) and 0.64 (0.54; 0.75), respectively. Stable reductions in PPD and mBoP were evident up to 6 months after the initial treatment and 3 months after the second treatment. All 63 implants were reported to have stable radiographic levels of osseous support.

    CONCLUSIONS:

    This case series demonstrated that an oscillating chitosan brush is safe to use and seems to have merits in the non-surgical treatment of dental implants with mild peri-implantitis. To measure the effectiveness of the method, a multicenter randomized clinical trial needs to be undertaken.

    Objectives: This study seeks to review some of the approaches employed to address health and well-being issues in the elderly population.

    Methods: This article reviews and analyses a range of projects and approaches designed for the elderly population and aimed at preserving and/or enhancing physical and cognitive capabilities in later life.

    Results: Various intervention measures have been developed across the globe to preserve and/or enhance physical and cognitive capabilities of the elderly population. A selection of these measures is described in this article.

    Conclusions: Approaches which combine games psychology and mechanics with enabling technologies designed to engage, influence and motivate elderly people can encourage healthy active aging lifestyles. Healthy active aging helps to realise a double dividend of reduced healthcare costs and an improved quality of life for the elder citizen.

    Bakgrund: Enligt nationella patientenkäten är patienterna nöjda med vården, däremot skattas inte delaktigheten lika högt.  Ett sätt att öka delaktigheten är personcentrerad vård, som dessutom kan ge en mer effektiv vård med högre kvalitet. 

    Syfte: Studiens syfte var att beskriva och analysera ett förbättringsarbete om att öka patientens delaktighet genom införande av personcentrerad vård på en kardiologisk vårdenhet.

    Metod: Förbättringsarbetet genomfördes enligt Nolans förbättringsmodell. Studien var en fallstudie med induktiv ansats. Kvantitativ datainsamling skedde genom enkäter till patienter och medarbetare före och efter fem månaders förbättringsarbete. Kvalitativ datainsamling skedde genom foksugruppsintervjuer med medarbetare.

    Resultat: Enkäterna visade inga tydliga skillnader efter fem månaders förbättringsarbete. Både patienter och medarbetare skattade patientdelaktigheten högt. Det som skattades lägst av båda grupper var hänsynstagande till arbete, levnadsvanor och tidigare erfarenhet av sjukhusvård.

    Fokusgruppsintervjuerna visade att välinformerade patienter i större utsträckning kunde vara mer delaktiga. Medarbetarna fick en bättre förståelse av patienten, samtidigt fanns oro över att missa viktig information. Viljan fanns till personcentrering, men förutsättningarna saknades.

    Slutsats: Förbättringsmodellen är lämplig för införande av personcentrerad vård för att kunna testa vad som fungerar praktiskt. För att lyckas behövs tid och stöd från ledningen. Medarbetarna behöver träning, coaching och få vara delaktiga i förbättringsarbetet.

    Abstract [en]

    Previous research has shown that wheelchair-seated passengers using special transportation services (STS) have been injured without being involved in a vehicle crash. In order to estimate incident rate andsocietal costs for these vehicle-related injuries, the focus needs to be adjusted towards a traveler/incidentoriented perspective. The aim of the project was to utilize such a perspective, related to travelers using STS in Sweden. In order to address the chosen perspective, a mixed method approach was used, involving quantitative as well as qualitative research techniques applied on four different sets of data: one hospital-based record,two sets of STS incident report databases, and interviews with wheelchair-seated STS passengers. The results showed that the injury incidence rate in STS is considerable, i.e. 10 per 100,000 trips. However, this high incidence rate is not due to road traffic collisions, but to non-collision injury incidents involving many elderly and frail passengers, who easily sustain traumas ranging from minor to moderate. The costs were estimated to be 23 million USD per annum or 14 USD per trip. Results suggest that future injury prevention measures should focus on safety in boarding and alighting procedures, as well as thecontinuing development of WTORS.

    Abstract [en]

    Several transportation factors concerning older and disabled people are under transition in Sweden at present. By the year 2010, the public transit system must be fully accessible for all passengers. The present survey studied older people, in order to assess the perceived travel opportunities. Questionnaires were sent to a sample of older citizens (75+) in three Swedish mid-sized municipalities. The general conclusions were that even though older people show appreciation of the existing travel opportunities, there was evidence for restricted mobility for some sub-groups of these older people, due to various perceived barriers. These groups have few optional modes, and despite various accessibility measures, special transportation services – the mandatory demand-responsive transport service – continues to provide crucial mobility. Hence, there is more to be done regarding accessibility and usability in public transit for older people. Further studies have to clarify reasons for bus travel cessation. Even larger efforts have to be put into accessibility improvements, in particular intermediate transit solutions in order to meet the regulations and policies.

    BACKGROUND: Emergency department-based palliative care services are increasing, but research to develop these services rarely includes input from emergency clinicians, jeopardizing the effectiveness of subsequent palliative care interventions.

    AIM: To collaboratively identify with emergency clinicians' improvement priorities for emergency department-based palliative care for older people.

    DESIGN: This was one component of an experience-based co-design project, conducted using semi-structured interviews and feedback sessions.

    SETTING/PARTICIPANTS: In-depth interviews with 15 emergency clinicians (nurses and doctors) at a large teaching hospital emergency department in the United Kingdom exploring experiences of palliative care delivery for older people. A thematic analysis identified core challenges that were presented to 64 clinicians over five feedback sessions, validating interview findings, and identifying shared priorities for improving palliative care delivery.

    RESULTS: Eight challenges emerged: patient age; access to information; communication with patients, family members, and clinicians; understanding of palliative care; role uncertainty; complex systems and processes; time constraints; and limited training and education. Through feedback sessions, clinicians selected four challenges as improvement priorities: time constraints; communication and information; systems and processes; and understanding of palliative care. As resulting improvement plans evolved, "training and education" replaced "time constraints" as a priority.

