    Abstract [en]

    Background:

    Parents with small children constitute a vulnerable group as they have an increased risk of sick leave due to stress-related disorders compared to adults without children. It has been shown that mothers and fathers to small children together spend more time in paid work than any other group, which could create negative stress and an experience of low occupational balance.

    Aim:

    The aim of this study was to examine associations between organisational factors and occupational balance among parents with small children in Sweden.

    Methods:

    Data were collected by a survey including questions about occupational balance, organisational factors and age, sex, employment rate, work position, monthly household income, number of children at home, separation/divorce last five years and overtime. The total number of parents included in this study was 718 (490 mothers and 228 fathers). Logistic regression models were applied to examine the odds ratios for occupational balance in relation to organisational factors.

    Results:

    Parents who experienced positive attitudes towards parenthood and parental leave among colleagues and managers were more likely to experience high occupational balance than parents who experienced negative or neutral attitudes. Having a clear structure for handover when absent from work was also strongly associated with high occupational balance.

    Conclusions:

    The result of the present study indicates that some organisational factors could be important for the occupational balance of parents with small children.

    Occupational therapists need to be cognizant of evidence-based role balance advice and strategies that women with multigenerational caring responsibilities can implement independently or with minimal assistance, as role balance may not be the primary goal during many encounters with this population. Hence, this study aimed to identify the viewpoints on the most helpful role balance strategies for working sandwich generation women, both from their own perspectives and from the perspective of occupational therapists. This was achieved through a Q methodology study, where 54 statements were based on findings from interviews, sandwich generation literature and occupational therapy literature. In total, 31 working sandwich generation women and 42 occupational therapists completed the Q sort through either online or paper administration. The data were analysed using factor analysis with varimax rotation and were interpreted through collaboration with experts in the field. The findings revealed similarities between working sandwich generation women and occupational therapists, particularly in terms of advocating strategies related to sleep, rest and seeking practical assistance from support networks. Differences were also present, with working sandwich generation women viewpoints tending to emphasize strategies related to coping with a busy lifestyle attending to multiple responsibilities. In contrast, occupational therapy viewpoints prioritized strategies related to the occupational therapy process, such as goal setting, activity focused interventions, monitoring progress and facilitating sustainable outcomes.

    Bakgrund: Vid depression och ångest försvåras förmåga att klara vardagens aktiviteter liksom att relatera till andra. Svårigheterna bidrar till höga sjukskrivningstal. Tree Theme Method® (TTM) är en arbetsterapeutisk metod som syftar till ökad förmåga att utveckla strategier och struktur i vardagens aktiviteter. TTM består av 5 sessioner och innebär att måla och berätta sin livsberättelse med fokus på vardagslivets aktiviteter samt skapa en bild av framtida mål. Fokus i tidigare studier avseende TTM har varit processutvärdering. Nu är det viktigt att utvärdera eventuella effekter av interventionen. I den här studien prövas TTM mot kontroll. Kontrollgruppen får annan arbetsterapeutisk behandling, dvs 5 sessioner om vardagens aktiviteter och psykiskt välmående i den aktuella situationen. Hypotesen är att interventionsgruppen får högre grad av struktur i vardagens aktiviteter och förbättrad psykisk hälsa jämfört med kontrollgruppen.

    Syfte: Undersöka effekter av TTM jämfört med kontrollgrupp avseende aktivitet, psykiska symtom och självskattad hälsa.

    Metod: Randomiserad multicenterstudie i primärvård och vuxenpsykiatri. Totalt 130 patienter inkluderas. Inklusion är fastställd depression och/eller ångest, 18-65 år samt aktivitetsproblem. Patienter lottas till TTM alternativt annan arbetsterapeutisk behandling. Deltagarna besvarar självskattningsformulär avseende vardagens aktiviteter, psykiska symtom samt självskattad hälsa. Detta sker före och direkt efter behandling samt 3 och 12 månader efter avslutad behandling. Icke-parametrisk statistisk analys kommer att användas.

    Resultat/preliminärt resultat: Hittills har 55 patienter inkluderats. I nuläget finns inga preliminära resultat. Presentation av baslinjemätningar sker vid arbetsterapiforum.

    Slutsats: Det är betydelsefullt att kunna erbjuda patienter med depression och ångest olika behandlingsmetoder. Den här studien kan bidra till högre grad av evidens för arbetsterapeutisk behandling.

    Depression and anxiety disorders are a major concern in western countries, and because these often have a negative affect on everyday life interventions based on activities in everyday life are needed. The Tree Theme Method® (TTM) is a client-centred occupational therapy intervention designed to increase the ability to cope with, and to enhance satisfaction with, everyday life, both at home and at work. The aim of this study was to compare the short term outcomes of the TTM intervention with regular occupational therapy treatment for people with depression and/or anxiety disorders.

