    Abstract [en]

    Background Experience of balance in everyday activities where work is an essential part is important to health and well-being, as has also been observed in previous studies in people with rheumatoid arthritis (RA). The Valued life activity scale (VLA-swe) is a questionnaire in which patient’s first report if the separate activities are valued or not to perform and secondly difficulties to perform these activities. Occupational Balance Questionnaire (OBQ) focuses on satisfaction with the amount and variation of occupations.

    Objectives The objectives were to 1) describe the relationship between performance of valued activities and experienced occupational balance, and to 2) identify aspects associated with low occupational balance in persons with RA.

    Methods 368 persons (age 18–65 years, 77% women) with RA responded to a questionnaire measuring occupational balance (OBQ) and performance of valued life activities (VLA-swe). Other aspects of interest were activity limitations measured by Health Assessment Questionnaire (HAQ), pain (measured by VAS), continuous stress (stressed continuously for more than a month during the last 12 months), children at home, education, and living situation. The relation between OBQ and performance in VLA across genders and Workers/Non-workers were analysed using non-parametric correlation analyses. To identify the impact of different aspects on the likelihood that participants would report lower occupational balance, OBQ was analysed using workers/nonworkers, stress, gender, age, pain and difficulties performing valued activities as independent variables in logistic regressions models. The study was approved by the Regional Ethics Committee (Dnr2011/452–31).

    Results The OBQ was significantly related to difficulties to perform valued activities reported by VLA (r=-0.41, p<0.001). Having more difficulties performing valued activities was the strongest predictor of lower occupation balance and increased the risk of reporting lower occupation balance with nearly five times (OR=4.54, p 0.001). Continuous stress increased the risk of having lower occupation balance more than three times (OR=3.27, p<0.0001) than those who not reported being stressed. The other variables show no significant impact on the likelihood that the participants would report lower occupational balance.

    Conclusions The results showed support for the relationship between occupation balance and performance of valued life activities and highlights to identify what’s important for the individual and to assume that in the rehabilitation. The results also show the importance of ability to manage stress, in order to enable for retaining ability to work and achieve high occupational balance.

    Abstract [en]

    Background. Recently, the Occupational Balance Questionnaire developed in Sweden was translated into Norwegian. No studies to date have examined the measurement properties of the Norwegian version of this questionnaire.

    Aim. The study is aimed at examining the psychometric properties of the Norwegian version of the Occupational Balance Questionnaire, the OBQ11-N.

    Methods. Along with sociodemographic data, 180 occupational therapy students enrolled at two Norwegian universities completed the OBQ11-N as well as one question each related to health and quality of life and some sociodemographic variables. Rasch analysis was employed for examining rating scale functioning, item and person validity, dimensionality, and differential item functioning.

    Results. Item categories were ordered, but there were potential gaps in the measurement of the construct. Person reliability was fair, whereas item reliability was low. Point biserial correlations were positive, indicating that all items contributed to the construct. Factor loadings were low for two items, and there were indices of a second underlying dimension and item redundancy. Many people were not aligned with the items, and some items functioned differently across various demographic variables.

    Conclusion and Significance. The OBQ11-N did not function as an adequate measure of occupational balance in a sample of students. Potentially, the detected measurement problems may be solved by adding more relevant items to a larger item pool, from which the best fitting items should be selected.

    Abstract [en]

    Background:

    Parents with small children constitute a vulnerable group as they have an increased risk of sick leave due to stress-related disorders compared to adults without children. It has been shown that mothers and fathers to small children together spend more time in paid work than any other group, which could create negative stress and an experience of low occupational balance.

    Aim:

    The aim of this study was to examine associations between organisational factors and occupational balance among parents with small children in Sweden.

    Methods:

    Data were collected by a survey including questions about occupational balance, organisational factors and age, sex, employment rate, work position, monthly household income, number of children at home, separation/divorce last five years and overtime. The total number of parents included in this study was 718 (490 mothers and 228 fathers). Logistic regression models were applied to examine the odds ratios for occupational balance in relation to organisational factors.

    Results:

    Parents who experienced positive attitudes towards parenthood and parental leave among colleagues and managers were more likely to experience high occupational balance than parents who experienced negative or neutral attitudes. Having a clear structure for handover when absent from work was also strongly associated with high occupational balance.

    Conclusions:

    The result of the present study indicates that some organisational factors could be important for the occupational balance of parents with small children.

    Introduction

    Occupational balance (OB) is related to many health indicators, including quality of life. The Occupational Balance Questionnaire (OBQ11) was developed to measure OB, and to date, no Arabic translations of the questionnaire exist. The aim of the study was to describe the translation process of OBQ11 to Arabic and to evaluate its content validity, internal consistency, construct validity, and convergent validity.

    Methods

    OBQ11 was translated to Arabic (OBQ11-A) following standard guidelines. Content validity feedback was obtained from ten Arabic-speaking occupational therapists and in a cognitive debriefing with seven volunteers. In addition, OBQ11-A and the Family Quality of Life Survey-2006 were administered to 67 Arabic-speaking participants from a larger study about OB among parents.

    Results

    High level of agreement was found on the content of OBQ11-A from the occupational therapists. Cognitive debriefing interviews indicated that OBQ11-A was easy to understand. Cronbach?s alpha for the total OBQ11-A score was 0.864 indicating good internal consistency. Exploratory factor analysis showed acceptable factor loadings for all items. The total scores showed positive statistically significant associations with Family Quality of Life Scores (r = 0.561, p < 0.001).