    CONCLUSION: Clinician priorities for improving emergency department-based palliative care were identified through collaborative, iterative processes. Though generally aware of older palliative patients' needs, clinicians struggled to provide high-quality care due to a range of complex factors. Further research should identify whether priorities are shared across other emergency departments, and develop, implement, and evaluate strategies developed by clinicians.

    BACKGROUND:

    Enhanced external counterpulsation (EECP) is a noninvasive treatment recommended for patients with refractory angina pectoris (RAP), which generally includes 35 one-hour sessions over 7 weeks. No study has described how patients experience the time before, during, and between sessions, as well as the time after EECP treatment.

    OBJECTIVE:

    The aim of this study was to describe how patients with RAP experience EECP treatment.

    METHODS:

    Semistructured interviews took place with 15 strategically selected patients (11 men, 58-91 years old) who had finished 7 weeks of EECP at the 2 existing EECP clinics in Sweden. Data were analyzed using inductive qualitative content analysis.

    RESULTS:

    The results were divided into 4 content areas, each comprising 3 categories: (1) experiences before EECP was initiated comprised uncharted territory, being given a new opportunity, and gain insight; (2) experiences during EECP sessions comprised physical discomfort, need of distraction, and sense of security; (3) experiences between EECP sessions comprised physical changes, socializing, and coordinating everyday life; and (4) experiences after 1 course of EECP treatment comprised improved physical well-being, improved mental well-being, and maintaining angina in check.

    CONCLUSIONS:

    Enhanced external counterpulsation was perceived as an unknown treatment option among these patients with RAP. Nurses should emphasize education before and during treatment based on individualized care needs to improve knowledge and treatment experience, as well as prevent discomfort. There is a great need to increase the awareness of EECP among healthcare professionals because it can improve the life situation for patients with RAP.

    Abstract [en]

    Background: Despite optimal pharmacological treatments and invasive procedures, patients with refractory angina pectoris (RAP) remain symptomatic. Enhanced External Counterpulsation (EECP) is a non-invasive therapy based on 35 one-hour sessions. External pneumatic cuffs wrapped around the patient’s lower extremities inflate during cardiac diastole to create retrograde diastolic counterpulsation which leads to an improved coronary perfusion. EECP has for the past decade been used in European countries and has recently been updated in the European Guidelines as a clinical treatment option for patients with RAP.

    Purpose: The purpose was to describe the design and outcome variables of studies using EECP as a treatment for RAP.

    Methods: Search methods: Databases (i.e., PubMed,Wiley Online Library, Science Direct) were used to identify abstracts, reviews and journal articles related to this subgroup and EECP. Selection criterias: The chosen key words were EECP and refractory angina pectoris. Between time period 2000 to October 2016. Data collection and analysis: The literature search resulted in a total of 155 hits. Detected duplicates, irrelevant studies and others (i.e., editorials, letters) were removed (n=75). Research area and study design were reviewed in all remaining articles (n=80). In 56 of these, where design and content allowed, further analyses regarding follow-uptime and outcomes variables were performed.

    Results: An uneven distribution regarding research area was found among the analysed papers with dominance (48%) of studies within medicine and biology. Only 12% of the studies were performed within nursing. Prospective and longitudinal designs dominated (47%). Out of eight retrospective studies five were registry studies. Only two studies used a RCT design. No study used a qualitative approach. Out of 24 prospective studies 18 (75%) had a follow-up of 12 months or longer. 52% used biomedical outcomes (i.e., hemodynamic, biomarker, arterial stiffness, peripheral vascular reactivity) and 41% used clinical/patient reported outcomes (i.e., quality of life, functional classes, physical capacity, psychological aspects). Cost effectiveness was calculated in 7% of the studies.

    Conclusions/implications: EECP is recommended in guidelines as a possible treatment for patients with RAP. There is, however, a great need for increased nursing research both with qualitative and quantitative approaches. RCTs are sparse, as well as cost-effectiveness-studies.

    Abstract [en]

    Background: Refractory angina pectoris (AP) is a persistent, painful condition characterized by angina caused by coronary insufficiency in the presence of coronary artery disease. It has been emphasized that there are possible underlying neuropathophysiological mechanisms for refractory AP but chronic ischemia is still considered to be the main problem. These patients suffer from severe AP and cannot be controlled by a combination of pharmacological therapies, angioplasty or coronary bypass surgery. AP has a negative impact on quality of life and daily life. Enhanced external counterpulsation (EECP) is a therapeutic option for these patients.

    Aims: The aim of this study was to evaluate EECP after six months regarding physical capacity and health-related quality of life (HRQoL) in patients with refractory AP.

    Methods: This was a study with single case research experimental design involving 34 patients treated with EECP. Six minute walk test (6MWT), functional class with Canadian Cardiological Society (CCS) classification and self-reported HRQoL questionnaires as Short Form 36 (SF-36) were collected at baseline and after treatment. CCS class and SF-36 were repeated at six months follow-up.

    Results: Patients enhanced walk distance on average by 29 m after EECP (p<0.01). CCS class also improved (p<0.001) and persisted at six months follow-up (p<0.001). HRQoL improved significantly and the effects were maintained at follow-up after the treatment.

    Conclusion: Patients with refractory AP receive beneficial effects from EECP both in physical capacity and HRQoL. As other treatment options for this patient group are scarce, EECP should be offered to improve physical health and HRQoL in these patients.

    Background and aims: The nine-item Internet Gaming Disorder Scale -Short Form (IGDS-SF9) is brief and effective to evaluate Internet Gaming Disorder (IGD) severity. Although its scores show promising psychometric properties, less is known about whether different groups of gamers interpret the items similarly. This study aimed to verify the construct validity of the Persian IGDS-SF9 and examine the scores in relation to gender and hours spent online gaming among 2,363 Iranian adolescents.

    Methods: Confirmatory factor analysis (CFA) and Rasch analysis were used to examine the construct validity of the IGDS-SF9. The effects of gender and time spent online gaming per week were investigated by multigroup CFA and Rasch differential item functioning (DIF).