    Abstract [en]

    Background

    Depression and anxiety disorders are increasing among the general population in the Western world. Individuals may need several kinds of treatment in order to maintain health, such as cognitive behavioural therapy (CBT) and drug treatment. However, having an everyday life that “works” is also important, suggesting a need for interventions based on activities that facilitate a satisfying everyday life. There is still lack of such evidence-based interventions. The Tree Theme Method® (TTM) is an occupational therapy intervention designed for a client-centred context in which an individual develops strategies to become an actor in his or her everyday life. Previous studies of the TTM have focused on process evaluation; therefore, further studies are needed to evaluate the method’s effects. The aim of this paper is to outline an intervention that can evaluate the effects of the TTM in terms of psychological symptoms, as well as everyday occupations and well-being, in patients suffering from depression and anxiety.

    Methods/Design

    This randomized clinical trial includes patients from three Swedish counties randomized to either intervention or treatment as usual. Men and women aged 18–65 years who have been diagnosed with either depression or anxiety are eligible for inclusion. Data collection is carried out at baseline, and outcomes are assessed at the end of intervention, as well as at 3 months and 12 months after intervention ends. The outcomes measured are psychological symptoms, everyday activities, and health-related factors.

    Discussion

    Depression and anxiety may create difficulties for individuals in the activities of their everyday lives to the extent that they require diagnosis and intervention. Despite this reality, evidence-based interventions that focus on everyday activities are lacking. Therefore, it would be useful to design a specific method for occupational therapy intervention that does precisely that. This study provides insight into the effects of the TTM, comparing it to occupational therapy treatment as usual.

    Abstract [en]

    In his response to our article “Hannah Arendt’s vita activa: A valuable contribution to occupational science,” Turnbull (2017) outlined some tensions he perceived with our discussion of “her approach to philosophy, politics and science”. In our reply, we express appreciation of his interest in Arendt related to occupational science and the contribution his article makes, as well as clarifying some points in his critique. We argue that Turnbull’s main arguments do not address our primary purpose of presenting Arendt’s vita activa and provide examples of its relevance for occupational science. We thus focused on activity-related aspects of Arendt’s thoughts, to raise awareness of this work amongst occupational scientists. However, we are thankful for Turnbull’s reflections, which broaden the insights of Arendt’s thinking and contribute to a better understanding of human occupation.

    Abstract [en]

    Occupational science is undergoing dynamic development and claims have been articulated that human occupation must be understood from multiple ontological standpoints. Hannah Arendt (1906–1975) is known for her work The Human Condition in which she explored human occupation from a philosophical and political standpoint. She distinguished the modalities labor, work and action, and labelled them vita activa. The aim of this paper is to present Arendt and her vita activa, in order to provide examples of its relevance for occupational science, showing how vita activa can assist occupational scientists to take a deeper perspective on human occupation. According to Arendt, human occupation is always conditioned. The condition for labor is necessity, which reflects human biological needs and represents the basics of life. The condition for work is utility, as something persistent and durable is produced. Action is the activity that takes place between people without the intermediary of things. Similar to occupational science, vita activa is concerned with human doing but their origins differ. Arendt also emphasized the public sphere as an arena for human occupation, a viewpoint that is shared with recent occupational science literature. The need to expand the scope of occupational science to encompass all aspects of human occupations, including the deleterious, has been expressed and vita activa can contribute to broadening this perspective. Examples of the need for sustainability in working life are also presented in this paper.

    Abstract [en]

    AIMS AND OBJECTIVES: To describe how nurses communicate with older patients and their relatives in a department of medicine for older people in western Sweden.

    BACKGROUND: Communication is an essential tool for nurses when working with older patients and their relatives, but often patients and relatives experience shortcomings in the communication exchanges. They may not receive information or are not treated in a professional way. Good communication can facilitate the development of a positive meeting and improve the patient's health outcome.

    DESIGN: An ethnographic design informed by the sociocultural perspective was applied.

    METHODS: Forty participatory observations were conducted and analysed during the period October 2015-September 2016. The observations covered 135 hours of nurse-patient-relative interaction. Field notes were taken, and 40 informal field conversations with nurses and 40 with patients and relatives were carried out. Semistructured follow-up interviews were conducted with five nurses.

    RESULTS: In the result, it was found that nurses communicate with four different voices: a medical voice described as being incomplete, task-oriented and with a disease perspective; a nursing voice described as being confirmatory, process-oriented and with a holistic perspective; a pedagogical voice described as being contextualised, comprehension-oriented and with a learning perspective; and a power voice described as being distancing and excluding. The voices can be seen as context-dependent communication approaches. When nurses switch between the voices, this indicates a shift in the orientation or situation.

    CONCLUSION: The results indicate that if nurses successfully combine the voices, while limiting the use of the power voice, the communication exchanges can become a more positive experience for all parties involved and a good nurse-patient-relative communication exchange can be achieved.

    RELEVANCE TO CLINICAL PRACTICE: Working for improved communication between nurses, patients and relatives is crucial for establishing a positive nurse-patient-relative relationship, which is a basis for improving patient care and healthcare outcomes.

