    Abstract [sv]

    I samverkan mellan Jönköpings kommun och Institutet för gerontologi, Hälsohögskolan i Jönköping, genomfördes en intervjustudie parallellt med införandet av rambeslut under hösten 2015. Att respektera individens rätt till självbestämmande och inflytande är grundläggande värden och förhåll-ningssätt i svensk äldreomsorg. Rambeslut i hemtjänsten är ett salutogent arbetssätt som syftar till att i högre utsträckning stärka individens möjlighet till självbestämmande och att utöva inflytande över insatsernas utformning avseende hur och när de ska utföras. Den här studien gav viktig kunskap kring äldre personers uppfattning av begrepp som inflytande och självbestämmande och vad hemtjänsten bör fokusera på för att de äldre ska ha ett reellt inflytande och självbestämmande. Resultaten används till att utveckla äldre personers möjlighet till inflytande och självbestämmande.

    Denna rapport förmedlar 92-åriga personers upplevelser av att vara och bli mycket gammal och visar hur man kan anpassa sig till livet i så hög ålder. Därför har deltagarna i djupintervjuer ombetts berätta fritt om sina erfarenheter av dagliga livet, kroppen och hälsan, familjen, minnen från förr och hur man såg på framtiden.

    Studien är en uppföljning av intervjuer som gjordes sju år tidigare, vid 85 års ålder, med samma 41 deltagare (samt ytterligare 88 personer) varav 30 var kvinnor och 11 män. Även dementa personer deltog. Alla kvinnorna var nu ensamstående, medan 4 män fortfarande levde tillsammans med sina hustrur. Vid 92 år bodde 10 personer på någon form av institution.

    I studierna har en kvalitativ forskningsmetod - fenomenografi - använts och resultaten från båda har här jämförts. Vid analysen av intervjuerna vid 85 års ålder fram-trädde sju olika kategorier, som speglar hur man levde, upplevde och anpassade sig till livet i så hög ålder. Samma typ av analys gjordes av 92-årsintervjuerna och det visade sig att inga kategorier hade tillkommit eller försvunnit. Heterogeniteten, d.v.s. de stora skillnaderna mellan individerna, fanns alltså kvar även om kategorierna hade ändrat karaktär något.

    Det hade skett en förändring i kategoritillhörigheten så att fler nu fanns i de åt höger, d.v.s. åt det mer negativa hållet. Några hade emellertid flyttats åt vänster och knappt hälften var kvar i samma kategori som vid 85 år, vilket visar på både stabilitet och förändring i deltagarnas liv.

    Förändringarna av kategorierna och i kategoritillhörigheten kunde härledas till förändringar i deltagarnas levnadsbetingelser och då främst till hur man upplevde försämringarna av sin hälsa.

    Det mest påfallande i jämförelsen av intervjuerna var, att ingen av deltagarna upplevde att undgått åldrandets negativa verkningar vid 92 års ålder. Trots de stora individuella olikheterna, fanns mycket gemensamt hos deltagarna. Alla hade t.ex. varit med om att kroppen hade försvagats i varierande grad, vilket medförde att man kände sig osäker och inte längre hade kontroll över kroppen och därför inte heller över sitt dagliga liv.

    För att kunna hantera de svårigheter man nu mötte använde sig deltagarna av olika anpassningsstrategier med tankens hjälp, som att sänka ambitionerna, att tappa lusten för det man inte längre kunde göra och väga det som var negativt mot mer positiva erfarenheter. Även om man försökte sysselsätta sig för att inte "bli sittande" så var anpassningsstrategierna nu mindre aktiva till sin karaktär än för sju år sedan.

    Om motgångarna kommit successivt kunde de accepteras - inte sällan hänförde man dem till åldern som tog ut sin rätt - men om deltagarna drabbats nyligen hade de betydligt sämre beredskap att anpassa sig till dem.

    Kroppen satte betydligt snävare gränser vid 92 år och man upplevde framför allt att ens frihet hade inskränkts sedan man blivit beroende av hjälpmedel eller av andra personer för att förflytta sig. Det fanns dock några som fortfarande gick långa promenader och dansade varje vecka (en man) även om antalet danser var färre nu. Det var också vanligt i de flesta kategorier att man försökte sköta sin kropp med rörelser och hälsosam mat, paradoxalt nog även personer som sagt att de inte ville leva längre.