    Conclusions

    OBQ11-A may prove useful for assessing OB in Arabic-speaking populations. Further research is needed to establish its reliability.

    BACKGROUND: Occupational balance is a central concept in occupational therapy and is commonly defined as an individual's perception of having the right amount and variety of occupations in daily life. This individualistic and subjective nature of occupational balance brings with it certain challenges when applied in practice. The aim of this discussion paper is to highlight the challenges associated with the lone application of individualistic occupational balance in practice situations, and propose some way forward.

    METHODS: The Meleis' Theory to Practice to Theory strategy was used. Practice-based vignettes were used as a point of departure to discuss these challenges. This was followed by theorising the importance of including the others' perceptions of a person's pattern of daily occupations.

    CONCLUSION: Besides a goal of increasing a person's occupational balance in clinical practice, striving towards creating a suitable pattern of daily occupations from a community perspective also seems to be a relevant goal.

    Background: People with psychiatric disabilities often find it difficult to maintain a satisfactory occupational balance.

    Aims/objectives: This study aimed to investigate the validity and reliability of the Turkish version of the Occupational Balance Questionnaire (OBQ11-T) in mental health.

    Material and methods: OBQ11-T was applied to 149 people for construct validity analysis and to 61 of them for reliability analysis. Validity was determined using factor analyses. The reliability of the OBQ between the first and second evaluations was assessed using the intraclass correlation coefficient (ICC) for each item and the total OBQ11-T score. Internal consistency was assessed using Cronbach’s alpha.

    Results: The results of factor analysis revealed one factor that explains 35.94% of the total variance in the model. OBQ11-T item 1 had the lowest and OBQ11-T item 4 had the highest factor loadings. The model fits the data according to the indices of relative fit (RMSEA = 0.087, CMIN/DF = 2.129, CFI = 0.901). There was an excellent correlation between test and retest OBQ11-T total scores (ICC = 0.905). All items of the OBQ11-T showed good reliability. Cronbach’s alpha for the OBQ11-T total score was 0.839, indicating acceptable internal consistency.

    Conclusions and significance: The current study showed that OBQ11-T is a valid and reliable tool for measuring the self-rated occupational balance of people with mental illness.

    Occupational therapists need to be cognizant of evidence-based role balance advice and strategies that women with multigenerational caring responsibilities can implement independently or with minimal assistance, as role balance may not be the primary goal during many encounters with this population. Hence, this study aimed to identify the viewpoints on the most helpful role balance strategies for working sandwich generation women, both from their own perspectives and from the perspective of occupational therapists. This was achieved through a Q methodology study, where 54 statements were based on findings from interviews, sandwich generation literature and occupational therapy literature. In total, 31 working sandwich generation women and 42 occupational therapists completed the Q sort through either online or paper administration. The data were analysed using factor analysis with varimax rotation and were interpreted through collaboration with experts in the field. The findings revealed similarities between working sandwich generation women and occupational therapists, particularly in terms of advocating strategies related to sleep, rest and seeking practical assistance from support networks. Differences were also present, with working sandwich generation women viewpoints tending to emphasize strategies related to coping with a busy lifestyle attending to multiple responsibilities. In contrast, occupational therapy viewpoints prioritized strategies related to the occupational therapy process, such as goal setting, activity focused interventions, monitoring progress and facilitating sustainable outcomes.

    Abstract [en]

    BACKGROUND: Occupational balance is a central aspect in occupational therapy. Awareness of occupational balance/imbalance among occupational therapists could influence the orientation of their professional interventions.

    OBJECTIVE: To describe and compare the occupational balance status of occupational therapists working in Spain.

    MATERIAL AND METHODS: Cross-sectional descriptive study using an online questionnaire. Participants were occupational therapists working in Spain who answered a questionnaire including 22 questions on the sociodemographic and employment data and the Spanish version of the Occupational Balance Questionnaire (OBQ-E) ranging 0-65 where higher ratings equal better occupational balance.

    RESULTS: A total of 648 currently working occupational therapists participated. Mostly women, with median age of 32 years, without partner, children, or dependents. The median OBQ-E was 37 (IQR = 27; 45). Statistically significant differences were found between the medians of participants varying in the categories 'hired as an occupational therapist', weekly work hours, and population working with children and adults.

    CONCLUSIONS AND SIGNIFICANCE: Spanish occupational therapists presented a moderate occupational balance that varied between subgroups. Professional recognition, working hours, and the population they attend are aspects that influence their occupational balance. Knowledge of occupational therapists' occupational balance could contribute to the development of policies aimed at promoting it.

    Abstract [en]

    BACKGROUND: Stress-related disorders cause suffering and difficulties in managing occupations and relationships in everyday life. A previous study of women with stress-related disorders, who photographed well-being and talked about the photographs in interviews, showed that moments of well-being still exist but further knowledge is needed about their perceptions of participating in such a study.

    AIM: To describe how people with stress-related disorders experience taking photographs related to well-being in everyday life and reflecting on and talking about these photographs.

    MATERIAL AND METHODS: Twelve women, 27-54 years with stress-related disorders were recruited from primary healthcare centres. They participated in interviews based on the photographs and qualitative content analysis was used.

