    Titel: Röntgensjuksköterskans upplevelse av att arbeta med externa granskare under jourtid

    Syfte: Att undersöka hur röntgensjuksköterskan upplever arbetet med externa granskare under jourtid.

    Material och metod: Sex stycken röntgensjuksköterskor från två olika sjukhus i mellansverige intervjuades under perioden januari till mars 2017. Inklusionskriterierna för studien var minst två års erfarenhet av yrket samt minst ett års erfarenhet av att arbeta med externa granskare under jourtid. Tillstånd från verksamhetschefer och samtycke från informanterna erhölls innan studien startade. Data samlades in genom en kvalitativ metod med semistrukturerade intervjuer bestående av öppna frågor. Den insamlade datan analyserades med kvalitativ innehållsanalys enligt Graneheim & Lundman. Intervjuerna transkriberades och lästes igenom flera gånger. Därefter identifierades meningsbärande enheter och dessa kondenserades till koder. Genom koderna kunde kategorier och subkategorier identifieras.

    Resultat: I resultatet framkom fyra kategorier; Teleradiologi – tekniska aspekter, externa granskare jämfört med radiolog på plats, kommunikation och röntgensjuksköterskans kompetenser och egenskaper med tillhörande subkategorier.

    Konklusion: Informanterna upplever att det tar längre tid att kommunicera med externa granskare (EG) i jämförelse med att ha en radiolog på plats som kan ge svar direkt när röntgensjuksköterskan (RSS) är i behov av vägledning och snabba svar. I kommunikationen upplevs det också finnas språkbarriärer samt bristande kunskap, speciellt hos de sekreterare som tar emot samtal hos EG. Detta leder till frustration och blir ofta ett hinder i akuta situationer då RSS vill prata med en radiolog direkt. Under jourtid upplever RSS att denne får ta ett ökat ansvar samtidigt som förmågan att ta egna initiativ och beslut måste finnas. RSS upplever ingen förändring av bildkvalitén på de bilder som skickas i arbetet med EG. Informanterna upplevde i helhet att arbetet med EG fungerar bra och tillfredsställande under förutsättningarna att tekniken fungerar som den ska samtidigt som arbetsflödet inte ökar för mycket.

    I Sverige drabbas årligen omkring 7000 kvinnor av bröstcancer vilket är den vanligaste cancerformen hos kvinnor. Bröstcancer påverkar den hälsorelaterade livskvaliteten (HRQoL) både på kort- och lång sikt. HRQoL beskriver den inverkan hälsa har på funktionsförmåga och upplevda välbefinnande inom fysiska-, psykiska- och sociala livsområden. Syftet var att undersöka hur kvinnor med bröstcancer värderar sin hälsorelaterade livskvalitet. Litteraturöversiktens design utgick från en deduktiv ansats. Litteraturöversikt gjordes bland artiklar i databaser med inriktning omvårdnad. Resultatet innefattar 18 vetenskapliga artiklar, nio kvalitativa- och nio kvantitativa studier. HRQoL påverkas negativt inom fysiskt-, psykiskt- och socialt välbefinnande hos kvinnor med bröstcancer. Kvinnorna skattar fysisk funktion som försämrad vilket påverkar fysisk HRQoL negativt. Psykisk HRQoL påverkas även negativt då de upplever isolering, depression, ångest, förlust av kontroll och rädsla under bröstcancerbehandling. Kvinnorna skattar betydligt sämre social HRQoL än övrig population då de behöver avstå från dagliga aktiviteter. För att kvinnorna ska kunna uppnå god HRQoL behöver sjuksköterskor ha kunskap om vilka faktorer som påverkar HRQoL vid bröstcancer. Att även tillämpa personcentrerad omvårdnad kan vara betydelsefullt för att uppnå tillfredsställande omvårdnad och HRQoL. Förståelse för kvinnornas HRQoL är av betydelse för vägledning och stöttning genom sjukdomsprocessen.

    Abstract [en]

    Background: Chest pain is a common symptom among patients contacting the emergency medical services (EMS). Risk stratification of these patients is warranted before arrival in hospital, regarding likelihood of an acute life-threatening condition (LTC).

    Aim: To identify factors associated with an increased risk of acute LTC among patients who call the EMS due to non-traumatic chest pain.

    Methods: Several databases were searched for relevant articles. Identified articles were quality-assessed using the Scottish Intercollegiate Guidelines Network checklists. Extracted data was analysed using a semi-quantitative synthesis evaluating the level of evidence of each identified factor.