    AIMS AND OBJECTIVES: To explore and describe the content of the communication exchanges between nurses, patients and their relatives in a department of medicine for older people in western Sweden.

    BACKGROUND: Information, messages and knowledge are constantly being communicated between nurses, older patients and relatives in the healthcare sector. The quality of communication between them has a major influence on patient outcomes. A prerequisite for good care to be given and received is that there is mutual understanding between the parties involved.

    DESIGN: An ethnographic study was informed by a sociocultural perspective.

    METHOD: Data were collected through 40 participatory observations of meetings between nurses and older patients and/or relatives which covered 135 hr of nurse-patient-relative interaction, field notes, 40 field conversations with 24 nurses and 40 field conversations with patients (n = 40) and relatives (n = 26). Five semi-structured interviews were conducted with nurses. An ethnographic analysis was performed.

    RESULTS: The analysis identified three categories of content of the communication exchanges: medical content focusing on the patient's medical condition, personal content focusing on the patient's life story and explanatory content focusing on the patient's health and nursing needs. The content is influenced by the situation and context.

    CONCLUSIONS: Nurses would benefit from more awareness and understanding of the importance of the communication content and of the value of asking the didactic questions (how, when, what and why) to improve the patients' and relatives' understanding of the information exchanges and to increase patient safety.

    RELEVANCE TO CLINICAL PRACTICE: Nurses can use the communication content to create conditions enabling them to obtain a holistic view of the patient's life history and to develop an appropriate person-centred care plan.

    Högskoleutbildning för hållbar utveckling: En kartläggning vid Jönköpings högskola2014Report (Refereed)
    Abstract [sv]

    Högre utbildning i Sverige ska enligt Högskolelagens paragraf 5 främja hållbar utveckling (Svensk högskolelag 1992:1434). Hållbar utveckling har definierats som en ”utveckling som tillfredsställer dagens behov utan att kompromissa med kommande generationers möjligheter att tillfredsställa sina behov” (United Nations, 1987). Hållbar utveckling innebär ansvarstagande för kommande generationer och utsträcker sig globalt. När ekologiska gränser överskrids kan människors möjligheter till goda liv undermineras.

    Våren 2013 fick författarna till denna rapport pedagogiska medel för att kartlägga undervisning om och för hållbar utveckling vid Högskolan i Jönköping. Syftet var att undersöka omfattningen av hållbar utveckling i undervisningen på strategisk nivå, programnivå, kursplanenivå och undervisningsnivå. Ett ytterligare syfte var att utifrån en analys av resultatet föreslå åtgärder som kan stärka högskolan i arbetet med utbildning för hållbar utveckling. Kartläggningen började med ett liknande upplägg på samtliga fackhögskolor men fick också anpassas till förutsättningarna på varje skola.

    Resultatet av kartläggningen visar skillnader mellan de olika fackhögskolorna i hur hållbarhetsfrågor inkluderas i undervisningen. Allt från att ingen kursplan innehållande hållbar utveckling identifierats till att undervisningen måste inkludera ett visst antal högskolepoäng. Kartläggningen visar också att tolkningarna av hållbar utveckling och undervisning för/om hållbar utveckling varierar mellan enskilda lärare.

    På Hälsohögskolan är området osynligt i kursplaner och inkluderandet av hållbar utveckling i undervisningen bygger på enskilda lärares engagemang. På Högskolan för lärande och kommunikation skiljer det sig mellan olika program där inslaget av hållbar utveckling varierar från att behandlas ingående till att inte synas explicit. Internationella handelshögskolan har prioriterat arbete med etiska och sociala frågor i utbildning och ekologisk hållbarhet är mindre framträdande. Det finns dock kurser som inkluderar ekologiska, sociala och ekonomiska dimensioner. Jönköpings tekniska högskola har en genomarbetad strategi för hur hållbar utveckling skall finnas med i utbildningarna. Varje utbildning skall integrera minst sex högskolepoäng.

    Sammantaget konstateras fortsatta förbättringsmöjligheter för Högskolan i Jönköping på strategisk nivå, programnivå, kursplanenivå och undervisningsnivå. Skillnader mellan bolagen, programmen och kurserna kan utnyttjas som en tillgång i förbättringsarbetet. En framgångsfaktor verkar vara att som JTH ha med hållbar utveckling i strategidokument med tydliga mål på omfattning samt att kursplaner innehåller moment som förklarar hur hållbar utveckling avhandlas. Vi rekommenderar också att tydliga mål sätts upp på varje fackhögskola och att lärare ges tid och resurser att ta del av de verktygslådor för hållbar utveckling som finns (en lista på sådana finns i rapporten). Ytterligare förslag på hur implementering kan stimuleras ges också, så som skapande av nätverk, införlivande av ämnet i högskolepedagogiska kurser samt högskolegemensamma temadagar för studenter.