    Hur man upplevde sin hälsa varierade mycket både mellan individerna och inom en och samma dag. För det mesta kände sig dock deltagarna varken helt friska eller helt sjuka utan som något däremellan - begreppet hälsa tycktes ha fått en annan innebörd.

    Livet nu hade därför mest kommit att fokuseras på hemmet, det vardagliga och familjen (främst barnen). Många jämnåriga anhöriga och vänner hade avlidit och att vara den ende eller äldste kvar i släkten medförde en mycket egendomlig ensamhets- och tomhetskänsla.

    De flesta kunde inte själva klara de dagliga sysslorna och en fjärdedel hade därför flyttat till sjukhem eller servicehus med helinackordering. Av de hemmabonde fick knappt hälften hjälp av hemtjänsten, och de barn som bodde någorlunda nära hjälpte föräldrarna regelbundet. Eftersom flera hade svårt att hålla sig sysselsatta med läsning eller handarbete fungerade det hushållsarbete man fortfarande kunde utföra som ersättning. Några, om än få, klarade sig utan hjälp alls och ytterligare några hjälpte andra; gick ut med sonens hund, skötte om tvätt eller tog ut vänner med gångsvårigheter på promenad.

    Hälsans och kroppens oberäknelighet medförde att ordet inte var mycket vanligt när deltagarna skulle beskriva vad de brukade göra; orkar inte, vågar inte, kan inte, vill inte och inte nu. Nuet verkade flera uppleva som mindre verkligt, eftersom de mentalt inte helt hade lämnat den tid när de förmådde göra mer samt för att de inte såg eller hörde verkligheten nu så tydligt eller kunde röra sig obehindrat i den.

    Ett annat ord man ofta använde i intervjusvaren var men. Det fungerade som en förbindelselänk när man vägde negativa erfarenheter mot positiva för att inför sig själv komma fram till ett accepterande av livet, både som det var nu och som det varit tidigare. Denna anpassningsstrategi var vanlig också vid 85 år och jag kallar den "åldrandets dialektik".

    Det faktum att man inte kunde förvänta sig att leva speciellt många år till var en livsbetingelse som var gemensam för alla deltagarna. Det framgick tydligt att man inte längre ansåg att man kunde prata om en framtid i egentlig mening, i motsats till 85-årsintervjun då man önskade att framtiden skulle se ut som livet gjorde för tillfället. Framtiden vid 92 år var istället synonym med en inväntan av döden. Detta hade man också hade accepterat, inte uppgivet utan på grund av att man kände en livsmättnad - man var ganska nöjd med vad man upplevt i sitt mycket långa liv och den tiden som eventuellt var kvar trodde man inte kunde medföra mycket positivt. Det korta framtidsperspektivet och den opålitliga hälsan medförde att nästan alla deltagarna berättade att de inte planerade framåt utan nu tog en dag i taget för att bl.a. kunna uppleva någon form av kontroll över sina liv. Tiden, tyckte de flesta, gick fort och till och med väldigt fort för en del. Det verkade inte bero på hur aktiv man var, utan huruvida man upplevde att man var sysselsatt. Ensamhet och långsamhet var nära kopplade till varandra - kände man sig ensam, gick tiden också långsamt.

    Drygt en tredjedel av deltagarna berättade spontant om sitt dåliga minne och sina svårigheter att förstå. De kunde ingående beskriva hur det var att inte komma ihåg, förstå vad de läste, vad de hörde eller hur man skulle utföra praktiska sysslor, t ex i köket. Några var mycket förtvivlade men andra använde olika strategier för att försöka klara av sådana situationer. Intresset för omvärlden fanns kvar hos de flesta. Några var mycket engagerade i politik både i och utanför Sverige men det var också vanligt att man hade väldigt svårt att se mänskligt lidande i krig och svält på TV.

    Uppfattningarna om hur det var att åldras i Sverige var oftast tvetydiga man hade en positiv inställning till sina privata förhållanden (man var nöjd med den hjälp man fick) och en övervägande negativ om de allmänna samhällsförhållandena för äldre. Man oroade sig för vad man läst och hört om indragningar i verksamheter som berörde äldre och flera var upprörda över hur dåligt man tyckte politiker och tjänstemän skötte sig.

