Abstract [sv]

Suicid klassas i dag i Sverige som ett folkhälsoproblem och förebyggande insatser görs för målgrupperna med störst riskfaktorer som exempelvis psykisk sjukdom (framförallt depression) och missbruk av alkohol och droger. Syftet med denna studie är att undersöka hur missbruksenheter inom socialtjänsten arbetar med suicidprevention samt undersöka hur samarbetet med andra aktörer kring det suicidpreventiva arbetet beskrivs. Åtta socialsekreterare inom missbruksenheter har genom kvalitativa intervjuer delat med sig av sina erfarenheter och uppfattningar. Genom den tematiska analysen framkom tre teman: suicidprevention i det vardagliga arbetet, utbildning och erfarenhet och samarbete med andra aktörer. Resultatet har tolkats med hjälp av suicidprocessen och tidigare forskning inom området.

Socialsekreterarna visar genom sina utsagor att de först och främst arbetar suicidpreventivt genom samtal med klienterna om deras psykiska mående där eventuellt åtgärder som exempelvis kontakt med psykiatrin tas. Som hjälpmedel i det suicidpreventiva arbetet i studien används framförallt mätinstrumentet Addiction Severity Index (ASI) för att öppna upp för samtalet om klientens psykiska hälsa. De socialsekreterarna som gått suicidpreventiv utbildning beskrev att de fått både ökat självförtroende och handlingsberedskap att agera när de träffade någon med misstänkt psykiska problem. Erfarenheterna av att ha gått utbildningarna kan exempelvis vara att lättare kunna tolka suicidala tecken och kunna avgöra om individen befinner sig i suicidprocessen och vad som är lämplig adekvat åtgärd. Samarbetet som beskrivs i studien sker mestadels mellan socialtjänsten och psykiatrin i det suicidpreventiva arbetet. Samarbetet beskrivs som fungerande men största bristen beskrivs vara avsaknad av god kommunikation som en samarbetsgrund. Studien ger en bild över det suicidpreventiva arbetet som redan nu sker inom socialtjänstens missbruksenheter, och kan vara en del den fortsatta vidareutvecklingen inom området.

Abstract [en]

Suicide is currently classified in Sweden as a public health problem and preventive efforts are made for the target groups with the greatest risk factors such as mental illness (especially depression) and abuse of alcohol and drugs. The purpose of this study is to investigate how alcohol and substance abuse units in the social services work with suicide prevention and to investigate how the collaboration with other actors around the suicide prevention work is described. Eight social service workers within the alcohol and substance abuse units have shared their experiences and perceptions through qualitative interviews. The thematic analysis revealed three themes: suicide prevention in everyday work, education and experience and collaboration with other actors.  The results have been interpreted using the suicide process and previous research in the field.

The social service workers show through their statements that they first and foremost work on suicide prevention through conversations with clients about their mental state, where possible measures such as contact with psychiatry are taken. As an aid in the suicide prevention work in the study, the measuring instrument Addiction Severity Index (ASI) is primarily used to open up the conversation about the client's mental health. The social service workers who had undergone suicide prevention training described that they had both increased self-confidence and readiness to act when they met someone with a suspected mental problem. The experience of having completed the training may, for example, have increased the ability to interpret suicidal signs and be able to determine whether the individual is in the suicide process and what is an appropriate adequate measure. The collaboration described in the study takes place mostly between the social services and psychiatry in the suicide prevention work. The collaboration is described as functional, but the biggest shortcoming is described as the lack of good communication as a basis for collaboration. The study gives a picture of the suicide prevention work that is already taking place in alcohol and substance abuse units in social services and may be part of the further development in the area.