    Objective

    The purpose was to explore self-reported psychosocial health and work environments among different dental occupations and workplaces from a salutogenic perspective. A further purpose was to analyse possible associations between three salutogenic measurements: The Sense of Coherence questionnaire (SOC), the Salutogenic Health Indicator Scale (SHIS) and the Work Experience Measurement Scale (WEMS).

    Methods

    Employees in the Public Dental Service in a Swedish county council (n = 486) were invited to respond to a self-reported web survey including demographics, work-related factors, the SOC, the SHIS and the WEMS.

    Results

    This study showed positive associations between employee characteristics and self-reported overall psychosocial health as well as experienced work environment. Autonomy was reported more among men than women (P < 0.000) and to a higher degree by dentists and dental hygienists than dental nurses (P < 0.000). Meaningfulness, happiness, job satisfaction, autonomy and positive to reorganization were reported by personnels aged less than 40 years (P ≤ 0.047). Clinical coordinators reported significant better health (SOC, SHIS) and experienced more autonomy, better management and more positive to reorganization than other dental professions. Dental hygienists and nurses experienced less time pressure than dentists (P ≤ 0.007). Better health and positive work experiences were also seen in smaller clinics (P ≤ 0.29).

    Conclusion

    Dental professionals reported a high degree of overall psychosocial health as well as a positive work experience. Some variations could be seen between employee characteristics such as gender, years in dental care, professionals, managing position and workplace size. Identify resources and processes at each workplace are important and should be included in the employee's/employers dialogue.

    Validity of the Danish version of Occupational Balance Questionnaire (OBQ-DK)2016Conference paper (Other academic)
    Abstract [en]

    Background:Occupational balance is a well-known concept within occupational therapy. The OBQ (Wagman and Håkansson, 2014) is developed to measure occupational balance. The OBQ is a 13-item questionnaire with 4 response categories and has been translated into Danish. The aim of this study was to investigate the construct validity of the OBQ-DK using the Rasch model.

    Method:Postal survey data to working aged participants (n=366; 69% women) were included. Their responses to the OBQ-DK were evaluated by Rasch analysis, partial credit model.

    Results: Initially, Rasch analysis failed to support the unidimensional structure of the 13-item OBQ-DK (Chi-Square 104.5 (df 52; p <0.001). All items had ordered threshold. Differential Item Functioning was absent across contextual factors (sex, age, education). Adjustment of local item dependency between two items and removal of four misfitting items resulted in perfect fit to the Rasch model (Chi-Square 36.85 (df 32; p = 0.254). Reliability was high (PSI = 0.82). The four deleted misfitting items formed an additional unidimensional scale (Chi-Square 19.35 (df 16; p = 0.251). Reliability was acceptable (PSI = 0.78).

    Conclusion: The study provides evidence for OBQ-DK’s construct validity. The Rasch analysis produced a 2-dimensional, 13-item questionnaire assessing the persons’ perception of occupational balance. Further research is required to establish the measures’ validity in various populations.Application to Practice:The results and small number of items makes it feasible to implement the OBQ-DK into clinical and/or research settings, where measuring occupational balance is needed.

    Aim

    The aim of the literature review was to describe how adulthood transition is used in health and welfare.

    Design

    A qualitative design with a deductive approach were used.

    Methods

    As material, 283 articles published in scientific journals, between 2011–August 2013, were selected. The search was conducted August 2013. The data were analysed and sorted in a categorization matrix.

    Results

    Transition was identified as a process mainly related to the four types previously identified; developmental, situational, health-illness and organizational transitions. Another one transition was also identified, lifestyle transition.

    Education for sustainability: Transformative processes, actions and systemic change in a Swedish university2016Conference paper (Refereed)
    Abstract [en]

    Research on the development of education for sustainability places attention to transformative processes and systemic changes at the university. Literature mainly studies processes to develop courses, programs and management systems but seldom investigates transformative processes and actions triggering organizational change. The objective of this paper is to identify key actions and transformative processes embodying an education for sustainability and to explore the systemic change outcomes, using the perspective of social learning. We relied on a participatory research method for the exploratory case study in a Swedish university. Our findings showed multiple key actions that resulted in five transformational processes. Together these transformative processes resulted in three systemic changes: organizational changes in sustainability perceptions, organizational changes in the working structures and educational curricula, and individual changes in perceptions and engagement on sustainability. A novel finding was that networking cross disciplinary facilitated double loop refection and worked as catalyst for other transformational processes. The result indicates that development of education for sustainability is facilitated by a combination of multiple transformative processes taking place with both top-down and bottom-up rationale. These processes generate individual and collective actions. Individual and network champions and top-managers play a key role in developing education for sustainability. In order to foster systemic change at the university, the existence of this complexity is crucial for developing education for sustainability.