    Långt ifrån alla kände sig gamla vid 92 års ålder. De som inte gjorde det (ungefär hälften och i de främre kategorierna), trodde att det berodde på att de fortfarande var nyfikna och intresserade av omvärlden. De övriga, som kände sig gamla ibland eller ständigt, menade att det var för att de upplevde kroppen, främst benen, som ett hinder. Den egna identiteten påverkades dock inte av om man kände sig gammal eller inte - det var ingen som beskrev sig som gammal på frågorna som handlade om självuppfattning.

    Kompetensbehov och kompetensförsörjning inom informationssäkerhet från ett samhällsperspektiv2015Report (Other academic)

    På uppdrag av Myndigheten för samhällsskydd och beredskap (MSB) har en studie genomförts med syftet att komplettera resultatet från en tidigare genomförd förstudie (Åhlfeldt m.fl., 2014) med en analys av kompetensförsörjning och kompetensbehov på informations­säkerhetsområdet från ett samhällsperspektiv. Arbetet har genomförts av forskare från två lärosäten, Högskolan i Skövde och Karlstad Universitet, samt inom tre forskningsdiscipliner: pedagogik, informationssäkerhet och företagsekonomi.

    Uppdraget har varit att besvara följande frågeställningar:

    • Vilka är kompetensbehoven för att ha en god och balanserad informationssäkerhet som bidrar till samhällets informationssäkerhet?
      • Samtida kompetensbehov (nuläget)
      • Framtida kompetensbehov
      • Hur ska nödvändig kompetens erhållas och på vem ligger ansvaret?
      • Utifrån ovanstående frågeställningar, vilka är de viktigaste framgångsfaktorerna?

    Arbetet har genomförts i form av fokusgrupper med representanter från myndigheter och företag som har en nära verksamhetskoppling till samhällets informationssäkerhet och som är viktiga för att samhällets informationssäkerhet ska fungera.

    Resultatet visar att det finns stora brister avseende informationssäkerhetskompetens på alla nivåer i samhället. Tre tydliga områden pekas ut 1) nationellt - ökat behov av starkare styrning och ledning samt kravställning 2) organisation - ökat behov av kompetens från ledning till medarbetare men med starkt fokus på kompetenshöjande åtgärder på ledningsnivå samt vid upphandling och 3) medborgarperspektivet där framförallt skolområdet lyfts fram som ett viktigt insatsområde för kompetenshöjande åtgärder.

    För att uppnå nödvändig kompetens krävs utbildningsinsatser på alla ovan angivna områden. Dels utbildningar på akademisk nivå för informationssäkerhetsexperter men även övriga utbildningar inom t ex juridik och ekonomi. Även yrkesverksamma på organisationsnivå behöver riktade kompetenshöjande åtgärder som sätter informationssäkerhet i fokus utifrån organisationens verksamhetsbehov, allt ifrån ledningsnivå till medarbetarnivå.

    Resultatet visar även att ansvaret för samhällets kompetensförsörjning för informationssäkerhet ligger även den på alla ovan nämnda tre områden men med tydlig betoning på nationell nivå. Här betonas behovet av nationella krav för att medvetandegöra och lyfta informations­säkerheten i samhällsviktig verksamhet för att nå så många medborgare som möjligt.  

    Förslag på framtida arbete avseende utveckling av metoder för framtida studier av kompetensförsörjningen pekar främst på metoder för att angripa bristen på helhetssyn samt kompetensförsörjning för management och medborgare.

    Syftet med avhandlingen är att analysera framtidsverkstäder som fenomen föratt stärka äldre människors delaktighet och engagemang. Analysen sätts irelation till teorier och begrepp kopplat till medborgarengagemang ochdelaktighet i olika processer inom ramen för människors vardagsliv.

    Avhandlingen baseras på ett empiriskt material från olika projekt där metodenframtidsverkstad har använts. Framtidsverkstäder är en pedagogisk metod föratt stödja människor till att mötas på ett kreativt och konstruktivt sätt i syfte attarbeta med gemensamma angelägenheter.