    Results: The unidimensionality of the IGDS-SF9 was supported in both CFA and Rasch. However, Item 4 (fail to control or cease gaming activities) displayed DIF (DIF contrast = 0.55) slightly over the recommended cutoff in Rasch but was invariant in multigroup CFA across gender. Items 4 (DIF contrast = -0.67) and 9 (jeopardize or lose an important thing because of gaming activity; DIF contrast = 0.61) displayed DIF in Rasch and were non-invariant in multigroup CFA across time spent online gaming.

    Conclusions: Given the Persian IGDS-SF9 was unidimensional, it is concluded that the instrument can be used to assess IGD severity. However, users of the instrument are cautioned concerning the comparisons of the sum scores of the IGDS-SF9 across gender and across adolescents spending different amounts of time online gaming.

    Abstract [en]

    A major focus in nursing education is on the judgement of clinical performance, and it is a complex process due to the diverse nature of nursing practice. Difficulties in the development of valid and reliable assessment measures in nursing competency continue to pose a challenge in nursing education. A holistic approach in the assessment of competency comprises knowledge, skills and professional attitudes, wherein the notion of competency incorporates professional judgement and management skills in the clinical situation. Therefore, the thesis aims to develop a holistic clinical assessment tool with a reasonable level of validity and reliability to meet the needs of clinical education.

    The conceptual framework underlying this research is formed by establishing a theoretical connection between the practice of learning, and of pedagogy and assessment. This research consists of five studies. In Study I, a systematic review was conducted to explore the current assessment practices and tools for nursing undergraduates. In Studies II, III and IV, a qualitative approach with focus group discussions was adopted to explore the views of final-year undergraduate nursing students, preceptors, clinical nurse leaders and academics on the clinical assessment. Based on the multiple perspectives, it therefore addresses concerns in clinical assessment. In Study V, a holistic clinical assessment tool was developed, for which a psychometric testing was conducted.

    The systematic review indicated that limited studies adequately evaluate the psychometric properties of the assessment instrument. The qualitative studies have raised an awareness of professional and educational issues in relation to clinical assessment. Workload, time, availability of resources, adequate preparation of preceptors, and availability of valid and reliable clinical assessment tools were deemed to influence the quality of students’ clinical learning and assessment. In addition, the presence of support systems and formal educational programs for preceptors influenced their preparation and self-confidence. Nursing leaderships in hospitals and educational institutions have a joint responsibility in shaping the holistic clinical learning environment and making holistic clinical assessment for students. The involvement of all stakeholders in the development of a valid and reliable assessment tool for clinical competency is also essential to the process. The Holistic Clinical Assessment Tool (HCAT) was developed by the author based on the systematic review, qualitative findings and the core competencies of registered nurse from the professional nursing boards. The HCAT consists of 4 domains and 36 assessment items. Furthermore, testing of the psychometric properties indicated that the HCAT has satisfactory content validity, construct validity, internal consistency and test-retest reliability.

    In conclusion, the HCAT is meritorious in that it carries the potential to be used as a valid measure to evaluate clinical competency in nursing students, and provide specific and ongoing feedback to enhance the students’ holistic clinical learning experience. The HCAT not only functions as a tool for self-reflection for the students, but also guides the preceptors in clinical teaching and assessment. In addition, the HCAT can be used for peer-assessment and feedback. It is imperative that the clinical and academic institutions establish various levels of ongoing support for both students and preceptors in the process of clinical assessment.

    AIM: To explore the perspectives of preceptors about clinical assessment for undergraduate nursing students in transition to practice.

    BACKGROUND: The assessment of clinical competence is a complex process due to the diverse nature of nursing practice. As such, nurse preceptors play a crucial role in clinical assessment for nursing students. Although preceptors may enjoy the process of clinical teaching and assessment, they face challenges that need to be addressed to facilitate students' clinical learning and assessment.

    METHODS: An exploratory qualitative approach was adopted. A purposive sample of 17 preceptors from two tertiary hospitals in Singapore was recruited to participate in three focus group discussions from March to July 2014.

    FINDINGS: Five themes emerged from the data analysis: the need for a valid and reliable clinical assessment tool; meaningful reflection and feedback; varied methods in clinical assessment; high level of commitment and struggles with dual roles and the need to enhance the support system for preceptors.

    CONCLUSIONS: There is no doubt that a valid and reliable clinical assessment tool can serve as a guide for preceptors to enhance their clarity in assessment and feedback. It is essential for all stakeholders to be involved in the development of the clinical assessment tool. Workload, time, support system and formal educational programmes for preceptors influenced their preparation and self-confidence.

    IMPLICATIONS FOR NURSING AND/OR HEALTH POLICY: The themes that emerged from the focus group discussions could assist in the development of a more reliable and valid clinical assessment tool to evaluate the clinical competence of nursing students in transition to practice. Clinical and educational institutions could collaborate closely to develop an educational programme and a preceptorship handbook, focusing on pedagogical contents for clinical education and assessment.

    BACKGROUND: Consolidated clinical practicum prepares pre-registration nursing students to function as beginning practitioners. The clinical competencies of final-year nursing students provide a key indication of professional standards of practice and patient safety. Thus, clinical assessment of nursing students is a crucial issue for educators and administrators.

    OBJECTIVE: The aim of this systematic review was to explore the clinical competency assessment for undergraduate nursing students.

    DATA SOURCES: PubMed, CINAHL, ScienceDirect, Web of Science, and EBSCO were systematically searched from January 2000 to December 2013.

    METHODS: The systematic review was in line with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Published quantitative and qualitative studies that examined clinical assessment practices and tools used in clinical nursing education were retrieved. Quality assessment, data extraction, and analysis were completed on all included studies.