    RESULTS: One theme 'catching a glimpse of and momentarily capturing well-being' and four categories were identified: 'Photographing moments of well-being visualizes well-being', 'Using photographs could either facilitate or complicate the narrative', 'Changing perspective in everyday life' and 'Creating recollections of well-being'.

    CONCLUSIONS AND SIGNIFICANCE: Introducing a focus on well-being in everyday life despite living with a stress-related disorder might contribute a valuable complement to stress rehabilitation for occupational therapists and other health professionals. Using photographs as a basis for reflections about everyday life and health/well-being also seems positive for this group.

    Bakgrund: Vid depression och ångest försvåras förmåga att klara vardagens aktiviteter liksom att relatera till andra. Svårigheterna bidrar till höga sjukskrivningstal. Tree Theme Method® (TTM) är en arbetsterapeutisk metod som syftar till ökad förmåga att utveckla strategier och struktur i vardagens aktiviteter. TTM består av 5 sessioner och innebär att måla och berätta sin livsberättelse med fokus på vardagslivets aktiviteter samt skapa en bild av framtida mål. Fokus i tidigare studier avseende TTM har varit processutvärdering. Nu är det viktigt att utvärdera eventuella effekter av interventionen. I den här studien prövas TTM mot kontroll. Kontrollgruppen får annan arbetsterapeutisk behandling, dvs 5 sessioner om vardagens aktiviteter och psykiskt välmående i den aktuella situationen. Hypotesen är att interventionsgruppen får högre grad av struktur i vardagens aktiviteter och förbättrad psykisk hälsa jämfört med kontrollgruppen.

    Syfte: Undersöka effekter av TTM jämfört med kontrollgrupp avseende aktivitet, psykiska symtom och självskattad hälsa.

    Metod: Randomiserad multicenterstudie i primärvård och vuxenpsykiatri. Totalt 130 patienter inkluderas. Inklusion är fastställd depression och/eller ångest, 18-65 år samt aktivitetsproblem. Patienter lottas till TTM alternativt annan arbetsterapeutisk behandling. Deltagarna besvarar självskattningsformulär avseende vardagens aktiviteter, psykiska symtom samt självskattad hälsa. Detta sker före och direkt efter behandling samt 3 och 12 månader efter avslutad behandling. Icke-parametrisk statistisk analys kommer att användas.

    Resultat/preliminärt resultat: Hittills har 55 patienter inkluderats. I nuläget finns inga preliminära resultat. Presentation av baslinjemätningar sker vid arbetsterapiforum.

    Slutsats: Det är betydelsefullt att kunna erbjuda patienter med depression och ångest olika behandlingsmetoder. Den här studien kan bidra till högre grad av evidens för arbetsterapeutisk behandling.

    Background

    Depression and anxiety often reduce people's ability to cope with everyday occupations. There is a lack of knowledge about such problems in people of working age with depression and anxiety.

    Aim

    To describe which problems people with depression or anxiety disorders experience when performing everyday occupations and which occupations are affected.

    Materials and methods

    Data based on the Canadian Occupational Performance Measure was used in this cross-sectional study. A total of 118 participants aged 18-65 years, with depression or anxiety, were recruited from primary healthcare and general mental healthcare services. The data were analysed with descriptive statistics and directed content analysis.

    Results

    The participants rated a low level of occupational performance, and their satisfaction with performance even lower. They described a great number of problems with their everyday occupations. The most frequent problem areas concerned household management, socialization and personal care. Detailed descriptions of which type of problem they experienced during everyday occupations are included.

    Conclusions

    This study provides knowledge of which problems people with depression and anxiety disorders experience in everyday occupations within self-care, productivity, as well as leisure. Furthermore, they rate performance and satisfaction with performance of the five occupations they find the most important to change in everyday life.

    INTRODUCTION: Depression and anxiety disorders affect individuals' everyday lives, and treatments that can help them to perform everyday occupations are needed. Occupational therapy for this group has been evaluated from a short-term perspective but not from a long-term perspective; further research is thus warranted. The aim of the study was to investigate the longitudinal outcomes of the Tree Theme Method (TTM) compared with care as usual, provided by occupational therapists, in terms of everyday occupations, psychological symptoms, and health-related aspects.

    METHODS: This randomised controlled trial comprised a follow-up 3 and 12 months after an intervention. A total of 118 participants (19-64 years) with depression or anxiety disorders and problems with everyday occupations completed the base line questionnaires, 100 completed the follow-up at 3 months, and 84 completed the follow-up at 12 months. Imputations of missing data were performed using the last observation, and parametric analysis was used.

    RESULTS: Both groups showed significant improvements (P value ≤ 0.01) in everyday occupations, psychological symptoms and health-related aspects after 3 and 12 months. No significant differences were found between the groups.

    CONCLUSION: This study contributes with knowledge about the outcomes of occupational therapy for clients living with depression and anxiety disorders. Both TTM and care as usual lead to significant improvements over time concerning everyday occupations, psychological symptoms, and health-related aspects. The fact that both occupational therapy methods were associated with improvements for clients with depression and anxiety supports client-centredness in enabling an occupational therapist to choose the method best suited for the individual.

    Be WellTM – an intervention using photo-supported conversations to promote well-being in people living with stress-related illness2022In: Caring in a changing world, 2022Conference paper (Refereed)
    Abstract [en]

    Background: Stress-related mental health illnesses are increasing in all ages. Be-WellTM is a health-promoting intervention intended for primary healthcare. Be-WellTM involves 12 sessions based on photo-supported conversations. Patients use their mobiles to photograph situations about well-being, and the photos are used as a starting point for dialogues with their therapist.