    Den metodologiska ansatsen för analysen beskrivs i två steg. I det första stegetses framtidsverkstäderna som beskrivs i artiklarna I, II och III som exempel påen utvecklingsinriktad forskningsansats. I det andra steget görs i avhandlingensramberättelse en återanalys av resultatet i artiklarna utifrån fallstudiemetodik därframtidsverkstäderna beskrivs som enskilda fall.

    Avhandlingens resultat diskuteras utifrån att begreppet delaktighet sätts irelation till begrepp som gemenskap, ansvar, brukarinflytande ochmedborgarskap. I analysen görs en koppling till socialpedagogiska teorier ochforskning kring civilsamhället.

    Avhandlingens resultat bekräftar att äldre människors delaktighet utifrån envardagslivskontext ska ses som värdefull resurs i samhället och att det är viktigtatt denna resurs lyfts fram och tillvaratas. Det handlar inte bara om att äldremänniskor mår bra av att vara aktiva, träffa andra och fortsätta ha ansvar. Det handlar lika mycket om att samhället är i behov av äldre människors kunskap,erfarenhet och kompetens. Äldre människor, liksom människor i andra åldrar,är både konsumenter och producenter av olika samhällstjänster. För attmotverka ålderism i samhället är det viktigt att lyfta fram denna mer positiva bild av åldrandet.

    Abstract [en]

    The aim of this article is to examine the role of children’s awareness of the function of language use in expressing understanding. Preschool and elementary school children were asked two questions presenting a problem that could be described in classical mechanics and basic astronomy. They were then encouraged to reflect on their own language use when expressing their conceptions of the physical phenomena they had discussed. The article concerns learning from a phenomenographic perspective, with emphasis on the interplay between language use and meaning-making when developing understanding. Understanding is seen as an activity, while awareness of language use in understanding is regarded as an important aspect. 40 children in preschool and elementary school (six and ten years old respectively) participated in an empirical, qualitative investigation. A particular dialogue format was used, that has been developed within the intentional-expressive approach. Dialogue excerpts where the children expressed awareness of their own language and understanding were selected from the empirical material, and contextual analysis was used to delimit qualities of awareness expressed in these excerpts. The qualities of awareness were grouped in four descriptive categories: 1) awareness of the relation between language form and language meaning; 2) awareness of the nature of understanding; 3) awareness of the expressive function of language for new meaning; and 4) awareness of the function of language use in understanding. A widened definition of language awareness is proposed and discussed, in which awareness of the close relationship between language use and meaning-making is taken into consideration.

    Abstract [en]

    Aim: The overall aim of this thesis was to describe and explore the use and content of ICU-diaries and to develop and psychometrically test a questionnaire to detect physical and psychosocial problems for ICU patients in their recovery process.

     Methods: This thesis is based on four studies. Study I had an explorative descriptive design with a quantitative and qualitative approach. Data were collected by telephone interviews with staff at Swedish ICUs (n = 65) which kept ICU-diaries. One question “what was the purpose of keeping ICU-diaries” was analysed with latent content analysis, and the other data were analysed with descriptive and comparative statistics. Study II had an explorative descriptive cohort design with a concurrent mixed method approach. The sample in study II was a part of the sample in study IV in which 421 former ICU patients responded to a new developed questionnaire 3-set 4P two months after discharge from ICU. Patients from this sample who have had an ICU-diary (n = 115) responded to a questionnaire six months after discharge from ICU. Fifteen patients were interviewed about the content and usefulness of the ICU-diary. Data were analysed with descriptive statistics, descriptively by content and interviews with manifest content analysis and then combined at the interpretive level to seek convergence, as enable by the mixed method approach. Study III had a methodological design. In this study, the questionnaire 3-set 4P was developed and psychometrically tested in a pilot setting. In study IV, the questionnaire was further developed and tested based on psychometric evaluation of the 3-set 4P. In study III the questionnaire was responded by 39 patients and in study IV by 421 patients. Data in study III and IV were analysed with descriptive statistics and psychometrical tests.