    Results: In total, 10 of 1245 identified studies were included. These studies provided strong evidence for an increased risk of an acute LTC with increasing age, male gender, elevated heart rate, low systolic blood pressure and ST elevation or ST depression on a 12-lead ECG. The level of evidence regarding the history of myocardial infarction, angina pectoris or presence of a Q wave or a Left Bundle Branch Block on the ECG was moderate. The evidence was inconclusive regarding dyspnoea, cold sweat/paleness, nausea/vomiting, history of chronic heart failure, smoking, Right Bundle Branch Block or T-inversions on the ECG.

    Conclusions: Factors reflecting age, gender, myocardial ischemia and a compromised cardiovascular system predicted an increased risk of an acute life-threatening condition in the prehospital setting in cases of acute chest pain. These factors may form the basis for prehospital risk stratification in acute chest pain.

    Pain after a stroke is a symptom often forgotten, unnoticed although it is reported to be a great problem in care. The aim of this study was to describe disability after a stroke and how long-term pain influences everyday life according to the Multidimensional Pain Inventory - Swedish language version (MPI-S) and to test the reliability of this instrument. Forty-three persons were investigated 2 years after the stroke incident: 15 with central post-stroke pain (CPSP), 18 with nociceptive pain mainly in the shoulder and 10 with tension-type headache. Data collection was performed through the MPI-S and a questionnaire regarding assistive devices, also structured interviews based on the Activities of Daily Living (ADL) staircase and the Self-report impairment questionnaire. The results show that the persons suffered moderate to severe pain. Almost half were dependent in ADL. The most often reported impairments and use of assistive devices concerned mobility and/or motion. This was most frequent in persons with nociceptive pain. There were significant differences in persons with central pain and nociceptive pain compared with tension-type headache with regard to mobility- and/or motion-related activities. No statistical differences emerged between age, gender, different types of pain and the MPI-S scales, nor any significant differences in degree of pain as between different types of pain according to the Self-report impairment questionnaire. The reliability analysis of the MPI-S shows good homogeneity in all scales except Interference, Life Control and Affective Distress. This is the first study with MPI-S on mainly older persons and on stroke patients, thus further research is needed on this instrument as well as on which specific activities evoke the pain. This is in order to offer adequate treatment, care and support to persons with pain after a stroke.

    This study describes the meaning of pain and its implications for everyday life in 35 persons with symptoms of post-polio syndrome. The mean age of the study group is 65 years and the sex ratio of men to women is 1.5:1. The study persons were interviewed on two occasions in their homes and answered a pain questionnaire. The result shows that everyday vocabulary is used to express pain experiences. The study persons normally answered that it hurt, although the interviewers used pain in their questions. The results show that the lower back is the most common location of pain. Joint pains are most common in the upper extremities. The pain is worst in the evening and at night, and tangibly affects the daily rhythm. Physical strain and climatic factors commonly provoke pain, whereas rest and heat give relief. The study show that interviews and pain questionnaire should be supplemented with questions on activities so as to gain a comprehensive view of the difficulties experienced in everyday life.

    Increasing muscular atrophy and joint instability in the post-polio syndrome (PPS) leads to muscle and joint pain. The aim of this study was to describe how persons with post-polio syndrome (PPS) perceive their pain and how the pain affects their everyday lives. The Multidimensional Pain Inventory (MPI-S) was used together with supplementary questions concerning location and verbal description of the pain. The study group comprised 37 persons with PPS who had had pain for an average of 19 years. Most commonly reported was pain from the joints of the extremities, followed by pain from the lower back. The verbal description suggests that pain is a palpable health problem for this study group. The results of the MPI-S show that women had the most pain and that the younger the persons are, the more the pain involves negative stress experiences. Activities such as outdoor work, social and other activities away from home involve difficulties, according to the results. The study group could manage their pain, experienced social support, and especially women managed to carry out household chores despite their pain. The MPI-S demonstrates acceptable reliability for this study group.

    Abstract [en]

    BACKGROUND: No study has, to our knowledge, previously been published on health-related quality of life (HRQoL) in a group suffering from long-term pain after a stroke. AIM: The aim of the present study was to describe HRQoL in persons with long-term pain after a stroke, and to compare this with different types of pain conditions, age, gender and household status. DESIGN: This study has a design combining qualitative and quantitative methods. METHODS: Forty three participants suffering from long-term pain after a stroke were included. A qualitative interview was performed and then analysed by means of latent content analysis. In addition, two self-report questionnaires, SF-36 and the Hospital Anxiety and Depression Scale (HAD Scale), were used. RESULTS: The qualitative data revealed that physical and cognitive functioning, economic security and good relationships, support and having the ability to be together with family and friends were important factors with regard to experienced HRQoL. No significant differences were found in SF-36 and the HAD Scale with regard to the different types of pain. The older age group had decreased physical functioning in SF-36. The men had more decreased vitality than the women. CONCLUSION: The results show, that the participants in this study have a lower HRQoL due to their long-term pain than those in previous studies on stroke survivors. It is evident that further research is needed with longitudinal studies and larger populations to gain more knowledge and thereby provide better supportive care. RELEVANCE TO CLINICAL PRACTICE: Awareness and understanding of the patients' perceptions and transitions with regard to their life situation and suffering from long-term pain after a stroke is important in order to support a maintained or increased HRQoL. This is also important after the acute stage and rehabilitation, including quality of life of the relatives, especially to older and dependent persons.