    RESULTS: This review screened 2073 titles, abstracts and full-text records, resulting in 33 included studies. Two reviewers assessed the quality of the included studies. Fourteen quantitative and qualitative studies were identified for this evaluation. The evidence was ordered into emergent themes; the overarching themes were current practices in clinical assessment, issues of learning and assessment, development of assessment tools, and reliability and validity of assessment tools.

    CONCLUSION: There is a need to develop a holistic clinical assessment tool with reasonable level of validity and reliability. Clinical assessment is a robust activity and requires collaboration between clinical partners and academia to enhance the clinical experiences of students, the professional development of preceptors, and the clinical credibility of academics.

    The nature of nursing practice is diverse; therefore, clinical assessment is a complex process. This study explores the perceptions of clinical nurse leaders and academics on clinical assessment for undergraduate nursing education during transition to practice. An explorative qualitative approach was applied. Eight nurse managers, six clinical nurse educators, and eight academics from two tertiary hospitals and a university in Singapore participated in four focus group discussions. Thematic analysis was conducted. Four overriding themes were revealed: the need for a valid and reliable clinical assessment tool, preceptors' competency in clinical assessment, challenges encountered by the students in clinical assessment, and the need for close academic and clinical collaboration to support preceptors and students. Closer academic-clinical partnership is recommended to review the clinical education curriculum. Clinical and educational institutions need to work closely to design a learning program to enhance preceptors' competence in clinical pedagogy and assessment. Furthermore, a stress management program could build students' resiliency in coping with unfamiliar clinical environments. Ongoing support needs to be provided for both preceptors and students to enrich the preceptorship and learning experiences.

A holistic approach in assessment of competency is advocated.Difficulties in the development of valid and reliable assessment measures in nursing competency have resulted in thedevelopment of assessment instruments with an increase in face and content validity, but few studies have testedthese instruments psychometrically. It is essential to develop a holistic assessment tool to meet the needs of the clinicaleducation. The study aims to develop a Holistic Clinical Assessment Tool (HCAT) and test its psychometric properties.

    Methods:The HCAT was developed based on the systematic literature review and the findings of qualitative studies.An expert panel was invited to evaluate the content validity of the tool. A total of 130 final-year nursing undergraduatestudents were recruited to evaluate the psychometric properties (i.e. factor structure, internal consistency and test-retestreliability) of the tool.

    Results:The HCAT has good content validity with content validity index of .979. The exploratory factor analysis revealsa four-factor structure of the tool. The internal consistency and test-retest reliability of the HCAT are satisfactory withCronbach alpha ranging from .789 to .965 and Intraclass Correlation Coefficient ranging from .881 to .979 for the foursubscales and total scale.

    Conclusions:HCAT has the potential to be used as a valid measure to evaluate clinical competence in nursingstudents, and provide specific and ongoing feedback to enhance the holistic clinical learning experience. In addition,HCAT functions as a tool for self-reflection, peer-assessment and guides preceptors in clinical teaching and assessment.

    BACKGROUND: Assessment of clinical competence requires explicitly defined standards meeting the national standards of the nursing profession. This is a complex process because of the diverse nature of nursing practice.

    OBJECTIVES: To explore the perceptions of final-year undergraduate nursing students regarding clinical assessment at transition to practice.

    METHODS: An exploratory qualitative approach was adopted. Twenty-four students participated in three focus group discussions. Thematic analysis was conducted.

    FINDINGS: Five themes emerged: the need for a valid and reliable clinical assessment tool, the need for a flexible style of reflection and specific feedback, the dynamic clinical learning environment, students' efforts in learning and assessment, and the unclear support system for preceptors.

    CONCLUSIONS: Workload, time, resource availability, adequate preparation of preceptors, and the provision of valid and reliable clinical assessment tools were deemed to influence the quality of students' clinical learning and assessment. Nursing leadership in hospitals and educational institutions has a joint responsibility in shaping the clinical learning environment and providing clinical assessments for the students.

    Bakgrund: Urinkateterns design har inte förändrats sedan 1930-talet. Detta trots att komplikationer relaterat till urinkatetern kan vara allvarliga och vanligt förekommande. För att förhindra komplikationer krävs det att kateterbärarna använder sig av egenvård. För en fungerande egenvård behöver kateterbärarna goda kunskaper om urinkatetrar, vilket är sjuksköterskans uppgift att förmedla.Syfte: Att beskriva personers upplevelse av att leva med kvarliggande urinkateter.Metod: Litteraturöversikt med kvalitativa studier. 11 artiklar gick igenom kvalitetsgranskningen till resultatet. Fribergs 5-stegs modell användes för att analysera datan.Resultat: Att leva med urinkateter kan innebära en stor lättnad och möjliggöra ett friare liv för personerna. Dock kan det också innebära precis tvärtom, att urinkatetern hindrar personerna på grund av mycket oro och, tidskrävande skötsel och en påverkad kroppsbild. Kateterbärarnas relationer till människor i sin närhet har ett stort inflytande över upplevelsen av att leva med urinkateter.Slutsatser: Många personer med urinkateter upplever en ständig oro över problem som kan komma att uppstå. Här råder ett glapp mellan sjukvårdens kunskap och utbud och kateterbärarnas upplevelse och efterfrågan. Sjukvården är tillsynes inte medvetna om kateterbärarnas verkliga upplevelse. Detta kräver vidare forskning för att utreda förbättringsmöjligheter inom hälso- och sjukvården.

    Abstract [en]

    The purpose of this study is to explore prerequisites for a healthy lifestyle as described by individuals diagnosed with psychosis. Forty participants who had performed a lifestyle intervention focusing on physical activities and lifestyle education were interviewed. Conventional content analysis was used. The results are described in two categories: (1) Individual Prerequisites and (2) Being a Part of Society. The individuals said that they got stuck in a state of planning without taking action. It was pointless to make a bigger effort because the psychotic disorder could, at any time, worsen the prerequisites. They also said that they wanted to live like everybody else and therefore tried to adopt a normal lifestyle. Future interventions or professional support by mental health nurses and other health care givers should target the transition from planning to action to achieve a healthy lifestyle, and should help the individual to taking part in society.