     Results: The main purpose for keeping ICU-diaries was to provide a tool in the recovery by helping the patient remember and give time back. Keeping ICU-diaries was common although there was a difference in practice and patient recruitment among different hospitals (study I). An ICU-diary with content and photos in a chronological order describing the whole picture of critical illness and ICU stay could be a tool for the patient to construct a coherent individual story. The ICU-diary could be one piece to give a deeper understanding and meaning in the personal story and to give a realistic expectation of the recovery process. Absence of guidelines for keeping ICU-diaries could affect the possibility for the ICU-diary to be a helpful tool during the recovery process (study II). In study III, the 3-set 4P was developed to be used for identifying and evaluating former ICU patients’ physical, psychosocial problems and outcome during follow-up. The psychometrical tests showed acceptable validity and internal consistency reliability. The stability reliability was acceptable in two of three sets. The psychometrical tests of the further modified version of 3-set 4P in study IV showed good construct validity and internal consistency but it needs some modification before it can be used in clinical practice (study IV).

     Conclusion: Recovery can be a difficult process where different tools can be useful. Today there is no evidence about tools to use during follow-up. To promote high quality of the follow-up there is a need for evidence-based guidelines. The ICU-diary is one tool but this thesis shows that guidelines for keeping ICU-diaries have to be developed to meet the patients’ wishes in order for the ICU-diary to become a useful tool during the process to recovery. The 3-set 4P can after some modification be used at the follow-up clinic to identify the individual patient’s problems and create an individual program for recovery.

    Many ICU-patients have memory-gaps which may affect their recovery. A tool in the recovery can be an ICU-diary to explain and clarify thoughts and events from the ICU-period. There are different standards for the content in the ICU-diary. The aim of this study was to identify the preferred content and usefulness of an ICU-diary as described by ICU-patients.

    Method: a descriptive, exploratory cohort design with a mixed method approach. The patients answered a questionnaire (n=115) and participated in an interview (n=15) six months after the ICU-stay. Data analysis was carried out in three stages; the questionnaire was analysed by descriptive statistics and categorized by content (four open-ended questions) and the interviews were analysed by manifest content analysis.

    Results: The patients explained that detailed information about daily activities and medical facts had to be included to understand and give a sense of coherence of what had happened. The content in the ICU-diary had to be chronological in order to follow the process in which photos were an important part. The patients re-read the ICU-diary during the recovery which helped them to fill in the memory gaps and used it as a tool for communication.

    Conclusion; To construct a coherent story, it was essential that the ICU-diary was complete and were amplified by photos, all appearing in a chronological order. The results of this study could form a basis for further developments of standards and guidelines for ICU-diaries

    Background: Current studies reveal a lack of consensus for the evaluation of physical and psychosocial problems after ICU stay and their changes over time.

    Objectives: The aim was to develop and evaluate the validity and reliability of a questionnaire for assessing physical and psychosocial problems over time for patients following ICU recovery.

    Patients: Thirty-nine patients completed the questionnaire, 17 were retested.

    Methods and results: The questionnaire was constructed in three sets: physical problems, psychosocial problems and follow-up care. Face and content validity were tested by nurses, researchers and patients. The questionnaire showed good construct validity in all three sets and had strong factor loadings (explained variance >70%, factor loadings >0.5) for all three sets. There was good concurrent validity compared with the SF 12 (rs > 0.5). Internal consistency was shown to be reliable (Cronbach's α 0.70–0.85). Stability reliability on retesting was good for the physical and psychosocial sets (rs > 0.5).

    Conclusion: The 3-set 4P questionnaire was a first step in developing an instrument for assessment of former ICU patients’ problems over time. The sample size was small and thus, further studies are needed to confirm these findings.

    Abstract [en]

    This is a further development of a specific questionnaire, the 3-set 4P, to be used for measuring former ICU patients’ physical and psychosocial problems after ICU and the need for follow-up. The aim was to psychometrically test and evaluate the 3-set 4P questionnaire in a larger population. The questionnaire consists of three sets: “physical”, “psychosocial” and “follow-up”. The questionnaires were sent by mail to all patients with >24 hour length of stay at four ICUs in Sweden. Construct validity was measured with exploratory factor analysis with varimax rotation resulting in “physical set” three factors, “psychosocial set” five factors and “follow-up set” four factors with strong factor loadings and a total explained variance of 62 - 77.5%. Thirteen questions in the SF-36 were used for concurrent validity showing Spearman’s rs 0.3-0.6 in eight and <0.2 in five questions. Test-retest was used for stability reliability. In set follow-up the correlation was strong to moderate and in physical and psychosocial sets the correlations were moderate to fair. This could be due to that the physical and psychosocial status changed rapidly during the test period. All three sets had good homogeneity. In conclusion, the 3-set 4P showed overall acceptable results, but it has to be further modified in different cultures before being an instrument which may be fully operational in clinical practice.