    The aim of this study was to describe how persons with long-term pain after a stroke experience their care. The study is part of a larger research project concerning a group persons suffering from long-term pain after a stroke. Qualitative interviews were performed with 43 persons suffering from central poststroke pain, nociceptive pain or tension-type headache after their stroke incident. Content congruence emerged among the narratives (n = 43), and therefore the most information-rich ones (n = 23) were selected for deeper investigation by means of qualitative content analysis. The results reveal the patients' need for being respected, understood and supported, also for being given adequate time and information. Accessibility and continuity in the professional contacts and with regard to medical and physical treatment was emphasized. The participants' narratives highlight the importance of the professionals having knowledge of pain and pain management.

    The aim of this qualitative study was to describe pain, coping strategies, and experienced outcome of coping with long-term pain conditions after a stroke. Forty-three participants were interviewed: 15 with central post-stroke pain (CPSP), 18 with nociceptive pain, and 10 with tension-type headache. Analysis of the data was by content analysis. Pain-related problems described were incomprehensibility regarding the pain, disturbed sleep, fatigue, diminished capacity, mood changes, and stress in relationships. Different coping strategies were used; the most common were making the pain comprehensible, planning of activities, taking medications, communicating, and distractions. Changing body position, making comparison, and enduring the pain were common in central or nociceptive pain, rest and relaxation in tension-type headache. Communicating their pain gave a feeling of perplexity and resignation. Satisfaction was reported in the cases of consideration shown by others. Pain after a stroke requires specialized knowledge in order to understand the patient's experiences and to enhance coping.

    Abstract [en]

    The aim of this study was to classify and describe the characteristics of different long-term pain conditions after a stroke by clinical examination and pain assessment using the Pain-O-Meter and a Pain questionnaire. Pain was classified as central post-stroke pain (n = 15), nociceptive pain (n = 18), and tension-type headache (n = 10). In 65%, pain onset was within 1-6 months and the pain intensity revealed individual differences. Many pain descriptors was common, some were discriminating as burning in central and cramping in nociceptive pain, and pressing and worrying in headache. More than half with central or nociceptive pain had continuous or almost continuous pain. Cold was the factor mostly increasing the pain in central, physical movements in nociceptive pain, and stress and anxiety in headache. More than one-third had no pain treatment and two-thirds of those with central pain had no or inadequate prescribed pain treatment. The clinical findings support the classification of pain and describe discriminating and common pain characteristics in pain conditions after a stroke.

    Abstract [en]

    The purpose of the study is to describe what it like to live with a highly malignant brain tumor from a family perspective. It is a qualitative study in which 3 families, 3 patients, and 5 next of kin have described their experiences in 15 interviews. The study is prospective, with interviews occurring 2-3 weeks after surgery and 3 and 6 months after the onset of the illness. Inductive content analysis has been employed. The results indicate that when a highly malignant brain tumor is diagnosed, the effect on the family is devastating and there is a state of crisis. Characteristically, there is distancing and a sense of helplessness. The members of the family live from day to day in a state of constant anxiety and fear of losing the patient. The affliction limits the patient's capacity regarding activities of daily life, which increases the burden of the next of kin. The next of kin attempt to cope with their grief by occupying themselves with practical tasks and activities that they believe are meaningful. The family members have only good words to say about their encounter with healthcare staff and about the information given. Negative information that the family have not asked for can cause a long period of frustration and anxiety, and they believe that their hope has been taken away from them.