    Introduction: While current debates on higher education are concerned with “what are we actually teaching in Education for sustainable development“ (Kopnina and Meijers 2014) or “how can we evaluate what we teach“, there is a lack of research addressing how universities are enabling change and developing new higher education approaches. Dominant educational structures across the world are based on fragmentation rather than connections and synergy to achieve a holistic approaches to education for sustainability (Wals, 2009). Education for sustainability calls for new kinds of collective learning that are centered on a transmissive nature (i.e. learning as reproduction) but rather on a transformative nature (i.e. learning as change). Embodying an active care for sustainability on higher education implies having an education for sustainability that includes socialization for democratic skills and values, and the development of a personal- and collective sense of competence (Chawla and Flanders Cushing, 2007).

    Objectives: The aim of this paper is to explore how a university create approaches on education for sustainability by engaging – or not its faculty and students, and how such approaches support or not authentic sustainable citizenship. To fulfil this purpose, we rely on theory of situated learning (Lave and Wenger, 1991) and communities of practices (Wenger, 2011) to move beyond static approaches to more dynamic approaches on education for sustainability.

    Methods: We conducted a case study (Eisenhardt, 1989; Miles and Huberman, 1984; Yin 1984) to investigate how approaches to education for sustainability were changed and built in Jönkoping University. As the study progressed, there was a mixture of inductive and deductive research strategies. This means that theoretical development occurred by combining the perspectives and observation of actors with existing literature. We interviewed strategically chosen persons and analyzed official documents.

    Results: The study revealed the processes for stimulating education for sustainability in a university that allowed each school to define freely its approach to sustainability. As a result, communities of practices stimulating situated learning were originated at the university. There were a myriad of ‘working’ approaches and degrees of prioritization on education for sustainability. Such approaches emerged following different rationales. In a bottom-up rationale, institutional entrepreneurs/faculty members and students created smaller communities of practices linked to their courses and activities. In a top-down rationale a school included principles promoting sustainability. Finally, a mix rationale moved in-between those approaches.

    Conclusion: We propose that considering those rationales in a single university promotes communities of practice as well as authentic sustainable citizenship.

    Objective: By 2023, fewer dentists are expected in Sweden, at the same time as the demand for dental care is expected to increase. Older people, in particular, are expected to require more dental health than previous generations. To meet this demand, the public sector dentistry in Sweden is moving towards changes in division of labour among dental professionals, including dentists, dental hygienists and dental nurses. However, the impact of this reallocation on the physical and psychosocial wellbeing of employees is unknown. The aim of this study was to compare workplaces with an equal or larger proportion of dental hygienists than dentists (HDH) with workplaces with a larger proportion of dentists than dental hygienists (HD) on the physical and psychosocial work load, musculoskeletal and psychosomatic disorders and sickness presence.

    Material: A total of 298 persons employed in the Public Dental Service in a Swedish County Council participated in this study.

    Conclusion: The medium large clinics HDH reported 85% of employee’s with considerably more high psychosocial demands compared to employees in medium HD (53%) and large HD (57%). Employees in medium large clinics HDH also reported sleep problems due to work (25%) compared with employees in medium large clinics HD (6%), large clinics HD (11%) and small clinics HDH (3%). Clinic size does not seem to influence the outcome of the HD and HD clinics to any great extent. Of all employees, about 94–100% reported high precision demands and 78–91% poor work postures.

    Background:

    The research conducted in palliative care is often medically oriented. There are few studies clarifying the patient's preferences, priorities and desires in palliative care. The occupational therapy research conducted mostly concerns occupational therapy interventions based on the profession's experiences. Further knowledge is needed regarding what patients in palliative care want to prioritize.

    Aim:

    The aim was to describe what patients in palliative care describe as important at the end of life.

    Methods:

    A scoping review was conducted using the inclusion criteria: articles published in 2004-August 2015; written in English; participants>18 years with life-threatening illness without possible cure; focusing on the patient?s experience.

    Results:

    Seventeen articles were included in the review and they were based on interviews. The theme 'Continuing occupational participation is important for people at the end of life' was identified. This included five sub-themes: maintaining previous occupational patterns; feeling needed; being involved in the social environment; leaving a legacy; and living as long as you live.

    Conclusions:

    The results show that continued occupational participation is important for people in palliative care. Occupational therapists can contribute to this by taking a highly person-centered approach and gaining information about what matters most for their clients.

    Bakgrund: De pågående klimatförändringarna som är relaterade till människors aktiviteter i vardagen, innebär en mycket allvarlig hälsorisk. Arbetsterapins och aktivitetsvetenskapens expertområde är vardagliga aktiviteter, aktivitetsmönster och aktivitetsförändringar. Den kunskap som detta aktivitetsfokus har genererat kan också bidra till den viktiga frågan om ekologisk hållbarhet ur ett aktivitetsperspektiv.

    Syfte: Att utforska och beskriva vad arbetsterapeuter skrivit om att bidra till ekologisk hållbarhet och förhindra ytterligare klimatförändring.

    Metod: Studien är en litteraturöversikt. Publikationer åren 2008-2013 användes som data. Sökningar skedde på fyra olika sätt: i databaser, referenslistor, citeringar samt manuellt i tretton tidsskrifter.