    Aims and objectives: To describe and compare the extent and application of patients' diaries in Sweden.

    Background: Since 1991, patient diaries have been used in intensive care unit (ICU) follow-up in Sweden. There is paucity of relevant data evaluating the effect of this tool and also on what premises patients are enrolled. Likewise, data are sparse on the diaries' design, content structure and the use of photographs.

    Design: Descriptive explorative design by a semi-structured telephone interview.

    Methods: The interview results were analysed with descriptive statistics and differences between the ICU levels were explored by χ2 analysis. Qualitative manifest content analysis was performed to explore the purpose of diary writing.

    Results: Of all ICUs (n = 85), 99% responded and 75% used diaries. The source of inspiration was collegial rather than from scientific data. The main reason for keeping a diary was to help the patient to recapitulate the ICU stay. Discrepancies between the different levels of ICUs were detected in patient selection, dedicated staff for follow-up and the use of photographs. Comparison between the χ2 analysis and the content analysis outcome displayed incongruence between the set unit-goals and the activities for achievement but did not explain the procedural differences detected.

    Conclusion: The uses of diaries in post ICU follow up were found to be common in Sweden. A majority used defined goals and content structure. However, there were differences in practice and patient recruitment among the levels of ICUs. These discrepancies seemed not to be based on evidence-based data nor on ongoing research or evaluation but merely on professional judgement. As ICU follow-up is resource intense and time consuming, it is paramount that solid criteria for patient selection and guidelines for the structure and use of diaries in post-ICU follow-up are defined.

    Type 1 diabetes (T1D) and coeliac disease are both characterized by an autoimmune feature. As T1D and coeliac disease share the same risk genes, patients risk subsequently developing the other disease. This study aimed to investigate the expression of T helper (Th), T cytotoxic (Tc) and regulatory T cells (Treg) in T1D and/or coeliac disease children in comparison to healthy children. Subgroups of T cells (Th:CD4+ or Tc:CD8+); naive (CD27+CD28+CD45RA+CCR7+), central memory (CD27+CD28+CD45RA-CCR7+), effector memory (early differentiated; CD27+CD28+CD45RA-CCR7- and late differentiated; CD27-CD28-CD45RA-CCR7-), terminally differentiated effector cells (TEMRA; CD27-CD28-CD45RA+CCR7-) and Treg (CD4+CD25+FOXP3+CD127-) cells, and their expression of CD39, CD45RA, CD101 and CD129, were studied by flow cytometry in T1D and/or coeliac disease children or without any of these diseases (reference group). Children diagnosed with both T1D and coeliac disease showed a higher percentage of TEMRA CD4+ cells (P<0·05), but lower percentages of both early and late effector memory CD8+ cells (P<0·05) compared to references. Children with exclusively T1D had lower median fluorescence intensity (MFI) of forkhead box protein 3 (FoxP3) (P<0·05) and also a lower percentage of CD39+ and CD45RA+ within the Treg population (CD4+CD25+FOXP3+CD127-) (P<0·05). Children with exclusively coeliac disease had a higher MFI of CD101 (P<0·01), as well as a higher percentage of CD129+ (P<0·05), in the CD4+CD25hi lymphocyte population, compared to references. In conclusion, children with combined T1D and coeliac disease have a higher percentage of differentiated CD4+ cells compared to CD8+ cells. T1D children show signs of low CD39+/CD45RA+ Treg cells that may indicate loss of suppressive function. Conversely, children with coeliac disease show signs of CD101+/CD129+ Treg cells that may indicate suppressor activity.