    Abstract [en]

    This research studied the effects of a lifestyle intervention on health-related quality of life (HRQoL) and metabolic risk factors in persons with psychosis, using a longitudinal intervention design with a matched reference sample. In participants in the intervention group, scores on the EQ-VAS, which measured HRQoL, improved from 57.6 to 63.3 (SD = 17.8) (p = 0.05), and HDL cholesterol concentration increased from 1.03 to 1.11 (0.19) mmol/l. (p = 0.02). There was no significant change in body weight among participants. The results of this study indicate that a lifestyle intervention based on group meetings improves HRQoL in patients with psychosis and provides small improvements in metabolic risk factors.

    Childhood cancer survivors adjust to the physical, mental, and social difficulties associated with their illness and treatment. This process can be facilitated by social support from peers. For children, this is often problematic due to geographical, clinical, and age-related limitations. This paper reports on a stakeholder assessment study that confirms the relevance of a digital peer support service for childhood cancer survivors. The analysis establishes where in the existing health care process the digital peer support service should be introduced, what actors play a key role in facilitating service onboarding and use, and characterizes desirable user experience qualities. The analysis also yields a collection of design challenges to be addressed in the development of the digital peer support service.

    Abstract [en]

    A single nucleotide polymorphism (SNP) -420C> G of the resistin gene was screened in 248 colorectal cancer (CRC) patients and 256 controls. No significant difference in genotype distribution was found. However, we found an upregulation in 92% of the samples in the levels of resistin protein in cancer tissue (n = 83). Immunohistochemical analysis revealed heterogenous staining of resistin predominantly in the cancer tissue. Further, resistin induced secretion of MMP-2 and MMP-9 from monocytes. The results of this study suggest that resistin may play a partial role in CRC but that the -420C> G resistin polymorphism is not a potential genetic susceptibility factor.

    Abstract [en]

    Chemokines are believed to play a crucial role in local immunoresponse by regulating leukocyte movement in various tissues, including the intestinal mucosa. It has been suggested that they are key players in cancer biology, and several studies have identified leukocyte infiltration as a hallmark of most cancers. The chemokines CCL17 and CCL22 attract CCR4-bearing cells, which are especially polarised to Th2-type cells and regulatory T cells (Treg). Recent studies have revealed the participation of the CCL17 and CCL22 proteins in diseases such as atopic dermatitis and lymphoma. The purpose of this study was to assess the role of CCL17 and CCL22 protein expression in colorectal cancer (CRC) and to ascertain whether an association exists between promoter -431C>T CCL17 and -961G>A CCL22 gene polymorphisms in CRC versus non-CRC subjects. Using the ELISA assay, we noted a significantly higher expression of CCL22 in tumour tissue with a 2.3-fold up-regulation (tumour vs. paired normal tissue, n=78) but no significant difference in CCL17 protein expression. Immunohistochemistry revealed protein expression of CCL22 and CCL17 in the epithelial compartment of cancer tissue, in epithelial cells at the resection border that reflects normal tissue, and in some stromal cells such as lymphocytes, macrophages, and fibroblasts. Using a TaqMan system we screened for -431C>T CCL17 and -961G>A CCL22 gene variants in 245 CRC patients and 256 controls, but could not find any significant difference in genotype distribution or in allelic frequencies between the two groups. The genotype and allelic distributions of CRC patients were not related to tissue levels of CCL17 and CCL22 protein, and none of the variables were associated with plasma levels or clinical characteristics. To ascertain whether the tissue expression of CCL17 and CCL22 exerts an influence oil the pathogenesis of CRC, a forthcoming study oil the 5-year survival rate of CRC patients will be conducted.

    Bakgrund: En kronisk oförutsägbar sjukdom som Crohns sjukdom framkallar fysisk, psykologisk och social stress. Sjukdomen påverkar vardagen, försämrar livskvalitet och skapar oro. För att bedriva personcentrerad vård krävs insikt i hur denna oro upplevs.

    Syfte: Syftet med studien var att belysa sjukdomsrelaterad oro hos personer med Crohns sjukdom.

    Metod: Åtta kvinnor och fyra män mellan 30 och 64 år som skattat sin sjukdomsrelaterade oro som hög eller mycket hög identifierades. Intervjuer genomfördes och analyserades med kvalitativ innehållsanalys.

    Resultat: Analysen genererade tre kategorier: (1) Oro för sjukdomen i sig, (2) känslor runt oron, (3) samt hantering av oron. Sjukdomens oförutsägbarhet samt nedsatt funktion till följd av trötthet och bristande kontroll över tarmfunktionen var de mest framträdande orsakerna till oro. Oron skapade känslor av stress, skuld och besvikelse. Deltagarna uttryckte önskemål att få prata om och synliggöra oron men hade också funnit egna sätt att hantera denna.

    Slutsats: Det finns ett kvarstående behov av att ventilera och få förståelse för sjukdomsrelaterad oro, även efter många års sjukdom. Personer med Crohns sjukdom måste få prata om sin oro, vara en aktiv partner i vården och tillsammans med vårdgivaren finna sätt att hantera oron så att denna kan lindras.

    Abstract [en]

    Crohn disease, a chronic inflammatory bowel disease, affects everyday life, impairs quality of life, and creates worries. This study explored disease-related worries in persons with Crohn disease to gain deeper insight into how this worry is experienced and handled. Eight women and 4 men with Crohn disease who reported their disease-related worries as high or very high in a questionnaire were interviewed. The interviews were evaluated by qualitative content analysis, generating 3 categories: worries about the disease itself; feelings related to the worries; and management of the worries. The unpredictable course of the disease, impaired function due to fatigue, and lack of bowel control were the most prominent causes of worry. The worries created feelings of stress, guilt, and frustration. The participants expressed a need to talk about their worries, to make them visible and recognized, and to be understood. The participants found individual ways to handle the worries. Actively gaining control by changing focus, performing relaxation exercises, and seeking information were important self-care strategies. In addition, a strong and partially unmet need for expressing and venting concerns to one's caregiver was identified.