    Resultat/preliminärt resultat: Fjorton publikationer inkluderades i litteraturöversikten: nio artiklar, en bok och fyra bokkapitel. I resultatet identifierades fyra olika sätt att bidra till ekologisk hållbarhet – genom att anpassa aktiviteter, samarbeta med andra på olika nivåer, utforska människors aktiviteter samt genom att varna för konsekvenser av klimatförändringarna ur ett aktivitetsperspektiv.

    Slutsats: De identifierade sätten att bidra till ekologisk hållbarhet kan användas som en utgångspunkt för vidare studier och interventioner.

    Aim: The overall aim of the present thesis was to conceptualize life balance from an empirical and occupational therapy perspective in order to contribute to knowledge regarding the use of the concept in occupational therapy. The specific aims were to:

    • explore the perceptions of life balance among working people who had not recently been on long-term sick leave
    • explore what is considered important for life balance and whether or not this differs between people
    • use perceptions of life balance data from men and women in a healthy population to validate the Model of Lifestyle Balance
    • clarify the content in the concept of occupational balance as used within published articles related to occupational therapy

    Methods: In Study I, 19 participants were interviewed about their perceptions of lifebalance. In Study II, 32 participants sorted statements about life balance according to their importance. In Study III, data from Study I were reanalysed in relation to Matuska and Christiansen’s Model of Lifestyle Balance. In Study IV a concept analysis of occupational balance in occupational therapy was conducted, encompassing 43 articles.

    Results: Life balance was shown to be a subjective, dynamic, health-related and multidimensional concept. Similarities and differences between what the participants consider to be important for their life balance were revealed. Occupational balance was, in the concept analysis, shown to be a subjectively defined perception of having the right amount and variation of occupations.

    Conclusions: The results of the thesis indicate that life balance and occupational balance are two separate concepts. While life balance includes occupational balance, it goes beyond it and also includes other aspects. Future research is requried to investigate the generality of the findings.

    Abstract [en]

    Objective: Climate change is of great concern today, and is an important health issue. The changes are related to human occupations and, therefore, an occupational perspective can make important contributions to ecological sustainability. The aim of this scoping literature review was to explore and describe what has recently been written about how occupational therapy/therapists/science can contribute to ecological sustainability and the prevention of more severe climate change.

    Methods: Publications were used as data. Searches were conducted in databases, journals, reference lists, and citations. Fourteen items, with content related to the contribution of occupational therapy/therapists/science to climate change and ecological sustainability; written in English; having an occupational therapist among the authors; and published between 2008 and 2013; were included.

    Results: The results revealed four different ways that occupational therapists/scientists can contribute: by adapting to prevent climate change and to existing changes; by cooperating with others; by exploring people's occupational choices and the relationship between occupation and ecology; and by warning of the consequences of the changes.

    Conclusion: These ways of contributing can be used as a point of departure for further research on this important topic.

    There is general agreement that global climate changes are one of the greatest threats to humanity. Therefore, occupational therapists should use their knowledge about occupations to contribute to the ecological aspect of sustainable development. Similarly, when working with clients, therapists' occupational perspective can expand to include an ecological dimension. A conceptual framework would be valuable and, while empirical research is required, it would appear that the Model of Human Occupation (MOHO) is useful in this context.

    Abstract [en]

    Occupational balance is a frequently used concept in occupational therapy, but it is complex and differences in content exist. Further knowledge would be valuable for scholars, practitioners, and measurement development. Concept analysis is a way to clarify concepts. The present concept analysis used Walker and Avant's procedure for analysing the concept of occupational balance, with 43 articles included in the analysis. The results showed that occupational balance can be defined as the individual's perception of having the right amount of occupations and the right variation between occupations. Three perspectives of occupational balance were identified: in relation to occupational areas, in relation to occupations with different characteristics, and in relation to time use. The knowledge gained may guide the use of the concept, and some suggestions are made for its use and for further research.

    Stressrelaterad ohälsa ett stort problem som leder till enskilt lidande och stora kostnader. Stress kan ses som obalans mellan en individs krav och resurser. Tankar kring balans mellan olika delar av tillvaron har lång historia och aktualitet inom arbetsterapi. I FSA’s etiska kod är det ett filosofiskt grundantagande att ”Människans hälsa kräver en balans mellan aktivitet och vila”. ’Balans’ har tidigare undersökts ur olika perspektiv och  i olika populationer. Ingen studie har dock identifierats när det gäller en mer generell population. Studiens syfte är undersöka vad personer i yrkesverksam ålder, utan sjukskrivning >1månad i sträck inom två år, anser att ’balans i den dagliga tillvaron’ är och vilka faktorer som ses som mer eller mindre viktiga för att uppleva balans. För att söka svar på frågan vad ’balans i den dagliga tillvaron’ är genomförs kvalitativa intervjuer som analyseras enligt Grounded Theory (GT).  Deltagarna rekryteras via teoretiskt urval och datainsamling planeras pågå tills mättnad erhålls. De faktorer som framkommer i GT-analysen formuleras som påstående vilka undersöks genom Q-metodik. Deltagarna tar ställning till dessa påståenden genom viktning och svaren förs in i form av en normalfördelningskurva och analyseras statistiskt. Vissa intervjudeltagare ingår också i denna del av studien tillsammans med nyrekrytering via tillgänglighetsurval. Studien är pågående och förhoppningen är att ökad kunskap, om ’balans i den dagliga tillvaron’ kan bidra till att förbättra prevention för dem som upplever obalans i sin tillvaro och kanske riskerar sjukskrivning. Den ökade kunskapen kan också leda till mer anpassade åtgärder och rehabilitering av dem som drabbats av t.ex. stressrelaterad ohälsa.