    BACKGROUND: Three reported Swedish cases of hepatitis C in patients receiving an intravenous immunoglobulin (Gammagard, Baxter Healthcare, Deerfield, IL) were among the first to bring to light a worldwide outbreak of hepatitis C associated with non-solvent/detergent (SD)-treated Gammagard. In February 1994, all implicated batches of Gammagard were recalled and exposed patients traced. STUDY DESIGN AND METHODS: Sera from all identified and hepatitis C-viremic Swedish and Danish patients (n = 14) exposed to the implicated batches underwent hepatitis C virus genotyping and sequencing of the core region and hypervariable region 1 of E2. Genomic amplification was also done on 15 non-SD-treated batches of Gammagard. RESULTS: Twelve patients were infected with subtype 1a and surprisingly, two with subtype 2b. Analysis of the core region showed identical sequences in four patients and the only consistently positive batch. Five patients shared another sequence, whereas three other subtype 1a patients each manifested unique sequences. The two subtype 2b isolates were identical. Genomic fingerprinting of the hypervariable region confirmed identity within each group with great stringency. Amplification with isolate-specific primers showed mixed infection in one patient whose exposure was confined to a single batch. CONCLUSION: The few batches implicated presumably were contaminated with several strains.

    Bakgrund: Den svenska skolan står inför utmaningar gällande försämrade resultat i grundläggande kunskapsområden samt ofullständiga betyg. Samtidigt redovisas brister gällande elevhälsa och särskilt stöd. Det finns en möjlighet att komplettera svensk skolverksamhet med arbetsterapeutens kunskaper i form av beteendevetenskap, pedagogik och medicin, samtidigt som arbetsterapeuten ser vardagen ur ett helhetsperspektiv. Syfte: Syftet var att beskriva elevers uppfattning om arbetsterapi i skolan. Metod: Data samlades in med hjälp av en enkät som utformades av författarna för examensarbetets syfte. Enkäterna besvarades av elever som erhållit arbetsterapeutiska interventioner i skolverksamhet. Resultat: Resultatet redovisar att flera olika erhållna interventioner angavs och de erhållna interventionerna var till nytta för eleverna på ett flertal olika sätt. Nästan alla elever angav också att de i större utsträckning kunde slutföra sina studier med hjälp av aktuell arbetsterapeut. Vidare redovisas en övergripande bild av hur medvetna elever och lärare är om arbetsterapeutens roll på deras skola. Eleverna uppfattar att de har en hög medvetenhet, medan hälften av eleverna uppfattar att deras lärare är medvetna. Slutsats: De deltagande eleverna beskriver att de erhållna arbetsterapeutiska interventionerna har varit till nytta på flera olika vis. Till följd av denna beskrivning förstärks författarnas bild av att arbetsterapeuten bör ha en betydande roll inom svensk skolverksamhet.

    Abstract [en]

    Characterization of vulnerable plaques in the carotid artery is critical for the prevention of ischemic stroke. However, ultrasound-based methods for plaque characterization used in the clinics today are limited to visual assessment and evaluation of plaque echogenicity. Shear Wave Elastography (SWE) is a new tissue characterization technique based on radiation force-induced shear wave propagation with potential use in plaque vulnerability assessment. The purpose of this study was to develop an experimental setup to test the feasibility of SWE for carotid plaque characterization. A carotid artery phantom with a soft inclusion in the wall, mimicking a vulnerable plaque, was constructed (10% polyvinyl alcohol (PVA), 3% graphite) by exposing the vessel and plaque to three and one freeze-thaw cycles (6h freeze, 6h thaw) respectively. An Aixplorer SWE system (Supersonic Imagine) was used to measure the shear wave speed (cT) in the vessel wall and plaque. The Young's modulus (E) was then calculated via the Moens-Korteweg (M-K) equation. For comparison, eight cylinders (d = 4 cm, h = 4 cm) were constructed for mechanical testing from the same PVA batch, of which four were exposed to three freeze-thaw cycles (mimicking the vessel wall) and four to one freeze-thaw cycle (mimicking the plaque). The Young's moduli for the cylinders were obtained via a displacement controlled mechanical compression test (Instron 5567) by applying 5% strain. The mean shear wave speed was 2.6 (±0.7) m/s in the vessel wall, 1.8 (±0.7) m/s in the plaque, resulting in Evessel = 11.5 (±0.5) kPa, Eplaque = 4.3 (±0.5) kPa. The compression tests resulted in E = 64.2 (±11.1) kPa in the hard cylinder and E = 9.7 (±3.1) kPa in the soft cylinder. The results showed that it was possible to distinguish between the arterial wall and the plaque. The disagreement between mechanical testing and SWE can be explained by the fact that the shear wave does not propagate monochromatically in cylindrical geometry. To achieve a better calculation of the elastic modulus, the frequency dependency of the shear wave velocity must be considered.