    Bakgrund: Kardiovaskulära sjukdomar räknas till de vanligaste folksjukdomarna i Sverige. De kardiovaskulära sjukdomar som arbetet behandlar är, mykoardiell, ischemi, hjärtinfarkt, hjärtsvikt och arytmier. Insjuknandet förändrar livssituationen i många avseenden för den drabbade. Det är känt sedan tidigare att insjuknandet i kardiovaskulära sjukdomar påverkar den sexuella hälsan vilket har inverkan på personers livskvalitet. Syfte: Att beskriva upplevd sexuell hälsa hos vårdsökande personer med kardiovaskulär sjukdom. Metod: En kvalitativ litteraturöversikt med induktiv ansats genomfördes utifrån åtta vetenskapliga artiklar. Resultat: Resultatet beskrevs genom fyra huvudteman. Första temat som framkom var sexuella attityder och vanor. Vårdsökande personer med kardiovaskulär sjukdom upplevde en förändrad prioritet av sexuallivet och en förändring i sexuella mönster.  Andra temat som framkom var sexuell hälsa innefattar intima relationer. Det framkom att upplevelsen av sexuella hälsa påverkades av hur partnern hanterade situationen. Tredje temat var kroppsliga och själsliga upplevelser. Vårdsökande personer upplevde rädsla och ångest inför att utöva samlag. Kroppsliga förändringar bidrog till nedsatt sexuell hälsa. I fjärde temat, kommunikation, framkom det att sexuell hälsa var ett tabubelagt samtalsämne. Sjukvården upplevdes brista i ansvaret att informera om hur sexuallivet kunde fungera efter insjuknandet i en kardiovaskulär sjukdom. Slutsats: Förändringar i sexuellt mönster, rädsla, kroppsliga symtom, partner och information är faktorer som påverkar upplevelsen av sexuell hälsa vid kardiovaskulär sjukdom.  Vårdpersonal behöver ta ansvar och initiativ till att efterfråga om önskan att samtala kring ämnet finns för att främja sexuell hälsa.

    Abstract [en]

    Background: In serious health conditions, like childhood cancer, parent proxy reports are used for obtaining information. Previous studies have shown controversial results on agreement between children’s and parents’ ratings. In addition, there is lack of proxy studies in research including young children. The aim of the present thesis is (a) to examine how young children as self-raters and mothers as proxy-raters report over time on cancer related health issues, and (b) to explore the factors that may affect the agreement of each mother-child pair.

    Method: A longitudinal quantitative research design was chosen. Eight young children with cancer aged three to six years and their mothers were followed with questionnaires every six months for four time points. One measure on children’s feelings about their health situation and one measure on perceptions of their everyday functioning were completed by children and mothers at each time point. Descriptive statistics were used for data analysis.

    Results: Higher frequency of agreement was found in T4 (18 months after the diagnosis) for both measures. Between the two measures, higher frequency of agreement was found for the functioning measure. The mother’s educational level was found to be correlated with higher frequency of agreement (functioning measure).

    Conclusion: The time progress, the mother’s educational level, the number of siblings, the specific shared experience, like preschool, and the concrete and observable issues, like “functioning” rather than “feelings”, were found to be correlated with higher frequency of agreement between young children with cancer and their mothers. The convenient and small sample imposes the need for further research.

    Aims

    Early detection of diabetes is important for the prevention of diabetic complications. The best adiposity index for indicating Type 2 diabetes mellitus remains unclear. We aimed to identify the optimal adiposity measure among BMI, waist circumference, waist–hip ratio and waist-to-height ratio to indicate undiagnosed Type 2 diabetes and impaired fasting glucose in Chinese adults.

    Methods

    A total of 7567 participants aged 20–79 years were included in this study. Impaired fasting glucose was defined as a fasting plasma glucose level of 6.1–6.9 mmol/l in participants without diabetes. Undiagnosed Type 2 diabetes was identified as fasting plasma glucose ≥ 7.0 mmol/l when neither a history of diabetes nor use of hypoglycaemic drugs was present. Body weight, height, waist and hip circumferences were measured following standard procedures. Data were analysed using logistic regression and areas under the receiver operating characteristic curves.

    Results

    Of the 7567 participants, 536 were defined as having impaired fasting glucose and 690 were patients with Type 2 diabetes, including 290 (3.8%) persons with undiagnosed diabetes. In multivariate logistic regression, the odds ratios of waist-to-height ratio (≥ 0.5) were stronger than BMI (≥ 24 kg/m2), waist circumference (≥ 85 cm in men and ≥ 80 cm in women) and waist–hip ratio (≥ 0.85) for undiagnosed Type 2 diabetes and impaired fasting glucose. Among the four indices, waist-to-height ratio ≥ 0.5 showed the largest area under the receiver operating characteristic curve for diagnosing undiagnosed Type 2 diabetes (0.725, 95% CI 0.693–0.756) and impaired fasting glucose (0.662, 95% CI 0.638–0.687).

    Conclusions

    By comparison with BMI, waist circumference and waist–hip ratio, waist-to-height ratio ≥ 0.5 may be the best indicator for undiagnosed Type 2 diabetes and impaired fasting glucose.

    BACKGROUND: Recovery after anesthesia and surgery is a complex process and depends on many factors such as patient, anesthesia and surgery conditions as well pre-existing comorbidities.

    OBJECTIVES: The aims of this study were to translate the 40-item quality of recovery score (QoR-40) into Persian and evaluate its psychometric properties in Iranian patients.

    PATIENTS AND METHODS: We enrolled patients candidate for elective general surgery undergoing general anesthesia from July 2013 to December 2013 at Shahid Rajaee Hospital, Qazvin, Iran. Translation was performed based on Beaton's and Bullinger's recommendations. Estimates of internal consistency, test-retest reliability, concurrent validity, predictive validity and clinical validity were performed.

