Abstract [sv]

Bakgrund: Vård av barn i hemmet ökar i Sverige, men står totalt sett för en låg andel av de som vårdas i hemsjukvård. Det finns en strävan att barn i största möjliga utsträckning ska vårdas i sin hemmiljö. I en familjefokuserad omvårdnad behöver distriktssköterskan etablera en god vårdrelation och det krävs att både barnet och familjen görs delaktiga i vården.

Syfte: Syftet var att beskriva distriktssköterskors erfarenheter av att vårda barn i kommunal hemsjukvård. 

Metod: Intervjuer med tolv distriktssköterskor och två sjuksköterskor genomfördes. Materialet analyserades med kvalitativ innehållsanalys.

Resultat: Relationen med barn och familj samt barnanpassad vård var betydelsefulla komponenter i distriktssköterskornas omvårdnadsarbete. Att visa hänsyn i familjens hem var viktigt. Samarbete i egen verksamhet och med barnklinik var nödvändigt. Distriktsköterskorna hade behov av stöd för att hantera svåra känslor. Det saknades kunskap och rutin samt organisatoriska förutsättningar i arbetet för att kunna utföra säker vård. Arbetsmiljön i hemmet var inte alltid optimal.

Slutsats: Vård av barn var resurskrävande i både tid och bemanning. För att kunna utföra en familjefokuserad och barnanpassad omvårdnad fanns behov av förändring i organisationens utformning. Det fanns också ett behov av utbildning om det sjuka barnet samt kunskap om hur familjefokuserad omvårdnad tillämpades i praktiken. 

Abstract [en]

Background: In Sweden, home nursing of children is increasing, but overall, it stands for a small percentage of patient with homecare service. Efforts are made to provide home nursing to a greater extend for children. When providing family nursing, the district nurse needs to establish a good relationship with both the child and the family to make them involved. 

Purpose: The purpose was to describe the district nurses´ experiences of caring for children in homecare service.

Method: Interviews were conducted with twelve district nurses and two general nurses. The material was analysed with qualitative content analysis.

Result: In nursing, two important components were the relationship with the children and their families and adapting the care to the child´s needs. Showing consideration in the family home was important. Co-operation through own activity and with the paediatric clinic was necessary. There was a need of support for the district nurses ´ to handle difficult emotions. Knowledge, routine, and organizational conditions were deficient to enable the provision of safe care. The condition of the work environment was not always satisfactory.

Conclusion: Large resources in time and staffing were required when carried for children. In order to be able to conduct family nursing with a child-friendly care there was a need of structural change in the organization.  There was also a need for education about the sick child and knowledge about how family nursing was applied in practice.