    Aim: The aims of this study are to evaluate the feasibility of Be-WellTM, and to compare the outcomes concerning health and well-being in the intervention group with those of a control group.

    Method: This ongoing study is conducted in Swedish primary healthcare. A total of 70 patients in working age, living with stress-related disorders will be recruited. The intervention group receive the intervention Be-WellTM in addition to care as usual, and the controls only receive care as usual. Prior to and directly after the intervention, and after 6 months, the participants complete questionnaires and take part in qualitative interviews about stress and well-being in their present life-situation and experiences from participating in Be-WellTM. Non-parametric and qualitative analysis will be used.

    Results: Twenty-nine of 35 participants have been recruited to the intervention and 28 of 35 participants to the control group. We will present the research design and preliminary outcomes from the baseline and follow-up data.

    Conclusion: If the intervention Be-WellTM is found to be feasible with positive outcomes, the health-promoting intervention Be-WellTM can be useful as a complementary intervention in primary healthcare for patients with stress-related illness.

    Implications for Caring in a changing world: Probably, photo-supported conversations may promote well-being to other patients in different life situations, but future research is warranted.

    Background: There is need for interventions for people with depression and anxiety, not only focusing on psychological symptoms, but also how to cope with everyday life. The TTM involves that individuals during five sessions, paint and tell their life story. The focus is on everyday occupations, and finally, they create a picture of their future goals. In this study, the TTM was compared to regular occupational therapy.

    Objective: The aim of this study was to evaluate the TTM compared to regular occupational therapy regarding everyday occupations, psychological symptoms and health in people with depression and/or anxiety disorders.

    Methods: 118 clients, 18-65 years, diagnosed with depression or anxiety in primary care and mental health care were randomized to the TTM or regular occupational therapy. Before and after treatment as well as 3 and 12 months afterwards they answered self-assessment questionnaires. Parametric statistical analysis was used.

    Results: There were continued positive significant outcomes of everyday occupations, psychological symptoms and health shown in both groups; 3 months as well as 12 months after completed treatment. No significant differences were found between the groups.

    Conclusions: The TTM can contribute to an increased number of interventions for people suffering from depression and anxiety.

    Background

    Depression and anxiety disorders are increasing among the general population in the Western world. Individuals may need several kinds of treatment in order to maintain health, such as cognitive behavioural therapy (CBT) and drug treatment. However, having an everyday life that “works” is also important, suggesting a need for interventions based on activities that facilitate a satisfying everyday life. There is still lack of such evidence-based interventions. The Tree Theme Method® (TTM) is an occupational therapy intervention designed for a client-centred context in which an individual develops strategies to become an actor in his or her everyday life. Previous studies of the TTM have focused on process evaluation; therefore, further studies are needed to evaluate the method’s effects. The aim of this paper is to outline an intervention that can evaluate the effects of the TTM in terms of psychological symptoms, as well as everyday occupations and well-being, in patients suffering from depression and anxiety.

    Methods/Design

    This randomized clinical trial includes patients from three Swedish counties randomized to either intervention or treatment as usual. Men and women aged 18–65 years who have been diagnosed with either depression or anxiety are eligible for inclusion. Data collection is carried out at baseline, and outcomes are assessed at the end of intervention, as well as at 3 months and 12 months after intervention ends. The outcomes measured are psychological symptoms, everyday activities, and health-related factors.

    Discussion

    Depression and anxiety may create difficulties for individuals in the activities of their everyday lives to the extent that they require diagnosis and intervention. Despite this reality, evidence-based interventions that focus on everyday activities are lacking. Therefore, it would be useful to design a specific method for occupational therapy intervention that does precisely that. This study provides insight into the effects of the TTM, comparing it to occupational therapy treatment as usual.

    Bakgrund/Syfte

    Stressrelaterad ohälsa är ett stort samhällsproblem. Vi har utvecklat en hälsofrämjande behandlingsmetod med fotostödda samtal, Be-WellTM, med förhoppningen att bidra till att minska den stressrelaterade ohälsan. Metoden fokuserar på det som främjar hälsa och välbefinnande trots ohälsa och inte på symtom och problem i vardagen. Be-WellTM bygger på kunskap om betydelsen av hälsa, välbefinnande, vardagliga aktiviteter och aktivitetsbalans. Be-WellTM baserat på erfarenheter från tidigare studie i primärvården där kvinnor med stressrelaterad ohälsa fotograferade sådant som relaterade till deras välbefinnande trots stress. I intervjuerna framkom att kvinnorna blev uppmärksamma på sitt välbefinnande, vilket möjliggjorde perspektivförskjutning från det som inte fungerar i vardagen till det som fungerar.

    Syftet är att utvärdera Be-WellTM för personer i primärvården med diagnosticerad stressrelaterad ohälsa.

    Metod/Tillvägagångssätt

    Be-WellTM innebär att deltagare tar fotografier med mobil på sådant som ger välbefinnande i vardagen, för att sedan använda dem som utgångspunkt för reflekterande samtal med behandlare vid sammanlagt 12 tillfällen. 