    RESULTS: All estimates of internal consistency were high (Cronbach's alpha = 0.89 for global estimates, subscales between 0.89 and 0.93). All test-retest scores and subscales were between 0.71 and 0.88.The correlation with a recovery visual analogue scale was 0.51, and all subscales correlated significantly with comparable subscales of the SF-36. An exploratory factor analysis found five-components and explained 52% of the variance. A confirmatory factor analysis based on the five-components, yielded good fit statistics (CFI = 0.93).

    CONCLUSIONS: Overall, the Persian version of the QoR-40 was both conceptually and linguistically equivalent to the original English QoR-40. This study revealed that the Persian version of the QoR-40 is a valid and reliable instrument to assess the recovery quality in Iranian patients after surgery.

    Given that there is a lack of instruments assessing internet-related addictions among Chinese population, this study aimed to validate the Chinese version of the nine-item Internet Gaming Disorder Scales- Short Form (IGDS-SF9), Bergen Social Media Addiction Scale (BSMAS), and Smartphone Application-Based Addiction Scale (SABAS) among Hong Kong university students. Participants aged between 17 and 30 years participated in the present study (n = 307; 32.4% males; mean [SD] age = 21.64 [8.11]). All the participants completed the IGDS-SF9, BSMAS, SABAS, and the Hospital Anxiety and Depression Scale (HADS). Confirmatory factor analyses (CFAs) were used to examine the factorial structures and the unidimensionality for IGDS-SF9, BSMAS, and SABAS. CFAs demonstrated that the three scales were all unidimensional with satisfactory fit indices: comparative fit index = 0.969 to 0.992. In addition, the IGDS-SF9 and BSMAS were slightly modified based on the modification index in CFA. The Chinese IGDS-SF9, BSMAS, and SABAS are valid instruments to assess the addiction levels of internet-related activities for Hong Kong university students.

    Abstract [en]

    Research aim: The aim was to explore the experience of practising occupational therapists in transferring their learning from the Mindful Therapeutic Relationship (MTR) workshop into their own clinical practice. The workshop was developed based on the Intentional Relationship Model (Taylor 2008) and focuses on therapists being aware and mindful of the therapeutic relationship processes that occur during and after intervention.

    Research design: Thirteen occupational therapists participated in the workshop and subsequently completed between one and four reflection notes. These notes were subjected to a qualitative thematic analysis.

    Research method: Participants of the workshop were asked to write a weekly reflection for a period of four–six weeks to submit to the researcher. Thematic analysis strategies (Guest et al 2012) were used.

    Results: The findings were organised into three themes: therapeutic relationship reasoning, barriers to implementing the current learning and facilitators of transferring theory into practice.

    Conclusions: The therapists value learning and practising strategies that can support therapeutic relationships. Reflection skills seem essential for establishing and developing therapeutic relationships; such skills may need to be facilitated by the work environment.

    Abstract [en]

    Swedish Child and Youth Habilitation Services (C-YHS) for children with disabilities and their families' build on regular planning meetings involving families and professionals, and appointments and interventions implemented between meetings. This study explores the content of issues discussed at planning meetings, and the relation between content and activities implemented in everyday interventions. Longitudinal data from five families and their C-YHS-teams were used. Data were analyzed using conventional content analysis. The results illustrate a process with a high degree of correspondence between families' concerns, experienced problems, the formal decisions and the activities they generated. Concerns were focused on the future, and related actions focused on supporting adults in the environment, mostly the parents, thus indirectly relating to the child. Problems were focused on the current situation, and to a larger extent concerned actions directly related to the child. Although a family-centred service, interventions focused on the proximal environment, may be underreported.

    In family-centered intervention, one important issue is to support families' different ways to manage stress. The purpose of this study was to investigate whether professionals encourage positive coping strategies in families of children with disabilities. Data (videotaped planning meetings and interviews) from 5 families enrolled in intervention programs were used. The data were analyzed deductively by applying theoretical concepts of 4 types of positive coping. Goal-directed problem-focused coping was the most frequently used type of coping, together with spiritual beliefs and practices. Professional encouragement of families' own strategy to solve problems occurring in everyday life must focus not only on explicit problem solving but also on the use of supplemental coping types.

    Problem solving is recognized as a skill, helping families of children with disabilities to manage problems in everyday life. Family problem-solving skills may therefore be seen as an important outcome of a child and youth habilitation service. The aim of this pilot feasibility study was to examine the design of a future web-based questionnaire study focusing on problem-solving patterns in relation to resources in families of children with disabilities. The descriptive statistical analyses built on data from 13 families and findings showed an overall satisfactory score distribution for three of the included instruments, whereas two instruments showed floor effects in one third of the items. Findings indicated design problems with data collection related to adapting questionnaires to a web-based survey format and to problems with the stop function that was added. Implementing the main study using web-based surveys needs critical considerations according to the choice of the web tool and the recruitment process.

    Bakgrund: I Sverige bedöms mellan 70 000 till 75 000 att ha behov av palliativ vård av drygt 90 000 avlidna personer årligen. Palliativ vård utgår från att lindra lidandet och andra fysiska, psykiska och andliga problem som uppstår i samband med livshotande sjukdom. De närstående utgör en stor och viktig del av vården för den palliativa personen. Vid vård i livets slutskede är målet att bidra till välbefinnande och värdighet och ge stöd till de närstående.

     

    Syfte: Syftet var att beskriva närståendes upplevelse när en familjemedlem vårdas inom palliativ vård.

    Metod: En litteraturöversikt genomfördes av 14 vetenskapliga kvalitativa artiklar som analyserades och sammanställdes.

     

    Resultat: Resultatet visar att fler närstående har ökat behov av stöd och hjälp i den palliativa fasen. Fem huvudteman presenteras utifrån de närståendes upplevelser, att känna delaktighet, att känna stöd, att känna hjälplöshet och maktlöshet, att känna tillit och trygghet och att känna förhoppning. 

