Constraining acts within Elder care, a dark necessity? : The professionals reflections on the Elders right to self-determination (English)
Abstract [en]

The purpose of the study is to get the professional care-givers comprehension and awareness of constraining acts towards elder individuals within a nursing home. On what basis these acts are justified by the care-givers according to law. The professional’s approach towards the usages of constraining acts on an individual level but also in the role of the inter-professional team. This purpose is achieved through a qualitative research method with semi-structured interviews. The result from the interviews has been thematically analyzed and linked to Foucault’s ideas of normalization power and previous research. Our research shows that there is a complexity in the usage of constraining acts from professional care-givers in respect of the Elders autonomy. This complexity is based on that the acts within the constraining spectra doesn´t cover the Elders right to self-determination within the context. The results reveal that the care-givers within the inter-professional team use their professionalism to create alternate strategies to avoid or limit the usage of constraining acts within the nursing home.  In our analysis based on Foucault’s ideas on normalization and power the alternate strategies from caregivers can be viewed as a productive exercise of power. This because the ideas are based on the terms from the caregiver who is in control. Constraining acts are viewed by the professional care-givers as dark necessity in that sense that the acts lacked a legal basis and simultaneously were necessary to address the inevitable issue. The lack of legal basis causes the professionals to end up in ethical dilemmas where the results of the acts doesn´t always end up so well. In the usage of constraining acts the professional strives to act according to what is believed to be in the Elders best interest and to preserve the individual’s self-determination as far as possible.