    I en pågående kontrollerad studie i primärvården får interventionsgruppen Be-WellTM av arbetsterapeut som tillägg till sedvanlig behandling.  Inklusionskriterier är personer i yrkesverksam ålder med stressrelaterad ohälsa. Data från frågeformulär, intervjuer och fotografier samlas in före och efter intervention, samt sex månader efter intervention och behandlarna intervjuas också. Datainsamling beräknas vara avslutad början av 2023. Kvantitativa som kvalitativa analyser kommer att genomföras.

    Resultat/Preliminärt resultat

    Indikationer från intervjuer tyder på att patienter och behandlare uppskattar metoden. Vi planerar att presentera metoden Be-WellTM samt preliminära resultat i samband med Arbetsterapiforum.

    Slutsats/Praktisk tillämpning

    Presentationen ger kännedom om en möjlig behandlingsmetod som är enkel att införa i primärvården.

    BACKGROUND: Stress-related illnesses constitute a huge problem in society. The primary care services in Sweden form the first line of care whose role is to coordinate interventions for reducing symptoms, as well as health-promoting interventions. There is lack of knowledge concerning health-promoting interventions for these illnesses. The aim of this study is to evaluate whether photo-supported conversations about well-being (Be Well™) as an intervention, in addition to care as usual within the primary care services, improves health and well-being for patients with stress-related illnesses. The intervention will be compared to a control group, who receive care as usual. A further aim is to conduct a process evaluation.

    METHODS/DESIGN: This ongoing project has a quasi-experimental design, using quantitative and qualitative methods, and includes patients from primary care centres in two Swedish counties. Seventy patients, 20-67 years, with stress-related illnesses will be recruited. They constitute an intervention group, which receive the intervention together with care as usual, and a control group, which receive care as usual. The intervention, photo-supported conversations about well-being, involves 12 sessions. Care as usual entails medication, occupational therapy, physiotherapy and/or psychotherapy. Data collection is carried out at baseline, and outcomes are assessed directly after the intervention, as well as six months after completion of the intervention. The outcomes are evaluated based on factors related to health, well-being and everyday occupations. Furthermore, data concerning experiences of well-being and perceptions of the intervention will be collected in interviews. The therapists will also be interviewed about their experiences of performing the intervention. Data will be analysed with non-parametric statistics, and qualitative methodology.

    DISCUSSION: The project is based on the concept that focusing on well-being despite living with stress-related illness may positively impact health and well-being as well as activity-related aspects, and that photo-supported conversations about well-being can contribute a complement to other treatment and rehabilitation. A strength is the use of a wide range of methods: such as quantitative measures, photographs, and qualitative interviews with participants and therapists. The results will thus provide knowledge about potential effects of this health-promoting intervention.     Background: Occupational balance (OB) is an important concept in occupational therapy and is considered as an essential component of health and well-being.

    Aim: The aim of this study was to investigate the validity and reliability of the Turkish version of the Occupational Balance Questionnaire-11 (OBQ11-T).

    Materials and methods: The OBQ11-T was administered to 116 individuals for construct validity analysis and to 58 individuals for reliability analysis. Validity was determined using explanatory and confirmatory factor analysis. In confirmatory factor analysis, RMSEA (root mean square error of approximation) value, comparative fit index (CFI), and Chi-square to degrees of freedom ratio (CMIN/DF) were analysed. The test-retest method and Cronbach's alpha coefficient were used to assess the reliability and internal consistency of the OBQ11-T, respectively.

    Results: The model fit the data according to the indices of relative fit (RMSEA = 0.076, CMIN/DF = 1.661, and CFI = 0.875). The correlation coefficient between test and retest OBQ11-T total scores was 0.922. Total scores showed a significant statistical relationship (p<0.01), indicating good reliability. Cronbach's alpha for OBQ11-T total score was 0.785, indicating acceptable internal consistency.

    Conclusions and significance: The current study demonstrates that the OBQ11-T is a valid and reliable tool for measuring the self-rated OB of healthy individuals.

    Introduction: Occupational balance (OB) is an important concept in occupational therapy and is considered as an essential component of health and well-being. The aim of this study was to show differences in OB and quality of life (QoL) between mothers of children with cerebral palsy (CP) and typically developing children.

    Methods: Thirty-six mothers of children with CP and 36 mothers of typically developing children participated in the study. The mothers’ OB was evaluated using the Turkish Occupational Balance Questionnaire-11 (OBQ11-T), and their QoL was evaluated with the Nottingham Health Profile (NHP).

    Results: The OBQ11-T, total score, and the item ‘balance between obligatory and voluntary occupations’ score differed significantly between the groups (p < 0.05). There were also significant differences in NHP’s aspects of emotional reaction, social isolation, and sleep scores between the groups (p < 0.05). A negative correlation was detected between OBQ11-T total and NHP total scores in mothers of children with CP (p < 0.01).

    Conclusion: This study demonstrates that occupational therapists should pay attention to balance between obligatory and voluntary occupations to promote occupational balance. Also, QoL in mothers of children with CP should be evaluated. Therefore, occupational therapists should implement interventions to increase them when necessary for mothers of children with CP.

    IMPORTANCE: Although occupational balance (OB) is a construct of importance to occupational therapy, existing OB assessments have not been validated in clinical populations.

    OBJECTIVE: To examine the validity and reliability of the 11-item version of the Occupational Balance Questionnaire (OBQ11) in U.S. adults with Type 1 diabetes.

    DESIGN: Data were analyzed from adults with Type 1 diabetes enrolled in a larger longitudinal study examining the relationships among blood glucose, emotion, and functioning. Dimensionality of the OBQ11 was assessed with item response theory (IRT); convergent validity was tested by examining whether associations between the OBQ11 and other constructs were consistent with a priori hypotheses.