    A life in balance is commonly related to health and well-being. However, our knowledge regarding the perceptions of life balance among the general population is limited. Our aim was to explore the perception of life balance among working people without recent long-term sick leave. Individual interviews were conducted with 7 men and 12 women and analyzed according to grounded theory, aiming at achieving a description. The results showed that life balance includes four interrelated dimensions: activity balance, balance in body and mind, balance in relation to others, and time balance. Life balance was regarded by the participants as health related, promoted by a sense of security, and affected by context and individual strategies. Life balance was also seen as being individually defined and dynamic. The results indicate that the perceptions of life balance might be quite general because they show similarities with previous research on different populations.

    Poor citizens from European Union (EU) member countries begging to support themselves are now common in affluent EU countries. Their lack of basic amenities, such as access to a shelter and sanitation is not in line with human rights and also implies a risk for health issues. Despite this, we know little about how these vulnerable EU citizens, themselves, perceive their health situation. The aim of this study was to explore vulnerable EU citizens’ descriptions of their health. Eight females and 12 males from Romania, 19–64 years of age, participated in individual interviews that included health issues. Qualitative content analysis was used, identifying the main category “Begging abroad and health - for better and for worse” together with two categories and five subcategories. It is concluded that EU citizens begging abroad risk poorer health as a consequence of their attempts to improve their situation, both their own health and that of their relatives. Therefore, they need access to affordable health care as this may decrease the need for unwanted travelling abroad to beg.

    Aim: The aim of this study was to describe the relationship between occupational balance (measured by the occupational balance questionnaire [OBQ]) and self-rated health and life satisfaction. A secondary aim was to explore differences in occupational balance among adults in Sweden. Methods: The 153 participants (63% women), recruited using convenience sampling, answered a questionnaire comprising demographic questions, the OBQ, one item about self-rated health, and one about life satisfaction. The OBQ was analysed for correlation with subjective health and life satisfaction. The OBQ and its individual items were also analysed for correlations with age and for differences between men and women and participants living with children younger than 18 years versus not. Results: The OBQ was significantly positively correlated to self-rated health and life satisfaction, supporting the relationship between occupational balance and health. No significant correlation between age and the total OBQ was identified but some differences in occupational balance, related to whether the participant was living with or without children at home, were revealed. Conclusions: The results should be interpreted with caution due to the nature of the sample and data but they can serve as a point of departure for further studies and hypotheses regarding occupational balance in different populations.

    Objective: The concept of occupational balance is frequently used in occupational therapy but the fact that it has been defined and measured differently is a limitation. This article introduces the Occupational Balance Questionnaire (OBQ), which focuses on satisfaction with the amount and variation of occupations. It consists of 13 items measured on six-step ordinal scales. It has shown good content validity in a sample of 21 occupational therapists but other psychometric properties have not been investigated. The aim was to investigate the OBQ regarding internal consistency, test–retest reliability, and floor/ceiling effects. Methods: The OBQ was administered twice to a sample selected through convenience sampling. Internal consistency was investigated by Cronbach's alpha and test–retest reliability analysed with Spearman's Rho correlation for the total score and weighted kappa on each item. Potential floor/ceiling effects were explored by checking for the percentage of participants who scored lowest and highest. Results: The results demonstrated that the OBQ has good internal consistency (Cronbach's alpha 0.936) and sufficient test–retest reliability (Spearman's Rho for the total score was 0.926) and, thus, seems stable over time. No floor or ceiling effect was detected. Conclusions: The OBQ therefore showed promising reliability, although further instrument development studies to examine its construct validity are required.

    Bakgrund: Aktivitetsbalans är ett viktigt hälsorelaterat begrepp inom arbetsterapi. Det finns behov av studier där aktivitetsbalans mäts på samma sätt i olika populationer för att generera fördjupad kunskap om hur aktivitetsbalansen ser ut vid olika diagnoser och hur den skiljer sig från personer i generell befolkning. I studien används Occupational Balance Questionnaire (OBQ), baserad på definitionen att aktivitetsbalans är en persons upplevelse av att ha rätt mängd aktivitet och rätt variation mellan olika aktiviteter i sin vardag.