    Objective

    To explore the relationship between certain clinical variables and metabolic HbA1c at diagnosis correlated to HbA1c at follow-up (p < 0.001). There was a clear gender difference regarding HbA1c. Girls had higher values both at diagnosis and at follow-up (p < 0.001). Girls also had lower BMI and pH at diagnosis than boys (p < 0.001). In contrast, girls with the highest body mass index (BMI) at follow-up had higher mean HbA1c at follow-up in 2010 (p < 0.001). Having a mother and/or a father with high BMI implied higher HbA1c at diagnosis (p < 0.003).

    Conclusions

    HbA1c at diagnosis seems to predict metabolic control years later. There is a gender difference at diagnosis as female patients have higher HbA1c than males at diagnosis as well as at follow up. As metabolic control is very much correlated to complications there is a need to early identify patients at risk of poor metabolic control. Even though we do not know whether a high HbA1c level is mainly due to severity of the disease or to behavioral patterns, new ways to treat and support these children, especially girls, are needed.

    The purpose of the study is to investigate how primary school teachers understand the concept of research-based teaching. The data is collected through qualitative research interviews and analyzed via life-world phenomenology. The study's overall conclusion is that teachers are complacent about research-based knowledge or exclude it from their teaching. The result shows five different dimensions; external, internal, relational, conscious critical and unconscious. The two categories of internal and relational dimension can characterize the meaning of teacher as researcher. These two categories show teachers understanding and being an example that a practice paradigm of research-based teaching is under development.

    Increasing customisation of products results in decreasing production batch sizes, especially in the final assembly. Industry must therefore increase their capability to handle smaller batches as well as radically decrease set up time between different product groups and new products. This paper suggests the need for further development, primarily addressing time parameters in dynamically changing assembly systems. We propose customer demands and decrease non value-adding tasks. In proactive assembly systems, the full and complementary potential of human operators and technical systems is utilised. Criteria for proactivity in assembly systems are reviewed from automation, information, and competence perspectives.

    Abstract [en]

    Aim: To culturally adapt and evaluate the psychometric properties of the Young Children's Participation and Environment Measure (YC-PEM) for use by caregivers of Swedish children with and without disabilities, aged 2–5 years.

    Methods: Thirteen cognitive interviews and two focus groups with caregivers of children with and without disabilities were conducted to evaluate the cultural relevance of YC-PEM content for use in Sweden. Per participant feedback, a revised version of the Swedish YC-PEM was created and pilot tested with caregivers of children with disabilities (n = 11) and children with typical development (n = 22).

    Results: User feedback informed content revisions to 7% of items. Internal consistency estimates of the Swedish YC-PEM pilot version were acceptable and ranged from .70 to .92 for all but two of the YC-PEM scales. Mean percentage agreement between raters ranged from 47% to 93% across YC-PEM scales for inter-rater, and 44% to 86% for test-retest. One of twelve YC-PEM scales revealed significant group differences between young children with and without disabilities.

    Conclusions: This study contributes preliminary evidence for the use of some scales within a culturally adapted YC-PEM in Sweden. Further validation with larger samples will allow for parametric testing to evaluate its psychometric properties.

    Bu çalışma, büyük veri analizini sosyal medya araştırmalarında kullanımı için yöntem önerisinde bulunmaktadır. Büyük veri araştırmalarında, tümden gelim ya da tüme varım yerine alternatif olarak “abduction” yaklaşımı kullanılabilir (Kitchin, 2014). Bu yaklaşımda veri, en iyi açıklamanın çıkarılması açısından araştırmanın başlangıcından sonuçlanmasına kadar geçen her aşamada dikkat edilmesi gereken önemli bir unsurdur. Sosyal medya araştırmalarında veri hacmi büyüklüğü ve verinin oluşum hızının yüksekliği sebebiyle, büyük veri analizi benimsenen yöntemler arasında öne çıkmaktadır (Jacob, 2009). Bu bağlamda, sosyal medya araştırmalarında analitik ve yapısal bir yöntemin takip edilmesi, araştırmaların ve çıktılarının bilimsel niteliğinin korunması açısından önem taşımaktadır. Çalışmamızın amacı, sosyal medya analitiğine örnek oluşturmak ve özellikle Twitter analizi için büyük veri analizine dayanan bir araştırma yöntemi önerisinde bulunmaktır.
