Abstract [sv]

Den svenska offentliga äldreomsorgen växte starkt från 1950-talet, och nådde sin högsta nivå på 1980-talet, för att därefter minska under 2000-talet. Med hjälp av riksrepresentativa befolkningsundersökningar från 1954 och fram till 2009 studerar vi mönster i äldres hjälpbehov och insatser från familj och offentlig omsorg. Ansvarsförhållandet mellan stat och familj har skiftat över tid, men omsorg från anhöriga och det offentliga överlappar allt mer. De offentliga insatserna minskar samtidigt som allt fler på sikt får hjälp, men mindre och senare i livet. Allt fler äldre har anhöriga, som ger mer omsorg.

Abstract [en]

We estimate how much caregiving men and women respectively do, and how much of the caregiving is done by older (65+) and younger persons, inside their household and for other households, in Spain and in Sweden. To assess this, we use self-reported hours of caregiving from two national surveys about caregiving, performed in 2014 (Spain, N = 2003; Sweden, N = 1193). Spain and Sweden have dissimilar household structures, and different social services for older (65+) persons. Caregivers, on average, provide many more hours of care in Spain than in Sweden. Women provide about 58% of all hours of caregiving, in Spain in all age groups, in Sweden only among younger caregivers. The reason is the dominance of partner caregivers among older Swedes, with older men and women providing equal hours of care. Family caregiving inside the household is more extensive in the more complex Spanish households than in Swedish households. Family care between households prevails in Sweden, where the large majority of older persons live with a partner only, or alone. This is increasingly common in Spain, although it remains at a lower level. We estimate that older persons provide between 22% and 33% of all hours of caregiving in Spain, and between 41% and 49% in Sweden. Patterns of caregiving appear to be determined mainly by demography and household structure.

Ordet fattigvård får många att erinra sig, kanske med en rysning, Astrid Lindgrens kommandora och Albert Engströms skildring av Fattighuset. Den moderna välfärden och äldreomsorgen ser vi ofta som ett totalt brott med det förflutna, däribland fattigvården.

I denna skildring av fattigvården i Huddinge, först i socknens regi och sedan i kommunens, visas hur fattigvården gradvis gick över i dagens offentliga omsorg. Då som nu utgör de äldre en stor grupp i fattigvården-omsorgen och vi möter många människoöden, varav inte alla stillatigande och tacksamt fann sig i sin lott.

Den ”moderna” offentliga omsorgen bevarar många drag av det förflutna och bygger på den grund som fattigvården lade redan för flera hundra år sedan. Detta arv av lokala lösningar med kollektiv finansiering och beslut i ordnade former hade inslag man kan stanna upp och begrunda, men har samtidigt visat sig funktionsdugligt och är kanske rentav något att vara litet stolt över. Även den senaste tidens omsorgsmönster belyses i skriften.

Den offentliga omsorgen avspeglar helt enkelt livsvillkoren i stort, då som nu. De flesta svenskar var fattiga och fattigvård-omsorg blir därefter. Perspektivet sträcker sig från 1700-talet fram till 2017 för Huddinge socken-kommun. Denna representerar ganska väl utvecklingen i stort i Sverige, men livsöden och offentlig administration blir gripbara saker först när man ser dem i deras lokala sammanhang.

Abstract [en]

Usually it is women who take care of family members in the home, but with age, gender differences become less pronounced and, from 80 years onwards, there are more men caring for a family member – generally their partner – than women. Social and demographic changes are presenting new challenges for public services. In particular, in two-person households with elderly inhabitants, one of whom is dependent, it is necessary to tackle not only the needs of the dependent partner but also those of the carer partner. For this reason, carer support programmes are needed.

의지할 데 없는 사람들을 돌보는 것은 스웨덴에서는 수백 년 동안 지역사회와 공공의 책임이었다. 1956년 이전 에는 주로 빈곤계층과 병약한 사람들을 돌보는 것이 전부였지만 그 이후부터는 모든 취약계층으로 돌봄이 확대 되었다. 시설보호보다는 돌봄 서비스를 지향했으나 돌봄 서비스가 정착되기까지는 시간이 많이 걸렸다. 1956 년에는 65세 이상 인구의 6%가, 1975년에는 9%가 시설에서 보호를 받았으며 당시 새롭게 도입된 재가돌봄서 비스는 서비스 대상으로 65세 이상 인구의 16%를 목표로 삼았다. 재가돌봄서비스는 이후 다양화되었고 제한 적인 재정 때문에 줄어들었다. 그러나 조사 결과 가족 간의 유대가 강화되고 가족들의 지원, 도움, 돌봄이 늘어나 면서 이러한 긴축으로 인해 사람들의 불행이 증가하지는 않은 것으로 나타났다. 이전 세대의 노인보다 파트너와 함께 거주하는 노인들이 증가했고 이전 세대의 노인보다 자녀를 둔 노인들의 수도 늘어났다. 가족의 돌봄은 공 공 서비스와 중첩될 때가 많은데, 대부분의 스웨덴 사람들은 공공 서비스를 생애 말기에 이용하며 이전보다 사 용 기간이나 횟수가 줄어들었다.