    SETTING: Three outpatient clinical sites in the United States.

    PARTICIPANTS: Data from 208 U.S. adults with Type 1 diabetes were included in the analyses (42% Latino, 29% White, 14% African American, 7% multiethnic, and 8% other).

    OUTCOMES AND MEASURES: Assessments administered include the OBQ11, Patient Health Questionnaire (depression), and Diabetes Self-Management Questionnaire.

    RESULTS: Overall, results from IRT models and correlational tests supported the reliability and validity of the OBQ11. For instance, higher scores on the OBQ11 were significantly associated with better self-ratings of diabetes management behaviors (r = .28, p < .001), lower depression symptoms (r = -.53, p < .001), and greater positive affect (r = .32, p < .001). A single-factor generalized partial credit model fit the OBQ11 acceptably well, supporting its unidimensionality.

    CONCLUSIONS AND RELEVANCE: The OBQ11 may be a reliable and valid measure of OB appropriate for use in clinical populations such as adults with diabetes. What This Article Adds: OB is not often formally assessed by occupational therapists in the United States, even though the contributions of OB to health and well-being are core components of the philosophy of occupational therapy. The current evidence supports the validity of the OBQ11 in a clinical population of adults with Type 1 diabetes and demonstrates significant associations between OB and health management behaviors. Study results may encourage greater consideration and assessment of OB in occupational therapy clinical practice in the United States.

    BACKGROUND: Sick leave due to long-term stress is increasing in Finland and other Western countries. Occupational therapists might contribute to the prevention of and/or recovery from stress-related exhaustion.

    AIMS/OBJECTIVES: To describe what is known about occupational therapy for stress-related exhaustion.

    MATERIAL AND METHODS: A five-step scoping review included papers published in six databases from 2000 to 2022. Extracted data was summarized to show the occupational therapy contribution within the literature.

    RESULTS: There were 29 papers meeting the inclusion criteria, of which a limited number described preventive interventions. Most articles described recovery-oriented occupational therapy with group interventions. The occupational therapists contributed with prevention measures, mostly targeting recovery in multi-professional interventions in terms of stress reduction or return-to-work.

    CONCLUSIONS: Occupational therapy involving stress management both prevents stress and supports recovery from stress-related exhaustion. Occupational therapists internationally use craft, nature activities or gardening as stress management measures.

    SIGNIFICANCE: Occupational therapy appears to be a potential treatment option for stress-related exhaustion internationally in conditions that could also work in Finland, for example in occupational healthcare.

    Introduction: Occupational balance and satisfaction with daily occupations are two central concepts in occupational science. The aim of the present study was therefore to explore associations between different aspects of occupational balance and satisfaction with daily occupations. A second aim was to explore whether different aspects of occupational balance predicted satisfaction with daily occupations 12 months later.

    Methods: The present study had an exploratory correlational longitudinal pilot study design and was part of a project in which the Tree Theme Method was evaluated for patients with depression and/or anxiety disorders. Data collected directly after the intervention (n=107) and 12 months later (n=84) were used in the present study. The Occupational Balance Questionnaire (OBQ) and the Satisfaction with Daily Occupations (SDO) were used, and binary logistic regressions were used to explore associations between them.

    Results: Statistically significant associations were found between the following aspects of occupational balance: Balance between work, home, family, leisure, rest, and sleep; Having neither too much nor too little to do during a regular week; Satisfaction with time spent in rest, recovery, and sleep and satisfaction with daily occupations directly after the intervention. The occupational balance item Balance between energy-giving/energy-taking occupations directly after the intervention predicted satisfaction with daily occupations 12 months later.

    Conclusion: This study provides some support for the assumption about an association between occupational balance and satisfaction with daily occupations. Studies with larger sample sizes and in other cultures are recommended to confirm the presence of these associations and/or to find other associations.

    Introduction: Occupational balance is the experience of having the right amount and right variation between work, domestic work, leisure, rest, and sleep. There is limited knowledge about which factors predict parents’ combined occupational balance, and if the combined occupational balance is associated with work and life satisfaction.

    Aim: The first aim was to explore whether domestic work and childcare at baseline predicted combined occupational balance at follow-up among working cohabiting parents. The second aim was to explore associations between different combinations of occupational balance, and work and life satisfaction at follow-up.

    Method: A sample of 139 cohabiting parents responded to a questionnaire. Cohabiting parents can experience their occupational balance differently, and in the present study their experiences were divided into high and low after the median. The parents’ balance was then combined in terms of high-high, high-low and low-low. Associations between childcare, domestic work, work and life satisfaction, and combined occupational balance were analysed with multinomial logistic regression analyses.

    Results: Satisfaction with the division of domestic work predicted a high-high occupational balance in parent couples. Associations were found between high-high as well as high-low combined occupational balance and life satisfaction, and between high-high occupational balance and work satisfaction.

    Conclusions: It seems important for both parents in a couple to experience satisfaction with the division of domestic work to experience high occupational balance as well as work and life satisfaction.