    Syfte: Syftet är att beskriva aktivitetsbalans hos personer i yrkesverksam ålder med gemensamma diagnoser (reumatoid artrit och ångest/depression) och hos personer från en generell population. Eventuella skillnader mellan män och kvinnor undersöks också. Ett ytterligare syfte är att kontrollera OBQs psykometriska egenskaper i grupperna med gemensam diagnos.

    Metod: Studien är pågående och data samlas in via frågeformulär. Deltagare är 50 personer med ångest eller depression (resultat från fler planeras till presentationen), 383 personer med reumatoid artrit och 217 personer från en generell population. Samtliga deltagare var i åldern 20-65 år. Aktivitetsbalans mättes OBQ vilket består av 13 påståenden som skattas på ordinalskala. Svaren summeras till en totalskala (0-65) och analyseras också avseende varje påstående. OBQ har visat god innehållsvaliditet. Det har också haft god intern konsistens och test-retestreliabilitet bland deltagare från generell population. Icke-parametrisk statistik används vid analys.

    Resultat/preliminärt resultat: Det preliminära resultatet visade att deltagarna med ångest/depression skattar lägre aktivitetsbalans (median 24) än de med reumatoid artrit (median 43) och deltagarna från en generell population (median 42). Samma resultat sågs i de enskilda påståendena. OBQ visade också god intern konsistens i samtliga grupper (Cronbachs alpha 0,874-0,947).

    Slutsats: Studien bidrar med kunskap om aktivitetsbalans i olika grupper.

    Bakgrund: Forskning har visat livsbalans som ett subjektivt, mångdimensionellt och hälsorelaterat begrepp. Ingen studie har dock undersökt vad människor ser som mer eller mindre viktigt för att uppleva balans i sitt liv eller om detta skiljer sig åt mellan människor.

    Syfte: Syftet är att utforska vad som anses viktigt för livsbalans och om detta skiljer sig åt mellan människor.

    Metod: Q metodologi användes och deltagarna var sexton män och sexton kvinnor. Samtliga var yrkesverksamma och i åldern 21-67 år. Varje deltagare sorterade 42 påståenden om livsbalans på en mall i enlighet med sin uppfattning om hur viktigt påståendet var för deras balans i livet. Deltagarnas mallar analyserades statistiskt, med faktoranalys och varimaxrotation, för att generera olika synsätt/faktorer.

    Resultat/preliminärt resultat: Både likheter och skillnader mellan deltagarna identifierades. Fyra olika synsätt framkom, dvs. liknande mönster av uppfattningar mellan några deltagare. I samtliga sågs dock goda relationer till de närmaste som mycket viktigt. Andra likheter identifierades också mellan vissa av synsätten och deltagarna i tre av dem ansåg det, till exempel viktigt att känna sig frisk och kry. Därutöver hade varje synsätt sitt unika perspektiv på vad som ansågs viktigt för livsbalans och de namngavs efter respektive perspektiv: ”aktivitetsbalans”, ”självförverkligande”, ”medvetenhet om egna behov” och ”ömsesidiga relationer”.

    Slutsats: Resultatet stöder att livsbalans är subjektivt, mångdimensionellt och hälsorelaterat även om det inte kan generaliseras. Det visar också på betydelsen av klientcentrering då det kan finnas skillnader mellan vad klienter ser som mer eller mindre betydelsefullt för sin livsbalans.

    Life balance seems subjective, health related, and multidimensional. However, the concept is complex. Exploring what people themselves consider more or less important for their life balance and whether this differs between people would develop new knowledge. Q methodology was chosen for the present study, in which 32 working men and women without recent long-term sick leave participated. They sorted 42 statements regarding life balance according to their importance for each participant's life balance. The analysis resulted in four different viewpoints concerning life balance. All four viewpoints considered good relationships with those closest to them, as well as knowing that these people were doing well, as important. Each viewpoint also showed a unique orientation towards what was considered important for life balance: occupational balance (viewpoint 1), self-actualization (viewpoint 2), self-awareness (viewpoint 3), and reciprocal relationships (viewpoint 4). The results. showed support for life balance as being a subjective, multidimensional, and health-related phenomenon. The results demonstrated the importance of relationships for life balance and heterogeneity in what people considered important for their own life balance.

    This article reports a comprehensive review of the research conducted regarding occupational balance. A scoping study method was used to explore and describe current research about occupational balance and to identify research gaps. Twenty-two articles published between 2009 and 2014 met the inclusion criteria. The articles reported studies conducted in eight countries on four continents, but the majority were conducted in Europe and North America. The articles contributed to knowledge about the concept itself, its importance, levels of occupational balance, and the relationship between occupational balance and health and well-being. They also described what is important for occupational balance or how to enhance/create/recreate it and differences between people. Several research gaps were identified which include the need for studies about perceptions of occupational balance among people beyond western societies. Furthermore, systematic research is warranted with regards to levels of occupational balance, and how to enhance it.