     

    Slutsats: De närstående uttrycker ökat behov av stöd och information inom den palliativa processen. Samt att som vårdpersonal är det är viktigt att uppmärksamma närståendes behov och känslor eftersom de upplever att de ställs åt sidan. Den palliativa vården upplevs vara en förändrad livssituation som är krävande och svår att komma ifrån som närstående. För att kunna skapa en bättre upplevelse av vården har sjuksköterskan en väsentlig roll, genom att bemöta och erbjuda stöd bidrar sjuksköterskan till positiva upplevelser, eftersom det är en komplex situation för de närstående.

    Sammanfattning

    På laboratoriet för Klinisk Patologi i Länssjukhuset Ryhov i Jönköping, snittas paraffinbäddade vävnader som kontroller i en liten mängd med jämna mellanrum för att täcka behovet vid analys av immunohistokemiska färgningar. Vid vissa perioder är behovet av kontroller stort. Detta gör att arbetet kräver mer tid för att hinna snitta alla kontroller som behövs. För att möjliggöra ett smidigt flöde och tidsbesparing skulle ett nytt sätt testas genom att förvara vävnadssnitten för kontrollerna i frysen. Därför syftet med denna studie var att kontrollera om antigenens kvalitet och stabilitet skulle påverkas efter infrysning av kontrollvävnadssnitten i -18 °C under olika tidsperioder (36, 22, 9 och 2 veckor). Detta gjordes genom att utvärdera frysta vävnadssnitt för immunohistokemisk färgning i jämförelse med nytagna vävnadssnitt. Vävnadssnitt som används blev frysta vid fyra olika tidsperioder. Totalt 47 frysta vävnadssnitt färgades med 16 olika antikroppar. Samtliga 47 vävnadssnitten som inkuberades med respektive antikropp för immunohistokemisk färgning godkändes.

    Resultat från denna studie konstaterade att frysta vävnadssnitt kunde bevara stabilitet hos antigen under lång tid. Färgintensitet presenterades som svag hos cirka 40 % av vävnadssnitten som förvarades frysta under ungefär 22 veckor. Denna studie kom även fram till att det fanns ett samband mellan den tiden som vävnadssnitten förvarades i frysen och antigen stabilitet i vävnadssnitten som behövdes för en fullständig immunofärgning.

    Sammanfattning

    Bakgrund: Diabetes är en sjukdom som ständigt ökar världen över och ses numera som en folksjukdom i Sverige. Orsaken till sjukdomen relateras ofta till livsstil, vilket ställer krav på att den drabbade personen gör livsstilsförändringar.

     

    Syfte: Att beskriva vuxna personers upplevelser av att leva med diabetes typ II.

    Metod: En litteraturöversikt med induktiv ansats har gjorts, där 12 artiklar av kvalitativ design har granskats kritiskt, analyserats och sammanställts.

    Resultat: Personernas upplevelser presenteras i tre kategorier med tillhörande subkategorier. Rädsla, skam och frustration var några av många ogynnsamma känslor som visade sig uppstå vid en diabetesdiagnos. Stöd från omgivningen gav kraft och motivation till egenvårdsskötsel, vilket i sin tur var en förutsättning för att uppnå kontroll och därmed hälsa.

     

    Slutsatser: En diabetesdiagnos skapar en livsomställning som både är känslomässigt och praktiskt svår att hantera.

    Hypertension is a global disease that many effected people in developing countries is not aware of. Hypertension is linked with cardiovascular disease. Prehypertension is not a disease but if not correctly treated, it could develop into hypertension. The aim of the study was to investigate if there are any differences in circadian blood pressure between two study groups, one group with normal blood pressure and one group with prehypertension. This study was a explorative study and its design is based on measurements of blood pressure values and a questionnaire was used to help get the data collection. 51 students volunteered to have their blood pressure taken from them and out of these 51, 24 where selected into two groups of 12 each for the Ambulatory blood pressure monitoring.

    hese 24 students would be a part of our study and an ambulatory (Schiller-102 plus) blood pressure monitor was used to collect the data. The prevalence of prehypertension findings in the clinical testing phase was 37% of the population. There was a variation between the groups during the day (systolic) but there was not a significant difference during the night.

    Sammanfattning

     

    Bakgrund: Tandvårdsrädsla är ett vanligt förekommande problem i tandvården vilket kan bidra till en sämre oral hälsa. Tidigare forskning visar vissa samband mellan tandvårdsrädsla och olika temperament men de mest signifikanta resultaten varierar. Syfte: Syftet med studien var att undersöka samband mellan tandvårdsrädsla och temperament hos ungdomar i nionde klass. Metod: Studien är en tvärsnittsstudie baserad på ett stratifierat urval av ungdomar i nionde klass. Två validerade enkäter innehållande självrapporterade frågor om tandvårdsrädsla och temperament delades ut till ungdomarna i klassrummen och samlades in vid samma tillfälle. Resultat: Totalt 111 deltog, 3 % av ungdomarna konstaterades vara tandvårdsrädda. Signifikanta samband sågs mellan blyghet och DFS-poäng. Kvinnorna hade signifikant högre medelvärde av DFS-poäng och emotionalitet jämfört med männen. Männen hade signifikant högre medelvärde i aktivitet. Inga signifikanta samband fanns mellan tandvårdsrädsla och utländsk- eller svensk bakgrund. Ungdomarna med svensk bakgrund hade högre medelvärde i temperamentet sällskaplighet jämfört med ungdomarna med utländsk bakgrund. Slutsats: Prevalensen av konstaterad tandvårdsrädsla var 3 % hos ungdomarna. Kvinnorna hade högre grad av DFS-poäng och emotionalitet. Männen rapporterade högre grad av aktivitet. Ungdomar med utländsk bakgrund hade en lägre grad av sällskaplighet. Ett signifikant samband fanns mellan DFS-poäng och temperamentet blyghet.