    Bakgrund/Syfte

    Aktivitetsbalans definieras som att: ”uppfatta livet som innehållande rätt mängd och rätt variation mellan olika aktiviteter i relation till: aktivitetskategorier, aktiviteter med olika karaktäristika samt tidsanvändning”. Aktivitetsbalans ses alltså som personens egna subjektiva upplevelse av sin mix av aktiviteter i aktivitetsmönstret oberoende av vilka aktiviteter som ingår. Baserat på denna definition utvecklades ett instrument för självskattad aktivitetsbalans (OBQ11) som är reliabilitets- och validitetstestat och avsett att användas både i praxis och forskning.

    Metod/Tillvägagångssätt

    Instrumentet har utvecklats stegvis. Först identifierades ett antal indikatorer på aktivitetsbalans baserat på forskning samt erfarenheter från praxis. Utifrån dessa indikatorer utvecklades ett antal påståenden som granskades av experter inom området. Baserat på deras synpunkter omformulerades och reducerades antalet påståenden. Denna första version testades med klassisk testteori. Därefter gjordes en Raschanalys och antalet påståenden reducerades till nuvarande version, OBQ11.

    Resultat/Preliminärt resultat

    Instrumentet består av 11 påståenden där svaren kan användas som en screening för att få en bild av hur aktivitetsbalansen ser ut. I praxis behöver OBQ11 kompletteras med en dialog för att få en större förståelse för personens aktivitetsbalans och vad som eventuellt behöver förändras för att personen ska uppleva en bättre aktivitetsbalans. Instrumentet kan också användas som uppföljning och utvärdering av interventioner i både praxis och forskning. Instrumentet självskattad aktivitetsbalans (OBQ11) ges from hösten 2022 ut av Sveriges Arbetsterapeuter.

    Slutsats/Praktisk tillämpning

    Instrumentet självskattad aktivitetsbalans (OBQ11) är testat i olika grupper, både i normalpopulationer och i olika diagnosgrupper och är validerat och reliabelt för dessa grupper. Det är ännu inte testat på barn och ungdomar.

    Background: Occupational balance is fundamental to occupational therapy and occupational science. Therefore, the Occupational Balance Questionnaire (OBQ) was developed and has previously been found psychometrically valid according to classical test theory.

    Aim: To investigate the internal construct validity of the OBQ using Rasch measurement theory.

    Material and methods: Data from two general population samples were used to investigate the psychometric properties of the OBQ according to Rasch measurement theory.

    Results: The analyses identified problems with the current response scale and multidimensionality of two items. As a result, a revised version, the OBQ11, was suggested and exhibited response categories that worked properly, good reliability (0.92), model fit and measurement invariance across age and gender groups. The hierarchical item ordering was in agreement with previous research.

    Conclusion: The new OBQ11 satisfies the measurement criteria defined by the Rasch model. However, further studies of additional samples are needed to validate its generic properties.

    Significance: The purpose of the OBQ11 is to measure occupational balance of individuals or groups, and to identify aspects of occupational balance in need of improvement. Our observations suggest that the OBQ11 is a valid and promising complement to other instruments.

    Purpose: The aim of the study was to describe how women with stress-related illness experience well-being in everyday life.

    Methods: The study was based on a reflective lifeworld research (RLR) approach and the methodological principles of openness, flexibility and bridling. Twelve women, aged 27-54 years, diagnosed with stress-related illness were included. Data were collected with lifeworld interviews based on photographs taken by the women relating to well-being in everyday life. The data were analysed for meaning.

    Results: Well-being emerged in situations where women could feel an unconditional beingness. This entails not having demands on oneself and includes some form of freedom from having to perform. The surroundings and supportive environments are important for this unconditional beingness to be present. In order to feel well-being in everyday life, the women need to balance their energy and find helpful tools that can achieve a balance in everyday life.

    Conclusions: Healthcare staff need to understand the importance of unconditional beingness in supportive environments for patients living with stress-related illness in order to support their health and well-being. It is also important to support patients in finding helpful tools that can aid them to achieve a balance in everyday life.

    In his response to our article “Hannah Arendt’s vita activa: A valuable contribution to occupational science,” Turnbull (2017) outlined some tensions he perceived with our discussion of “her approach to philosophy, politics and science”. In our reply, we express appreciation of his interest in Arendt related to occupational science and the contribution his article makes, as well as clarifying some points in his critique. We argue that Turnbull’s main arguments do not address our primary purpose of presenting Arendt’s vita activa and provide examples of its relevance for occupational science. We thus focused on activity-related aspects of Arendt’s thoughts, to raise awareness of this work amongst occupational scientists. However, we are thankful for Turnbull’s reflections, which broaden the insights of Arendt’s thinking and contribute to a better understanding of human occupation.

    Occupational science is undergoing dynamic development and claims have been articulated that human occupation must be understood from multiple ontological standpoints. Hannah Arendt (1906–1975) is known for her work The Human Condition in which she explored human occupation from a philosophical and political standpoint. She distinguished the modalities labor, work and action, and labelled them vita activa. The aim of this paper is to present Arendt and her vita activa, in order to provide examples of its relevance for occupational science, showing how vita activa can assist occupational scientists to take a deeper perspective on human occupation. According to Arendt, human occupation is always conditioned. The condition for labor is necessity, which reflects human biological needs and represents the basics of life. The condition for work is utility, as something persistent and durable is produced. Action is the activity that takes place between people without the intermediary of things. Similar to occupational science, vita activa is concerned with human doing but their origins differ. Arendt also emphasized the public sphere as an arena for human occupation, a viewpoint that is shared with recent occupational science literature. The need to expand the scope of occupational science to encompass all aspects of human occupations, including the deleterious, has been expressed and vita activa can contribute to broadening this perspective.     Abstract [en]

    Introduction: People from European Union (EU) countries such as Latvia, Romania and Bulgaria who beg on the streets have become a common part of the townscape in Sweden. While demanding situations in their home countries drive them abroad to earn their living, those who turn to begging still face challenges in their everyday lives. Additional knowledge is needed, from their perspective, about the occupational challenges of begging, as well as how they adapt to those challenges.