    The categorization of occupation that is commonly used today in occupational therapy and science lacks an empirical foundation. There is no common definition or conceptualization, which limits the ability to understand and evaluate the impact of occupation on wellbeing. Research within this subject has been conducted with adults. Research about children and youths has mainly focused on specific occupational forms and/or diagnosis, and the aim of the study was to identify experience based categories of occupation from children’s perspective. Through qualitative interviews with 10 children aged 12-15, data were collected which were then analyzed using qualitative content analysis. Eight experience based categories were identified: occupations focused on manipulating time, experiencing enjoyment and satisfaction, experiencing boredom and tediousness, occupations which lead do deeper engagement, relaxation and recovery through occupations, basic occupations of everyday life, occupations on a continuum based on level of challenge, and self-identification through occupation. The categories shows connections to earlier research within the field. The presented experience based categories shows a possible way of understanding children’s experience of occupation. Further research is needed to confirm their relevance and if there is a connection to wellbeing.

    The categorizations of occupation commonly used in occupational science and therapy today lack empirical foundations for children and adolescents. Eight experience-based categories were identified: experiencing change of time perception, experiencing enjoyment and satisfaction, experiencing challenge and competence, experiencing boredom and tediousness, experiencing deeper engagement, experiencing relaxation and recovery, experiencing need, necessity or neutrality, and experiencing self-identification. The categories cover a variety of experiences, and show connections to earlier research. The findings show a possible way of understanding adolescents’ experience of occupation, and contribute to earlier research with an adolescent perspective. Further research is needed to confirm their relevance within other groups and contexts and can be used in further exploration of conceptualization.

    The aim of this study was to describe how female patients conceive integrity within health care. A phenomenographic approach was chosen to focus on women’s conceptions. The participants were 15 strategically-chosen female patients who were interviewed after discharge from a hospital in Sweden. Three description categories were identified: ‘maintaining the self’, which represented the patient’s relationship to herself; ‘dignity’, which characterized the professional caregivers’ relationship to the patient; and ‘confidence’, which was associated with the relationship between patient and professional caregivers. Integrity implied having courage to set boundaries and have control of the private sphere, but also if necessary changing the boundaries of integrity. It is essential that professional caregivers are knowledgeable about all aspects of integrity and the importance of interactions with patients being characterized by dignity and confidentiality. Professional caregivers should take part in reflective discussions to identify situations in health care with an inherent risk of threatening or violating patients’ integrity, and how best to preserve it. Integrity is an abstract and complex concept that is not well-defined, and further research is needed to clarify its connection with other concepts.

    Abstract [en]

    The overall aim of this thesis was to explore and describe women’s recovery after a first myocardial infarction (MI) from an organisational, a relational and an individual perspective. In this thesis a longitudinal, explorative and descriptive design combining both quantitative (papers I-II) and qualitative (papers III-IV) methods has been used. Data was collected from healthcare professionals at 18 acute hospitals (paper I), which on two occasions answered a questionnaire dealing with cardiac rehabilitation efforts. From these 18 hospitals, 240 women who had suffereda first MI (paper II) were consecutively chosen to answer a questionnaire on three occasions on the subject of social support and social network. Descriptive and inferential statistics were used to analyse data over time. Paper III and IV had an exploratory and descriptive design based on an inductive, qualitative content analysis approach. Interviews were conducted with 20 cardiac rehabilitation nurses (CRN) (paper III) and with 26 women suffering a first MI (paper IV).

    The results showed that patients with MI, and their next of kin, were offered a well-functioning cardiac rehabilitation on both measurement occasions. None of the hospitals offered a CRP that was specifically designed for women (paper I). The women perceived that the extent of general support, support from relatives, and professional support changed positively over time (paper II). The CRNs experienced that women’s recovery was influenced by their ability to cope with the stresses of life, if they wanted to be involved in their own personal care and how they related to themselves, and their opportunities to receive support (paper III). Women experienced that ability to approach the new perspective of life depended on how they embraced the three dimensions; behaviour, i.e. women’s acting and engaging in various activities, social i.e. how women receive and give support in their social environment, and psychological i.e. their way of thinking, reflect and appreciate life (paper IV).

    In conclusion, the four studies show that women’s recovery after a first MI is depending on factors emerging from an organisational, a relational and an individual perspective. Using knowledge from these three perspectives the possibility of a holistic approach to women’s recovery process to health will increase and the risk of a reductionist thinking will decrease.

    Background: In the developed part of the world, coronary heart disease is the major cause of death and is one of the leading causes of disease burden. In Sweden, more than 30,000 people per year are affected by myocardial infarction and out of these approximately 40% are women. Nearly 70% of the women survive and after a myocardial infarction a recovery process follows. Today’s health care focuses more on treatment, symptoms and risk factors than on the individuals’ perceptions of the recovery process. Aim: To explore cardiac rehabilitation nurses’ experiences of factors influencing female patients’ recovery after their first myocardial infarction. Methods: Twenty cardiac rehabilitation nurses were interviewed. The study was conducted using qualitative content analysis. Results: The cardiac rehabilitation nurses experienced that women’s recovery after a first myocardial infarction was influenced whether they had a supportive context, their ability to cope with the stresses of life, if they wanted to be involved in their own personal care and how they related to themselves. Conclusions: Women’s recovery after a myocardial infarction was influenced by factors related to surroundings as well as own individual factors. The underlying meaning of women’s recovery can be described as the transition process of a recovery to health. Our findings suggest that a focus on person-centered nursing would be beneficial in order to promote the every woman’s personal and unique recovery after a myocardial infarction. Finally, the cardiac rehabilitation nurses’ experiences of factors influencing male patients’ recovery after their first myocardial infarction should be important to investigate.

    Background: After a myocardial infarction and discharge from the hospital a recovery process follows for the women. In order to facilitate their recovery, both a preventive and promotive perspective should be taken into consideration. Despite this, today´s healthcare focuses more on prevention and thus research into the promotion of women’s recovery process is needed.

    Aim: To explore how women’s recovery processes are promoted after a first myocardial infarction

    Methods: The study had an explorative and descriptive design based on qualitative content analysis.

    Findings: The women’s recovery process was promoted through using external and internal resources as well as embracing behaviour, social and psychological dimensions. The women embraced these dimensions to a varying extent and this process led to them being able to take in a new perspective on life.

    Conclusions: The women’s personal recovery is a multidirectional process with a desire to develop and approach a new perspective on life. It is important for cardiac rehabilitation nurses to not only focus on lifestyle changes and social support, but also on working actively with the women’s inner strength in order to promote the personal recovery of the women. Furthermore, it would be interesting to investigate men’s experiences of how their recovery process after a first Myocardial infarction is promoted.

    Abstract [en]

    Stress has a negative impact on students' daily lives and can be associated with recurrent pain. School nurses play a key role in supporting young people with stress-related pain. The purpose of this qualitative interview study was to elucidate school nurses' experiences of encountering students with recurrent pain when practicing person-centred care. The school nurses were based at public and private schools and worked with students aged 12-19. Data were collected through interviews with 18 school nurses and analyzed with deductive content analysis. The school nurses felt that actively listening to the students' narratives about daily life with recurrent pain, and co-creation of a health plan, encouraged the students to participate as partners in their own care and strengthened their relation with the students. The application of a person-centred approach in school health care meant that traditional knowledge transfer was replaced with a dialogue that reflects both the student's and school nurse's perspective.

    Utmanande beteende kan ofta innebära aktivitetsinskränkningar för individer som lider av beteendet men även för omgivningen. Detta innebär färre möjligheter till aktivitet och delaktighet och riskerar att gå ut över livskvalitén. Idag saknas ett fokus för att utveckla arbetsterapeutiska metoder inom området och forskningen som finns är begränsad. Syftet med examensarbetet var att visa vilka arbetsterapeutiska metoder som beskrivs i litteraturen för att minska utmanande beteende. Studiedesignen som användes var scoping review. Databaserna som användes var AMED, CINAHL, MEDLINE och PsycINFO. Inklusionskriterier för artiklarna var att de var tillgängliga i fulltext, skrivna på engelska och publicerade mellan 2003 och 2018. Sökning resulterade i 124 träffar varav 13 var dubbletter. Resultatet inkluderade 20 artiklar som motsvarade syftet varav 13 var publicerade i USA. Resultatet visade en stor bredd av metoder som delades in i fyra nyckelområden: aktivitetsbaserade metoder, miljöbaserade metoder, sensoriska metoder samt stöd och strategier. Detta är ett relativt outforskat område där det finns stora möjligheter för arbetsterapeuter att bidra med sin kompetens.

    Abstract [en]

    OBJECTIVE:

    To compare the number, frequency, enjoyment and performance in extracurricular activities of siblings of children with autism spectrum disorders (ASD) to their typically developing (TD) peers, and to identify differences between actual and desired participation.

    METHODS:

    A case-control study with 30 siblings of children with ASD and 30 siblings of TD children was conducted using the Paediatric Interest Profiles and a questionnaire.

    RESULTS:

    Siblings of children with ASD participated in fewer extracurricular activities than those with TD siblings. ASD symptoms were significantly associated with the sibling participating in fewer extracurricular activities. Children with TD siblings had higher enjoyment scores in relaxation activities than children with siblings with ASD.