    Aim: To describe the occupational challenges and related adaptations of vulnerable EU citizens begging in Sweden.

    Methods: We used a descriptive qualitative design. Individual interviews with 20 Romanian participants were conducted and analysed using qualitative content analysis with a deductive and inductive approach.

    Results: Six categories of occupational adaptation responses were identified: Keeping the family tier intact despite distance; Dealing with shame and stress; Managing body and mind when begging despite the monotony; Living as cheaply as possible; Trying to get a night's sleep without drawing attention to oneself; and Seeking alternatives for earning their living.

    Conclusion: Everyday life, when begging abroad, is filled with occupational challenges requiring occupational adaptations, for better or for worse, to sustain basic human needs. To further understand the needs of this vulnerable and marginalised group in society, human rights discourses that go beyond the individual level are needed.

    Ambulance personnel in emergency medical services are exposed to physical demands and stress during work, and an increased prevalence of ill health has been observed in this group. The aim was to compare hair cortisol concentration (HCC) among Swedish ambulance personnel with HCC in a population-based reference sample, to analyse differences between women and men, and differences due to work-related factors. Samples of hair 1 cm closest to the skin (5-10 mg) were collected and analysed for cortisol by radioimmunoassay. Moreover, the participants responded to a questionnaire regarding their work environment. The HCC among the ambulance personnel did not differ from the HCC in the population-based reference sample (median 19.2 vs. 22.2 pg/mg, p = 0.319), nor were there statistically significant differences between women and men. Furthermore, no associations were found between HCC and physical and psychosocial work demands, work stress, or rest and recovery from work. However, occupational balance was positively correlated with HCC (rp = 0.240; p = 0.044). The association remained statistically significant after adjustment for sex, age, hair bleaching, and corticosteroid treatment in a linear regression model. This study adds knowledge regarding HCC among ambulance personnel, and thus contributes to the overall picture of work environment and health for this group.

    Background: Social interactions between registered nurses, older patients and their relatives are essential and play a central role in developing a successful care relationship in healthcare encounters. How nurses interact with patients affects the patient's well-being. Limited time and demands for efficiency influence the encounter and complaints from patients and relatives often concern social interactions. Therefore, the aim of this study was to explore the social interaction in encounters between registered nurses, older patients and their relatives at a department of medicine for older people.

    Methods: The study has an ethnographic approach including participatory observations (n = 21) and informal field conversations (n = 63), followed by a thematic analysis with an abductive approach reflecting Goffman's interactional perspective.

    Result: The result revealed a pattern where the participants manoeuvred between interplay and context. By manoeuvring, they defined roles but also created a common social situation. Nurses led the conversation; patients followed and described their health problems, while relatives captured the moment to receive and provide information. Finally, nurses summarised the encounter using ritual language, patients expressed gratitude through verbal and non-verbal expressions, while relatives verbally confirmed the agreements.

    Conclusion: The social interaction between registered nurses, older patients and relatives was shaped by a pattern where the participants manoeuvred between interplay and context. When all participants assume responsibility for the social interaction, they become active and listen to each other. The approach adopted by nurses is crucial, thus training in communication and social interaction skills are important. When the asymmetry due to imbalance, is reduced, less misunderstanding and a satisfactory care relationship can be achieved.

    AIMS AND OBJECTIVES: To describe how nurses communicate with older patients and their relatives in a department of medicine for older people in western Sweden.

    BACKGROUND: Communication is an essential tool for nurses when working with older patients and their relatives, but often patients and relatives experience shortcomings in the communication exchanges. They may not receive information or are not treated in a professional way. Good communication can facilitate the development of a positive meeting and improve the patient's health outcome.

    DESIGN: An ethnographic design informed by the sociocultural perspective was applied.

    METHODS: Forty participatory observations were conducted and analysed during the period October 2015-September 2016. The observations covered 135 hours of nurse-patient-relative interaction. Field notes were taken, and 40 informal field conversations with nurses and 40 with patients and relatives were carried out. Semistructured follow-up interviews were conducted with five nurses.

    RESULTS: In the result, it was found that nurses communicate with four different voices: a medical voice described as being incomplete, task-oriented and with a disease perspective; a nursing voice described as being confirmatory, process-oriented and with a holistic perspective; a pedagogical voice described as being contextualised, comprehension-oriented and with a learning perspective; and a power voice described as being distancing and excluding. The voices can be seen as context-dependent communication approaches. When nurses switch between the voices, this indicates a shift in the orientation or situation.

    CONCLUSION: The results indicate that if nurses successfully combine the voices, while limiting the use of the power voice, the communication exchanges can become a more positive experience for all parties involved and a good nurse-patient-relative communication exchange can be achieved.

    RELEVANCE TO CLINICAL PRACTICE: Working for improved communication between nurses, patients and relatives is crucial for establishing a positive nurse-patient-relative relationship, which is a basis for improving patient care and healthcare outcomes.