    CONCLUSION:

    While results were mainly positive, some differences indicated that having a sibling with ASD may impact participation in extracurricular activities. Assessments of participation barriers, as well as support to minimise participation restrictions among siblings of children with ASD are required.

    BACKGROUND:

    Conceptualization of quality of care - in terms of what individuals, groups and organizations include in their meaning of quality, is an unexplored research area. It is important to understand how quality is conceptualised as a means to successfully implement improvement efforts and bridge potential disconnect in language about quality between system levels, professions, and clinical services. The aim is therefore to explore and compare conceptualization of quality among national bodies (macro level), senior hospital managers (meso level), and professional groups within clinical micro systems (micro level) in a cross-national study.

    METHODS:

    This cross-national multi-level case study combines analysis of national policy documents and regulations at the macro level with semi-structured interviews (383) and non-participant observation (803 hours) of key meetings and shadowing of staff at the meso and micro levels in ten purposively sampled European hospitals (England, the Netherlands, Portugal, Sweden, and Norway). Fieldwork at the meso and micro levels was undertaken over a 12-month period (2011-2012) and different types of micro systems were included (maternity, oncology, orthopaedics, elderly care, intensive care, and geriatrics).

    RESULTS:

    The three quality dimensions clinical effectiveness, patient safety, and patient experience were incorporated in macro level policies in all countries. Senior hospital managers adopted a similar conceptualization, but also included efficiency and costs in their conceptualization of quality. 'Quality' in the forms of measuring indicators and performance management were dominant among senior hospital managers (with clinical and non-clinical background). The differential emphasis on the three quality dimensions was strongly linked to professional roles, personal ideas, and beliefs at the micro level. Clinical effectiveness was dominant among physicians (evidence-based approach), while patient experience was dominant among nurses (patient-centered care, enough time to talk with patients). Conceptualization varied between micro systems depending on the type of services provided.

    CONCLUSION:

    The quality conceptualization differed across system levels (macro-meso-micro), among professional groups (nurses, doctors, managers), and between the studied micro systems in our ten sampled European hospitals. This entails a managerial alignment challenge translating macro level quality definitions into different local contexts.

    Objectives: Leprosy is endemic in many countries and results in activity limitations. There is a need for assessment tools to guide professionals in their evaluation and choice of intervention in order to improve conditions for leprosy-affected people. The purpose of our study was to evaluate the concurrent validity of the Amharic version of Screening of Activity Limitation and Safety Awareness (SALSA-am) scale with Amharic version of Disability of the Arm, Shoulder and Hand (DASH-am) questionnaire.

    Design: Thirty-eight individuals with nerve damage due to leprosy completed the SALSA-am and DASH-am questionnaires. Spearman's rank correlation was used to determine relationships between SALSA and DASH scores. Specificity, sensitivity and accuracy were calculated.

    Results: There was a good correlation 0.87 (P < 0.001) between SALSA-am and DASH-am scores. Sensitivity, specificity and accuracy were calculated with acceptable results.

    Conclusions: SALSA-am is considered a useful questionnaire for determining activity limitations in persons affected by leprosy, and showed good correlation with DASH-am. The concurrent validity was considered good.

    Patients' experience of abuse in health care (AHC) is common, and may cause long-lasting suffering. In Sweden, lifetime prevalence is estimated at 20 per cent among female patients and 8 per cent among male patients, and a background of other abuse is a risk factor. Most health care staff have experience of patients who have been abused, but the topic is surrounded by silence from both patients and caregivers. Models for training staff in groups have been evaluated and a more proactive stance can be fostered. Department heads and health care authorities are responsible for providing staff with options to counteract AHC. Caregivers need training in how to carry out consultations with patients who say they have been abused in health care, and this training should be included in efforts to increase quality of care. Changes are within reach but educational efforts are urgent.

    Forskning visar att var sjätte kvinnlig och var trettonde manlig patient har upplevt kränkningar i vården. En bakgrund av övergrepp ökar risken att som vuxen uppleva sig kränkt i vården. 

    Detta samband analyseras i artikeln liksom dess orsaker och konsekvenser för patienter och vårdgivare. På basen av mångårig interventionsforskning diskuteras även hur vården på både strukturell och individuell nivå kan förhindra att patienter kränks och speciellt patienter med en bakgrund av övergrepp. 

    Ett stort ansvar vilar på vårdens alla ledningsnivåer, från myndighet till klinik, för att skapa förutsättningar i form av tid, kunskap och verktyg så att vårdgivare kan agera mot att patienter kränks. Artikeln beskriver ett antal sådana verktyg.

    Earlier we identified an Immune Risk Profile (IRP) of very old individuals, 86-94 years of age, characterised by an inverted CD4/CD8 ratio and associated with persistent cytomegalovirus infection and an increase in the numbers of CD3+CD8+CD28- cells. In the present study we included data from a population-based sample in the age range of 20-79 years to examine the prevalence of individuals with an inverted CD4/CD8 ratio relative to age and gender across the entire adult lifespan. Immunological monitoring that was conducted included analysis of the numbers of T-cells in the subsets CD3+, CD3+CD4+, and CD3+CD8+ as well as CD3+CD8+CD28+, CD3+CD8+CD28-, and CD8+CD45RA+CCR7+. There was found to be a significant lowering of the numbers of CD3+, CD3+CD4+, and CD3+CD8+, and of the CD8+CD45RA+CCR7+ cells across the adult life-span. Notably, the prevalence of individuals with an inverted CD4/CD8 ratio increased from about 8% in the age range of 20-59 years to about 16% in the age range of 60-94 years. The mortality rate in individuals with an inverted CD4/CD8 ratio also increased significantly above the age of 60. Interestingly, the proportion of individuals with an inverted CD4/CD8 ratio was found to be significantly higher in men, whereas the numbers of CD3+CD4+ helper and CD8+CD45RA+CCR7+ naive cells and the CD4/CD8 ratio were found to be significantly higher in women. These results highlight the importance of functioning of the thymus in the development of IRP and may partly account for the differences between sexes in terms of longevity.

    This paper describes the implementation of an educational intervention to enhance sexual health among homeless people by including sexual and reproductive health and rights (SRHR) as a part of social work provision with this group. Adult service users in different forms of temporary accommodation were provided with the opportunity to participate in three group sessions. Seventeen sessions, six with women and eleven with men, took place at six different housing facilities in Gothenburg. The intervention implementation process (which involved preparation, creation, realisation and evaluation) is described, and factors of importance are identified. Service users appreciated the opportunity to receive information and discuss sexual health, rights and norms. The success of the work may be related to the fact that the project was anchored both in social services and among service users, constantly adjusted, and delivered using a respectful approach. Social work organisations and professionals have an important role to play in acknowledging and promoting service users? sexual health and rights, especially among disadvantaged and socially excluded groups including homeless people.

    The clinical utility of patients’ self-rated postoperative pain after major surgery – the perspective of healthcare professionals'2017Doktorsavhandling, sammanläggning (Övrigt vetenskapligt)
    Abstract [en]

    The Numeric Rating Scale (NRS) is suitable in postoperative settings, yet, the implementation has shown varying results. This has raised issues about the pain scales contribution to the identifying and understanding of pain. The aim of this thesis was to describe the clinical utility of patients’ self-rated postoperative pain after major surgery from a healthcare professional perspective.

    The aim of study I and II was to describe healthcare professionals’ perceptions of the use of pain scales, and to through considering critical incidents describe care experiences and actions taken by healthcare professionals’ when assessing pain. Participants in study I (N=25) and II (N=24) were enrolled- registered nurses and physicians with clinical experiences of pain scales. The aims of study III and IV were to determine the clinical applicability of NRS mode- and maximum- measures, and the NRS mode- and median measures at rest and during activity based on patients self-rated pain. The aim in study IV was additionally to determine the number of NRS ratings needed for the calculation of these measures. The number of surgical and orthopedic patients who completed study III were: n=157 and study IV: n=479.

    Study I and II confirmed earlier findings of patients’ self-reported pain scores as a facilitator in the understanding of their postoperative pain. Organizational routines, documentation devices, clinical competence, continuity in care, collaborative actions, time, and individual routines were healthcare related factors affecting the use of pain scales (I, II). Patient-related facilitating factors were patients’ ability and willingness to communicate pain, while disability and unwillingness to communicate or inconsistency in verbal communication with observed behaviors were barriers (II). Time and multidimensional communication approaches could bridge these barriers (I, II).

    Study III and IV showed acceptable reliability for the mode, median and maximum measures. Rank correlations for individual median scores, based on four ratings, versus patients’ retrospective self-rated average pain, were moderate and strengthened with increased numbers of ratings. The Svensson method showed an individual variation within the expected outcome and a significant systematic group change towards a higher level of reported retrospective pain. The calculated pain measures, particularly concerning pain at rest, generally were lower than patients’ recall of pain.

    The findings described beneficial effects of patient self-reported pain, however present healthcare did not fully support the utilization of pain scales. Because of the simple measurement characteristics, the use of daily NRS average pain measures, patients’ pain can be followed until resolved. The measures could additionally become important patient reported outcome measures and thus constitute new motivators to increase the utilization of pain scales.
